Inspection on May 30 - June 13, 1979 (Report Nos. 50-289/79-13 and 50-320/79-15)

Areas Inspected: Special, unannounced inspection of the fire prevention /

protection program including: new installations, existing installations, surveil-lance test requirements, housekeeping, administrative controls, plant fire brigade, local fire company assistance and fire brigade response. The inspection involved 123 inspection-hours onsite by two NRC regional based inspectors.

Results_:

In the eight areas inspectiid, no items of noncompliance were identified in seven areas; two apparent items of noncompliance were identified in one area (Infraction - failure to protect welding operations with portable fire extin-guishers - paragraph 2.b.(2); and Infraction - failure to remove accumulations of combustibles from a work area - paragraph 2.b.(1)).

New Installations The inspector examined the modifications in progress on the roof of the Unit 2 auxiliary building, in the Unit 2 fuel storage area, and outside the Unit 2 turbine building.

a.

Auxiliary Building Roof The inspector examined the work in progress for installation of charcoal filter beds, including the sprinkler system that has been installed to provide fire protection.

The inspector questioned the following practices which may result in delays of sprinkler activation and/or curtail the effectiveness of the sprinklers:

(1) Use of non-indicating valves which will not allow the operator to determine valve positions.. This practice appears to contradict the licensee's commitments listed in the Fire Hazard Analysis Report which states that sprinkler systems will meet the design and installation requirements of NFPA 13. -

(_2) Two out of the four sprinkler systems have valves installed downstream of the deluge valves in poor access areas.

To assure an open path for the sprinkler water during a fire emergency these valves should be locked and supervised in the open position.

(3) Negative air pressure inside the charcoal filter bed enclosures causes air to be sucked into the enclosure through the sprinkler piping.

Dirt in the air stream may plug up sprinkler discharge orifices thus rendering the sprinkler system ineffective.

(4) An operating and valve lineup procedure is not available to the operators, thus the manual activation of the sprinkler systems

may result in unexpected delay during an emergency.

This item is considered to be unresolved, pending a review of the licensee-provided information by an NRC inspector during a subscquent inspection.

b.

Fuel Storage Area The inspector examined the work in progress for installation of special holding tanks : including the fire protection / prevention equipment and practices.

(1) Accumulation of Unnecessary Combustibles The inspector found accumulations of combustible trash and un-necessary lumber in the fuel storage area.

The inspector con-sidered this contrary to the requirements of licensee procedure 1410-Y;26,1"W~ork: Request Procedure," (Rev. 3), which states, in paragraph 6.2.1: " Area Preparation: Shall be clear of accumu-lations of combustibles such as trash, rags, etc."

The inspector then cited the licensee for an infraction to 10 CFR 50, Appendix B, Criterion V, which states: " Activities affecting quality shall be prescribed by documented instructions...and shall be accom-plished in accordance with these instructions...."

Fire Extinguisher Protection The inspector found welding operations in progress in a pit accessible only by ladder from elevation 347.5 feet of the fuel handling building.with no readily accessible fire extinguishers in the area.

The inspector considered this contrary to the requirements of licensee procedure 1410-Y-26, " Work Request 2226 046

Procedure, " (Rev. 3), which states, in paragraph 6.2.7:

" Appropriate fire extinguishers shall be readily accessible at the work location." The inspector then cited the licensee for an Infraction to 10 CFR 50, Appendix B, Criterion V, which states: " Activities affecting quality shall be prescribed by documented instructions...and shall be accomplished in accord-ance with these instructions...."

c.

Auxiliary Diesel-Generator

-

The inspector examined the work in progress to protect the auxiliary diesel-generator hook-up outside the circulating water pump house.

The licensee was in the process of constructing a dike around the two diesel oil tanks, to protect the surrounding area from fire in the event of an oil spill from the tanks.

The licensee also comitted to providing a protective water sprinkler system for the area, but had not yet begun the installation of the system.

This item is considered to be unresolved, pending the completion of the fire-protection modifications in this area, and their inspection by an NRC inspector during a subsequent inspection.

3.

Existing Installations The inspector examined interior and exterior fire protection systems and equipment in Unit 1 and accessible areas of Unit 2.

a.

Hose Houses The inspector.noted during his examination of interior and exterior hose houses, that no 1 1/2-inch fire hose was available, although larger sized (21/2-inch) hose was available.

Since the smaller sized hose is easier for a single fire-fighter to control, the licensee stated that he would equip approximately half of the hose houses with 1 1/2-inch fire hose.

The inspector also noted, during his examination, that emergency lanterns were missing from the hose house at grade elevation of the

Unit 1 turbine generator building.

The licensee stated that he would replace these missing lanterns.

This is considered to be an unresolved item and will be re-examined by an NRC inspector during a subsequent inspection.

b.

Unit 2 Auxiliary Building The inspector noted, during his tour of the Unit 2 auxiliary building, a large wooden packing case and a stack of wooden planks near an electrical control panel on the 305 foot elevation.

The licensee stated that this material was brought in earlier to be used in the recovery operation; these combustibles would now be removed.

This is considered to be an unresolved item and will be re-examined by an NRC inspector during a subsequent inspection.

No items of noncompliance were identified.

4.

Fire Detection Surveillance In accordance with the licensee's technical specifications, fire detection instruments shall be demonstrated operable at least once per six months.

The last fire detection surveillance tests, identified by the licensee by task numbers 2333 SA-1 and 2333 SA-2 have been completed on January 30, 1979 and February 6,1979.

The inspectors noted that the tests must be rerun within a six-month period per the licensee Tech Specs.

This was brought to the attention of the licensee.

The licensee agreed to reinstate surveillance of the detectors in all accessible areas.

This is considered to be an unresolved item and will be re-examined by an NRC inspector during a subsequent inspection.

5.

Administrative Controls a.

Housekeeping The inspectors verified that the licensee conducts a weekly tour of plant areas to identify fire protection problems including accumulation of combustible materials.

The inspectors, however,-

identified several locations in the plant (detailed in paragraphs 2.b.(1) and 3.b) where excessive accumulation of combustibles created unacceptable fire hazards.

The licensee removed the combustibles from most areas prior to the completion of the inspection, and was planning to clear up all identified areas the near future.

The licensee also agreed to increase the frequency of plant tours, thus reducing the amount of time that accumulated combustibles in any plant area may go unnoticed.

b.

Plant Fire Brigade The inspectors verified that all members of the fire brigade have received appropriate training within the last 12 months. The licensee indicated that there have been no changes in the make-up of the fire brigades, and the responsibilities of ir.dividual fire brigade members remain the same as assigned prior to the March 28, 1979 incident.

c.

Local Fire Company Assistance The inspectors interviewed the chiefs of the Middletown and Londonderry volunteer fire departments.

The inspectors verified that the written agreements for adequate support by the offsite organizations are maintained.

Onsite orientation for members of the volunteer fire departments was held in December 1978.

No items of noncompliance were identified.

6.

Fire Brigade Responses The inspector verified that the fire brigade and the offsite volunteer fire department responded in a prompt and adequate manner to the facility fire alarms during fire emergencies.

No items of noncompliance were identified.

7.

Unresolved Items Unresolved items are matters about which more infonnation is required in order to ascertain whether they are acceptable items, items of noncompliance, or deviations.

Unresolved items disclosed during this inspection are discussed in paragraphs 2.a, 2,c, 3,a, 3,b, and 4.

8.

Exit Interview The inspectors met with the licensee representatives (denoted in paragraph 1) at the conclusion of the inspection on June 8,1979.

The inspectors summarized the purpose and scope of the inspection and findings. The
