STATEMENT OF VIOLATION Inadequate Access Control

. Technical Specification 6.3.2.B requires that security procedures shall be established and implemente Plant Services Procedure 1.1, " Station Security," Revision 9 dated October 3, 1988,Section V.D.3.b requires that, the Security Department temporarily inactivate an individual's badge when retraining requirements have not been accomplishe Contrary to the above, on October 23 and 24, 1988, the licensee did not temporarily inactivate the badge of an individual whose retraining requirements had not been accomplished, and the individual was allowed unescorted access to protected and vital area This is a Severity Level IV violatio (Supplement III)(298/8831-01)

REASON FOR VIOLATION The Nuclear Training Department is responsible for notifying CNS management of overdue training requirements. This notification is developed from training l records maintained by the Nuclear Training Department. Uncertainty concerning the validity of computer generated reports from the training records tracking system resulted in a delay in notification.

STEPS TAKEN AND RESULTS ACHIEVED Validation of computer data was performed against "hard copy" files.. A comprehensive report, identifying individuals overdue for retraining requirements, was developed and forwarded to CNS managemen Where appropriate, unescorted access to CNS was temporarily inactivated for certain individuals and arrangements made for retrainin In addition, a Nuclear Training Guideline has been written to provide direction for development and promulgation of a monthly report notifying nuclear division managers of overdue security training requirement CNS Security receives a copy of this repor CORRECTIVE STEPS WHICH WILL BE TAKEN TO AVOID FURTHER VIOLATIONS A thorough upgrade of the computerized training records tracking system . is planned to commence during the first quarter 1989. This upgrad,e will result in the development of record keeping procedures, appropriate training of records personnel, revalidation of existing records, and improved computtr data entry and reporting capabilities. This effort is expected to take six month DATE WHEN FULL COMPL1ANCE WILL BE ACHIEVED The District is currently in full compliance with station security procedure
