Licensee Action on Previous Inspection Findings (0 pen) Finding (320/79-23-09) Update on Skin Doses to Six Workers in August 1979. The licensee updated the dose assessment on December 5,1979, and the licensee results are being reviewed by an NRC consultant. The licensee results are summarized in Paragraph 6.

The NRC has obtained the services of a consultant to perform an independent review of the dose assessment. NRC review of this area continues.

Facility Procedure Review (Unit 2)

Operating and Emergency / Abnormal Procedures, Health Physics Procedures, Recovery Surveillance Procedures, and Special Op9 rating Procedures, including subsequent revisions, were reviewed by the NRC/THI Technical Support Staff this inspection.

Procedure review included both health physics and operations aspects with consideration of the following:

(1) the procedure, when implemented, would not degrade the containment of radioactive material, jeopardize core cooling, or result in excessive personnel exposure;(2)

the procedure conforms to the general criteria of TS 6.8, " Procedures", TS 6.11, " Radiation Protection Program," and,

ANSIN-18.7,1976;and,(3)

the technical content of the procedure is Odequate to perform the intended evolution.

Approximately 91 procedures / procedure changes were reviewed along with 32 Temporary Changes to the above types of procedures.

Composite staff comments on procedures were forwarded to the licensee.

No instances of failure to adequately res11ve these comments were identified.

4.

Review of Plant Operations (Units 1 and 2)

a.

Unit 1 (1) Shift Logs and Operating Recoros The inspector reviewed the following logs and records:

Shift Foreinan Log for December 3-21, 1979; and

Control Room Operator's Log Book for December 12-22,

1979.

The logs were reviewed to verify the following items:

Logkeeping practices and log book reviews are

conducted in accordance with established admin-istrative controls; and, Log entries involving abnormal conditions are

sufficiently detailed.

Acceptance criteria for the above review included inspector judgement, applicable Technical Specifications, and procedure AP 1012, " Shift Relief and Log Entries."

No items v r.oncompliance were identified.

(2) Plant Tour At various times during December 17-22, 1979, the inspector

conducted tours of the following accessible Unit 1 areas:

Auxiliary Building; Turbine Building; Control and Service Bui~1 ding; Control Room; and outside areas.

The following observations / discussions / determinations were made:

conditions and storage of materials and components to prevent safety and fire hazards, were observed; Systems and equipment in all areas toured were

observed for the existence of fluid isaks and abnormal piping vibrations; Control Room manning was observed on several occasions

during the inspection; Selected fire extinguishers were observed for

unobstructed access and adequate pressure and/or level;

Selected fire alarm reporting stations were observed

"no smoking" areas did not exhibit evidence of smoking.

Acceptance criteria for the above items included inspector judgment and requirements of 10 CFR 50.54(k), Regulatory Guide 1.114, applicable Technical Specifications, and the following procedures:

AP 1003, " Radiation Protection Manual;"

AP 1008, " Good Housekeeping;"

--

AP 1009, " Station Organization and Chain of Command;"

--

AP 1028, " Operator at the Controls;" and,

--

AP 1050, " Control of High Radiation Areas."

No items of noncompliance were identified.

(3) Status Meetings NRC/TMI Technical Support Staff members attended licensee meetings to observe and ascertain additional plant status information. The meetings attended included those listed below, and involved discussions of:

Plant Tours On a daily basis three shift inspectors completed a general plant tour including all control points and selected radio-logically controlled areas.

Observations included:

Cleanliness and housekeeping conditions;

Construction status and startup testing progreas;

--

Access control to radiologically controlled areas;

--

Use of survey meters including personnel frisking tech-

niques; Proper use of respiratory protection equipment;

Adherence to Radiation Work Parmit (RWP) requirements;

Various logs / records to ascertain current licensee

actions / evaluations / problem areas; Adherence to Health Physics and Operating Procedures;

Measurement Verifications The below listed measurements were independently obtained to verify the quality of licensee performance in these selected areas:

No items of noncompliance were identified during shift inspection reviews.

6.

Dose Assessment of the Skin Exposures to Six Workers.

The licensee updated the dose assessment on December 5,1979, of the skin exposures received by six workers on August 28, 1979.

The licensee results are summarized below giving the third calendar quarter, 1979, doses to these individuals.

,

10 CFR 20.101(a) sets a limit of 71/2 rems per calendar quarter on dose to the skin of the whole body, and 18 3/4 rems per calendar quarter on dose to the hands and forearms, feet and ankles.

0.7 A licensee medical consultant examined individuals a through f on the afternoon of August 30, 1979.

His report stated there was no indication of medically significant exposure to the skin of any of these individual ___

To avoid any continuation of the overexposure, the six individuals were restricted from radiation work for the remainder of the calendar quarter. There was no lost time off work.

The licensee has reviewed with the radiation protection personnel the corrective actions to preclude any future overexposures.

Personnel were retrained in the selection, use and limitations of survey instruments.

Protective clothing requirements were reviewed.

Planning and preparation for high dose rate jobs were reviewed.

The TMI-2 Radiation Protection Plan is being revised.

NRC is obtaining the services of a consultant to furnish an independent dose assessment of the six individuals.

NRC has reviewed the radiation protection program and directed the above corrective actions.

Onsite inspectors have stepped up the review of survey results and radiation work permits and observe licensee activities on every operating shift.

NRC will continue to review the licensee's performance to ensure that the corrective actions are carried out.
