NRC INSPECTION REPORT 50-275/98-10; 50-323/98-10 AND NOTICE OF VIOLATION
Dear Mr. Rueger:
From May 10 through June 20, 1998, a routine inspection was conducted at your Diablo Canyon Nuclear Power Plant, Units 1 and 2 reactor facilities. The enclosed report presents the scope and results of that inspection.
During the 6-week period covered by this inspection report, your conduct of activities at the Diablo Canyon Nuclear Power Plant facilitywas generally characterized by safety-conscious operations, generally sound engineering and maintenance practices, and good radiation protection support.
Based on the results of this inspection, one violation of NRC requirements was identified. This violation involved your staffs identification of the failure to follow a maintenance procedure resulting in performing maintenance on an incorrect auxiliary feedwater pump.
Removing oil from the wrong pump resulted in two auxiliary feedwater pumps being inoperable simultaneously, which was a potentially risk significant configuration. This violation is not being considered for enforcement discretion because of the inadequate immediate actions by your staff upon discovery.
Those actions were inadequate in that personnel failed to notify the control room in a timely manner so that action could be taken to prevent automatic start of the turbine-driven auxiliary feedwater pump, and personnel continued to work on the wrong component without work authorization or a clearance.
The violation is cited in the enclosed Notice of Violation (Notice) and the circumstances surrounding the violation are described in detail in the enclosed report. The NRC has concluded that information regarding the reason for the violation, the corrective actions taken and planned to correct the violation and prevent recurrence is already adequately addressed in this inspection report. Therefore, you are not required to respond to this letter unless the.
description therein does not accurately reflect your corrective actions or your position.
In that case, or ifyou choose to provide additional information, you should followthe instructions in the enclosed Notice.
