nspection on August 1-31. 1981 (Report No: 50-282/81-17; 50-306/81-19)

Areas Inspected: Routine resident inspection of plant operation, prepara-tion for refueling, surveillance, security, radiation protection, and followup on IE Bulletins and Licensee Event Reports. The inspection in-volved:a total of 114 inspector hours onsite by 2 NRC inspectors including 24 inspector hours onsite during off-shifts.

Results: No items of noncompliance were identified.

General-Unit 2 operated at power throughout the month. Unit 1 operated at power until August 31, when a turbine trip from 100% power' occurred due to a failure-in a turbine control logic circuit.

Preparations

The licensee has started grading operations in conjunction with modification of the intake and discharge canals.

Control Room Observations The inspe.ctor observed control room operations, reviewed applicable

.4 logs, conducted discussions with control room operators and observed shift. turnovers. The inspector verified the operability of selected emergency systems, reviewed equipment control records and verified proper return to service of affected components.

Additional containment inspections have not identified the source.

Leakage has not increased significantly, and is still too small to be of concern. The unit is scheduled to shutdown for refueling in September.

3.x Steam Generator Tube Leak f

On August 20th the Unit 2 air ejector offgas and steam generator blow-

down monitors:showed an activity increase, indicating a small. primary

,

to. secondary steam generator tube leak. The licensee initiated a l sampling schedule to monitor the amount of leakage. The leak appears to have stabilized at approximately 0.1 gallons per minute, about 10%

of the. leakage allowable by Technical Specification. The licensee is continuing to monitor the leakage, as a leak of this magnitude would l,

be difficult to find. The licensee is planning to shutdown for repairs if.the leak should increase, js 4.

Surveillance.

The -inspectors witnessed portions of surveillance testing of safety re-L lated system and components. The. inspection included verifying that the.

,

tests were scheduled and performed within Technical Specification require-ments, observing that procedures were'being followed by qualified opera-

-

tors, that LCO's were not violated, that system and equipment restoration

,

l-was: completed, and that test results were acceptable to test and Techni-cal Specification requirements.

l Tests witnessed included:

SP-1186 Diesel Cooling Water System Test.

The test was required because of preventive maintenance performed on D-1.

Test was satisfactory.

L The inspector reviewed surveillance testing of safety related systems L

and components to insure that the test procedure was adhered to, that documentation of informatioa was accurate, and that Technical Specifica-tions were complied with.

Through direct observations, discussions with licensee personnel, and review of records, the following event reports were reviewed to deter-mine that reportability requirements were fulfilled, immediate corrective action was accomplished, and corrective action to prevent recurrence had been accomplished in accordance with Technical Specifications.

P-RO-81-11 Inoperability of D-2 Diesel Generator While D-1 was Out of' Service for Preventive Maintenance.

The report accurately described the event. The licen-see's imediate corrective actions restored operabil-icy within 14 minutes of the trip and lockout when the 8enerator was synchronized to the bus. All offsite power resources were available and all other safeguards systems were operable. The licensee would have been required to place both units in cold shutdown if D-2 had not been demonstrated to be operable.

The inspectors reviewed refueling and fuel receipt procedures to insure

-that technically adequate approved procedures were available covering the receipt, inspection, and storage of new fuel and the handling and storage of spent fuel. The inspectors also reviewed the licensee's

program'for outage control.

The inspectors witnessed portions of the receipt, inspection, and

storage of new fuel..This included verification-of the qualification of fuel receipt inspectors, and verifing compliance with the fuel receipt' procedures.-
