On September 30, 2019, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at Turkey Point Units 3 & 4. On October 10, 2019, the NRC inspectors discussed the results of this inspection with Mr. Brian Stamp, Site Director, and other members of your staff.

The results of this inspection are documented in the enclosed report.

The NRC inspectors did not identify any finding or violation of more than minor significance.

SUMMARY

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an integrated inspection at Turkey Point Units 3 & 4 in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. Refer to https://www.nrc.gov/reactors/operating/oversight.html for more information.

No findings or violations of more than minor significance were identified.

PLANT STATUS

Unit 3 began the inspection period at rated thermal power. On September 26, 2019, the unit was down powered to 30 percent to reduce radiation levels in containment for workers performing maintenance on the containment inboard exhaust purge valve POV-3-2603. The unit was returned to rated thermal power on September 29, 2019, and remained at or near rated thermal power for the remainder of the inspection period.

Unit 4 operated at or near rated thermal power for the entire inspection period.

INSPECTION SCOPES

Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors performed plant status activities described in IMC 2515 Appendix D, Plant Status and conducted routine reviews using IP 71152, Problem Identification and Resolution. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

71111.01 - Adverse Weather Protection

Impending Severe Weather Sample (IP Section 03.03) (1 Sample)

(1) The inspectors evaluated readiness for impending adverse weather conditions for Hurricane Dorian from August 28 through September 3, 2019.

71111.04Q - Equipment Alignment

Partial Walkdown Sample (IP Section 03.01) (6 Samples)

The inspectors evaluated system configurations during partial walkdowns of the following systems/trains:

(1) A and B auxiliary feedwater (AFW) pumps while the C AFW pump was out of service (OOS) on July 2, 2019

(2) Unit 3 and Unit 4 class 1E 125-Volt DC battery systems while the 4A battery was OOS and the spare class 1E battery was supplying the 4A DC bus on July 15, 16, and 19, 2019

The inspectors evaluated fire protection program implementation in the following selected areas:

(1) Fire zones (FZs) 81, 82, 86 and 87 for the Unit 3 and Unit 4 start-up, auxiliary, and main transformers on July 01, 2019

(2) FZs 45, 47, 54 and 55 for the Unit 3 and Unit 4 charging pump rooms and component cooling water pump rooms on July 01, 2019

(3) FZ 122 for the electric and diesel driven fire pumps, jockey pumps, and raw water tank areas on August 12, 2019

(4) FZs 30, 31, 38 and 40 for the Unit 3 and Unit 4 containment spray pump and pipe and valve rooms on August 15, 2019

(5) FZs 4, 5, 6, 7, 8, and 9 for the 2 and 4 elevation common areas of the auxiliary building on August 21, 2019

(6) FZs 25, 25A, 98, 101, and 104 for the Units 3 and Unit 4 cable spread room, electrical equipment room, Unit 4 B DC equipment room, Unit 3 A DC equipment room, and class 1E spare battery room on August 22, 2019

(1)3B and 4A Component Cooling Water (CCW) Heat Exchangers, Cooled by Intake Cooling Water Section 02.02b (2)3A and 4B Residual Heat Removal (RHR) Heat Exchangers, Closed Loop, Section 02.02c

(3) Cooling Canals, Heat Sink, Section 02.02d, specifically Sections 02.02d#6 and 02.02d#7 were completed.

Licensed Operator Performance in the Actual Plant/Main Control Room (IP Section 03.01) (2 Samples)

(1) The inspectors observed and evaluated licensed operator performance in the Main Control Room during an unannounced fire drill on September 17, 2019.

(2) The inspectors observed and evaluated licensed operator performance in the Main Control Room during reactivity operations on Unit 3 while diluting the reactor coolant system boron concentration to maintain rated power, and during Unit 4 AFW train 2 surveillance testing on September 30, 2019.

Licensed Operator Requalification Training/Examinations (IP Section 03.02) (1 Sample)

(1) The inspectors observed and evaluated a requalification training simulator scenario administered to an operating crew on August 20, 2019

71111.12 - Maintenance Effectiveness

Routine Maintenance Effectiveness Inspection (IP Section 02.01) (1 Sample)

The inspectors evaluated the effectiveness of routine maintenance activities associated with the following equipment and/or safety significant functions:

(1) Action Request (AR) 2322902, B AFW pump tripped early at 6,403 rpm (event date was July 31, 2019), on August 26 and 28, and September 25 and 26, 2019.

Quality Control (IP Section 02.02) (1 Sample)

The inspectors evaluated maintenance and quality control activities associated with the following equipment performance activities:

(1) Work Order (WO) 40224302, 3A EDG radiator core replacement, quality control sample on August 12, 15, 16, 19, and 20, and September 16, 2019.

71111.13 - Maintenance Risk Assessments and Emergent Work Control

The inspectors evaluated the risk assessments for the following planned and emergent work activities:

(1) Unit 3 and Unit 4 online risk with BS-4-1403, basket strainer to intake cooling water (ICW) supply for the 4C CCW heat exchanger, and E232 condensing unit for the electric equipment room air handling unit V76 OOSs on July 1, 2019

(2) Unit 3 and Unit 4 online risk with the 3A1 battery charger and the 3A CCW heat exchanger OOSs on July 16 and 19, 2019

(3) Unit 3 and Unit 4 online risk with Unit 3 and Unit 4 AFW train 1 and A AFW pump OOSs and the Unit 4 control rods aligned for manual operation on August 12, 2019

(4) Unit 3 and Unit 4 online risk with the Unit 3 and Unit 4 AFW train 1, Unit 3 ICW A header, and the 3B charging pump OOSs on September 11 and 12, 2019

(5) Unit 3 and Unit 4 online risk with the 3A EDG, 3A charging pump, and the 4D 4kV switchgear room air conditioner 4S230 OOSs on September 16, 17, 19, and 20, 2019

71152 - Problem Identification and Resolution

Annual Follow-up of Selected Issues (IP Section 02.03) (3 Samples)

The inspectors reviewed the licensees implementation of its corrective action program related to the following issues:

(1) AR 232037232037 B AFW pump overspeed cam device not returned to required normal position causing overspeed trip during test. This issue was selected for follow-up to verify FPL's corrective actions were appropriate to address clarifications to procedural ambiguities that contributed to the overspeed trip and additional unavailability of the B AFW pump.

(2) AR 1823645 & 2306639, foreign material concerns in CCW systems due to degradation of valve rubber seats. This issue was selected for follow-up to ensure FPL intended to perform extent of condition inspections for a population of CCW rubber seated valves that if degraded could introduce foreign material in the Unit 3 and Unit 4 CCW systems.

(3) AR 2170587, tornado missile protection walkdowns. This issue was selected to verify that FPL resolved any identified tornado missile protection vulnerabilities that were not in conformance with Turkey Point's current license basis.

71153 - Followup of Events and Notices of Enforcement Discretion Event Report (IP Section 03.02)

The inspectors evaluated the following licensee event reports (LERs):

(1) LER 05000250/2019-001-00, Unit 3 Manual Reactor Trip Following Grid Disturbance (ADAMS accession: ML19179A290): The inspectors determined that it was not reasonable to foresee or correct the cause discussed in the LER therefore no performance deficiency was identified. The inspectors also concluded that no violation of NRC requirements occurred. The circumstances surrounding this LER are documented in Inspection Report 05000250/2019002 and 05000251/2019002 Section 71153 (ADAMS Accession No. ML19219B126).
