SUBJECT: REPORT NOS. 50-321/85-18 AND 50-366/85-18 This refers to your response of August 23, 1985, to our Notice of Violation issued on July 22, 1985, concerning mispositioned nitrogen makeup valves to the main steam isolation valves' pneumatic control system, which resulted in a reactor scram.
We have evaluated your response and found that it meets the requirements of 10 CFR 2.201.
In regard to the assignment of Severity Level, we still consider that the cited violation has been properly characterized as a Severity Level IV, since such unnecessary challenges to safety systems are of more than a minor concern.
Additfor. ally, an apparent problem in maintaining control over a system or evolution which affects the reactor plant, such as indicated by this event, could lead to a more serious concern if left uncorrected..
We note that certain features of your operation have been subsequently strength-ened to correct apparent inadequacies; the nitrogen makeup valves' control switches in the main control room were relabeled to emphasize the effect of shutting them, and plant policy concerning valve control and manipulation has been restated.
This should help strengthen the control room operators' aware-ness of the seriousness of a lack of control over positioning valves, especially those valves whose positions are indicated in and controlled from the control room.
Furthermore, the steps you are taking to clarify the permissible methods for shifting the drywell pnr.umatic supply by establishing a proceduralized set of instructions for swapping the pneumatic medium from nitrogen to plant air, should provide the needed control of such evolutions that was previously lacking.
Accordingly, we will examine the implementation of your corrective actions during future insaections.
