Notice of Violation and Enforcement Conference Meeting (NRC Inspection Nos. 50-272/89-16; 50-311/89-15)
This refers to the NRC safety inspection conducted on May 27 - July 10, 1989 at the Salem Generating Station to review the circumstances associated with the inoperability of the feedwater isolation system at both unit This event was identified by your staff and reported to the NRC, and involved the violation of a Technical Specification Limiting Condition for Operatio The subject inspection report was sent to you on July 14, 198 On July 26, 1989, an enforcement conference was conducted with Mr. S. LaBruna and members of your staff to discuss the violations, their causes, and your corrective action A summary of tre enforcement conference and your presentation notes are enclosed with this lette The NRC strongly supports comprehensive licensee programs for detection, cor-rection and reporting of problems that affect the safety of plant operation To the extent that your staff's identification and correction of this occur-rence ~eflects a management commitment to foster a questioning attitude toward plant activities and to fully support comprehensive corrective actions where necessary, the licensee's actions have been commendabl The violation, which is described in 'the enclosed Notice, includes the failure to conduct adequate surveillance testing of the feedwater isolation system and the failure to meet Technical Specification response time requirements when properly teste The NRC recognizes that the safety significance of this violation is small, since there is a high probability that the feedwater isolation valves would have operated within 10 seconds (instead of the required 7 seconds) and that subsequent analysis has shown this to be an

Nonetheless, any similar violations in the future may result in escalated enforcement actio You are required to respond to this letter and, in preparing your response, you should follow the instructions in the enclosed Notic After reviewing your response, including your proposed corrective action and the results of future inspections, the NRC will determine whether further enforcement action is necessar During the enforcement conference, we also discussed the importance of deter-mining the as-found condition of plant equipment, as well as, three unresolved inspection items documented in the subject repor You correctly stated that as-found testing is not a requirement for preventive maintenanc It was clarified that the importance of as-found testing relates to the surveillance testing program and specifically to the adequacy of the test frequenc You stated that feedwater isolation valves were the only components identified where the frequency of preventive maintenance component replacement/overhaul is the same as the Technical Specification surveillance frequency for that syste Further, you stated your intention to lengthen this replacement interva This resolves NRC concerns in this are With regard to the other unresolved inspection findings, these concerns will be addressed in a subsequent NRC inspection of your facilit In accordance with 10 CFR 2.790, a copy of this letter and its enclosures will be placed in the NRC Public Document Roo The responses directec by this letter and the enclosed Notice are not subject to the clearance procedures of the Office of Management and Budget, otherwise required by the Paperwork Reduction Act of 1980, Pub.
