This routine, unannounced inspection was conducted in the areas of concrete placement for the addition of the New Emergency Diesel Generator.

Results:

In the areas inspected, violations or deviations were not identified.

During this inspection the inspector verified that the concrete placement performed on April 10, 1990 on the Diesel Generator Roof slab was properly placed.

The inspector also reviewed concrete placements made August, September, October, November, December 89, and January, February, and March 90 and verified that the concrete was properly placed.

Structural Concrete Work Observation (46053)

The inspector observed placement of pour No.

EDG-76 in the New Emergency Diesel Generator Roof Slab.

The mix design was a 4000 psi pearock pump mix.

Acceptance Criteria examined by the inspector were as follows.

Specification CN-29, Concrete Materials and Mixes, Concrete Mixing and Transportation Specification NC-2. 11, Concrete Testing Placing, Curing And Finishing PCM 259, Emergency Diesel Generator Building Construction TS-10.3-1. 1, Concrete Preplacement TS-10.3.2. 1, Concrete Placement TS-10.3.3-1, Concrete Post Placement TS-10.4-1. 1, Concrete Field Testing TS-10.4-2. 1, Laboratory Testing of Concrete DWS 5610-C-1595, S.H.3/87-259, Embedded Steel and Penetrations Second Floor and Roof Plans DWS 5610-C-1592, S.H.1/87-259, Wall Reinfor cing

DWS 5610-C-1595, S.H.2/87-250, Second Floor and Roof Plans Reinforcing DWS 5610-C-159S, S.H.1/87-259, Second Floor and Roof Plans Masonry The review indicated that forms were tight clean and level and that rebar was properly installed and clean.

Placement activities pertaining to delivery time, free fall, flow distance, layer thickness and consolidation conformed to specifications.

Concrete placement activities were continuously monitored by construction and gA inspectors.

Examination of batch tickets indicated that the specified design mix was being delivered.

Samples of concrete were obtained from the pump line discharge and tested in accordance with specification requirements.

Test results indicated that the concrete being placed met the specification requirements for slump and temperature.

Structural Concrete Record Review (46055)

The inspector reviewed records for concrete placed August 89, September 89, October 89, November 89, December 89, January 90, February 90 and March 90.

Review of these records indicated that the rebar was properly installed, that the concrete was properly placed, that the concrete was properly monitored by construction and guality Control Inspectors, and that the compressive strength of the concrete met the compressive strength requirements'.

Exit Interview The inspection scope and results were summarized on April 12, 1990, with those persons indicated in paragraph 1.

The inspector described the areas inspected and discussed in detail the inspection results.

The licensee did not identify as proprietary any of the material provided to or reviewed by the inspector during the inspection.
