Licensee Action on Previously Identified Items (Closed) Unresolved Item (295/78-21-01): The control for hydraulic snubber fluid, and the compatibility of fluid dyes with snubber seals and fuild were considered to be questionable. A followup inspection was performed in March 1979 (Region III Inspection Report Nos. 50-295/79-02; 50-304/79-02).
The procurement and use of GE SF-1154 snubber fluid was reviewed by the inspector and no items of nonconformance were found. During this visit, the inspector discussed the compatibility of the dye with the snubber fluid and the seal problems with the licensee reprcsentatives.
It was stated that all safety related snubbers were filled with QC purchased fluid with Calco-Blue dye added to improving visibility. The inspector reviewed the licensee's snubber surveillance program for Unit 2 conducted during the period from May 15, 1980 to June 30, 1980, and the extensive seal replace-ment program for all safety related hydraulic snubbers, and considered the licensee measures to be adequate.
(Closed) Unresolved Item (295/78-21-03): Snubber acceptance lock up velocity and bleed rate did not take into consideration the temperature effect, and that the machine sensitivity and capacity could not match the acceptance range, i.e., 36 IPM machine maximum versus 40 IPM maximum acceptable. Sinc then, the licensee has procured one additional test machine with a maximum capacity of 100 IPM. The acceptance lock up velocity and bleed rate were increased to 100 IPM and 25 IPM respectively.
(Closed) Unresolved Item (295/78-21-04):
Snubber VC 48-S3 was welded to a leak chase channel and two other snubbers were observed with the fluid reservoirs mounted on the side instead of on the top.
In review of CY 1980 snubber inspection record, it was identified that checking the reservoir positions was part of the overall inspection program.

review of the licensee internal documentation, " Progress Report on Unresolved Items Relating to Safety-Related Snubbers," dated February 8, 1979, it was documented that, " IVC 48-53 has been relocated by Hunter Corporation according to specifications."
(Closed) Unresolved Item (295/78-21-07): There was rust on steam gener-ator B and D large hydraulic snubbers including the connecting piping system. During the period of November 1979 to May 1980, Bergen-Paterson steam generator snubbers were found gouging the piston rod at the gland seal surface locations. As a result of the damages identified, all B-P snubbers were repaired, upgraded, and tested. See Region III Inspection Reports 50-295/79-26; 50-304/79-25; 50-295/80-08; and 50-304/80-08 for detail.
(Closed) Unresolved Item (295/79-02-01; 304/79-02-01): Deficiencies identified in snubber inspection records.
In view of the licensee implementation of IEB 79-14 requirements and the recent updating of the snubber inspection and seal replacement program, this item is no longer considered to be an issue.
Functional or Program Areas Inspected
1.
Observation of Snubber Conditions The licensee is conducting an overhaul program to replace all seals for ITT-Grinnel (ITT-G) hydraulic snubbers. At present, 150 1.5" and 96 2" and 2.5" bore snubbers had been modified, tested, and re-installed. Another 314 snubbers in Unit 2 are scheduled to be over-hauled in the next one or two re-fueling outages. The inspector observed some of the snubbers at floor elevation 568'-0" and some of the snubbers not yet overhauled outside the missile barrier at various floor elevations, and had one comment. The 2 VC 279-S7, a 1.5" bore snubber, was installed on EL 568'-0" outside the missile barrier and had not yet received seal replacement. The reservoir unit was observed installed on the side of the cylinder in lieu or on the top.
Similar problems were observed and documented in IE:
Region III Inspection Report No. 50-295/78-21, Paragraph 5.
The licensee committed to test and change the reservoir position for this snubber and to conduct inspections to check for similar pro-blems. This is an unresolved item (304/81-24-01).
2.
Review of Snubber Surveillance Program The inspector reviewed the licensee TSS 15.6.48, "Grinnell Snubber Surveillance," Revision 4, dated May 21, 1981, and had two comments:

A letter from the Director, Division of Licensing, NRC-NRR, to all power reactor licenstes (except SEP licensees), subject:
Technical Specification Revisions for Snubber Surveillance, dated November 20, 1980, was received by CECO on December 4, 1980. The letter enclosed a copy of Revision 1 of the Inservice l-3-

Surveillance Requirements for snubbers under the STSs which upgraded the previous snubber surveillance requirements for mechanical snubbers. The letter requested CECO to submit a licensee amendment application to incorporate the applicable portion of these model TSs into Zion Appendix "A" TS within the next 120 days.
In discussion with the site technical staff, the inspector was informed that the site revision of the TS, " Technical Specification Change 81-6 concerning Hydraulic and Mechanical Snubbers" was completed on June 22, 1981, and the package was forwarded to CECO headquarters on June 23, 1981 for off-site review and submitted to CECO Nuclear Licensing Department for transmittal to the NRC. To the date of this inspection, no license amendment incorporating the up-graded snubber surveillance program has been forwarded to NRC-NRR. This is considered to be an unresolved item (304/81-24-02).

In review of Table 3, " Acceptance Criteria for Declaring Snubber Operable," the maximum locking velocity (LV) was increased from 40 IPM to 100 IPM. The inspector questioned this high acceptance LV.
During the inspection, a copy of S&L design report EMD File No. 020115 " Upper Limit Lock-Up Velocity Study for Hydraulic Snubbers for Zion Station Units 1 and 2," Revision 00, dated October 22, 1979, was presented for the inspector's review and evaluation. The S&L report was formally transmitted to CECO on December 12, 1979. Subsequent to the review, the inspector stated that he concurred with the S&L technical consideration and the analytical methology, and had no further questions.
3.
Review of Records The inspector reviewed the results of Unit 2 ITT-G snubber surveil-lance (TSS 15.6.48) based on TS 3.22 and 4.22 requirements. The surveillances were conducted from May 15, 1980 to June 30, 1980, and the results tabulated and reported on July 7, 1980.
Subsequent to the review, the inspector stated that he had no adverse comment.
4.
Observation of B-P Snubber Test Machine A B-P Series 25000 Test System was installed at Zion Station in August 1981 in addition to the existing ITT-G test machine. The new B-P machine can perform LV and BR tests for hydraulic snubbers up to 130 kips load capacity and with a maximum acceptable LV of 100 j
IPM. Machine adaptation for mechanical snubber testing in terms of lockup acceleration was a part of the optional installation designed into the test module system.
The inspector observed the various system operational modes and reviewed the technical manual and had no adverse comments.

Unresolved Items Unresolved items are matters about which more information is required in order to ascertain whether they are acceptable items, items of noncompliance or deviations. The unresolved items disclosed during this inspection are discussed in Paragraphs 1 and 2.a.
Exit Interview The inspector met with licensee representatives at the conclusion of the inspection on October 30, 1981. The inspector summarized the scope and findings of the inspection. The licensee acknowledged the findings reported herein.
