Plant Operations (71707) (71710) (93702) Units 1 and 2 operations were inspected and reviewed during the inspection period. The inspectors kept informed, on a daily basis, of the overall status of both units and of any significant safety matte-rs relating to plant operations. The inspectors conducted various plant tours and made frequent visits to the control roo Observations included the following: witnessing work activities in progress; verifying the status of operating and standby safety systems and equipment; and confirming valve and breaker positions, instrument and recorder readings, and annunciator alarm The inspectors verified that equipment was properly tagged,,

plant housekeeping efforts were adequate, that appropriate radiat,.dn ~ontrols were properly established, that clean areas were being controlled in accordance with procedures, that excess materi a 1 or equipment was stored properly, and that combustible material and debris were disposed of expeditiously. 1--leaks, piping vibrations, piping hanger and seismic restraint abnormalities, instrument calibration dates, and adequacy of fire fighting equipmen Certain tours were conducted on backshift Inspections included areas in the Units 1 and 2 cable vaults, vital battery rooms, fire pump house, steam safeguards areas, emergency switchgear rooms, diesel generator rooms, contra 1 room, auxiliary building, cab 1 e penetration areas,low level intake structure, and safeguards valve pit area The inspectors performed an ESF system walkdown on July 28 to verify valve positions for containment spray, recirculation spray, and safety injection valves in the Vnit 1 and Unit 2 safeguards valve pits. Also, a walkdown inspection of circulating water and emergency service water systems was conducted at the low level intake structur Reactor coolant system leak rates were reviewed to ensure that detected or suspected leakage from the system was recorded, investigated, and evaluated and that appropriate actions were taken, if require On a regular basis, radiation work permits (RWPs) were reviewed and specific work activities were monitored to assure they were being conducted per the RWP Selected radiation protection instruments were periodically checked, and equipment operability and calibration frequency were verifie Unit 1 began the reporting period in the final week of a refueling and maintenance outage that.began May 10, 198 The Unit was restarted on July 12, 1986, and completed low power physics testing on July 1 Following the compl~tion of physics testing, several rod control urgent failure alarms were experience Several cards were replaced during rod control troubleshooting, including a master cycler card and a supervisory data logging car Due to the possibility that the bank overlap sequence for control banks 1C 1 and 1D1 was off by several steps following troubleshooting, the unit was manually tripped on July 14 and the rod control sequence was rese Unit.1 was restarted early on July 15, and continued to operate at power for the duration of the reporting perio Unit 2 began the reporting period operating at powe On July 20, 1986, a tube leak on the 18 1 Recirculation Spray Heat Exchanger (RSHX)

was discovered during a containment entry to investigate increased flow to the containment sum Service water inlet and outlet valves to the 1 8 1 RSHX were shut to isolate the leak. On July 22, an unusual event was declared due to the rampdown required by Technical Specifications LCD for repair of the 118 1 RSH The unit came off line early on July 2 Seven tubes were plugged on the 18 1 RHSX and the remaining heat exchangers were hydrostatically tested. The unit was restarted on July 27, and operated at power for the remainder of. the reporting perio The inspector monitored the shutdown of the unit on July 23 and the restart of the unit on July 2 At 8:50 a.m. on July 19, 1986, a bomb threat was phoned in to the licensee 1s security shift superviso An unusual event was declared at 9:12 The licensee conducted a thorough search of the station and increased security at its access buildin No explosive devices were found. The unusual event was terminated at 4:19 In the course of monthly activities, the inspectors included a review of the licensee 1s physical security progra The performance of various shifts of the security force was observed in the conduct of daily activities to include:

Maintenance Inspections (62703)

During the reporting period, the inspectors reviewed maintenance activities to assure compliance with the appropriate procedure Inspection areas included the following: The *resident inspector observed the cycling of service water valve MOV-SW-1030 for MOVA TS testing *in accordance with PT 18.10 and PT 25. 2 on July 15t The resident inspector observed the uncoupled run of the turbine driven auxilliary feedwater pump following shaft replacement and repacking on July 16t The inspectors observed Instrument Technicians troubleshooting the cause of a Rod Control System Urgent Failure Alarm on Unit 1 on July 2 The inspector reviewed the applicable procedure (MMP-C-RS-043, RE SPRAY HEAT EXCHANGER REPAIR) prior to performance of repairs to Unit 2 Recirculation Spray Heat Exchanger 18 1 *
