Infraction 251/80-18-01:
Failure to follow concrete expansion anchor inspection procedure.
A follow-on inspection to the inspection documented on IE Report 50-251/80-26 was performed to evaluate the

licensee's corrective action for the violation.
The licensee's A/E per-formed an engineering examination of the Unit 4 IEB 79-02 inspection docu-mentation for the months of May and June 1979 to determine its adequacy and completeness.
The results of the engineering examination were reported on Bechtel'etter SFB-183 dated October 26, 1981.
The results were reviewed and,the reinspections required were sampled and inspected.
Although the re-inspections were complete, subsequent re-analysis and repairs had not been completed.
The inspector had no further questions.
The remainder of the work falls within the normal scope of IEB 79-02/79-14 and will be monitored
.
as part of the bulletin inspections.

Unresolved Item 251/80-18-03:
Ripout control.
A follow-on inspection to the inspection documented on IE Report 50-251/80-26 was performed to resolve this item.
On April 2, 1981, the licensee issued Maintenance Procedure 0734, Safety Related Supports/Restraints Removal and Replacement.
The procedure provides requirements for documenting the removal and restoration of safety-related pipe supports and restraints.
The inspector had no further questions on this item.
This item shall be closed.

Pipe Support Basepl'ates Using Concrete Expansion Anchor Bolts ( IE Bulletin 79-02)
On October 16, 1981, the licensee submitted a final response to IE Bulletin 79-02.
The response was reviewed and discussed with the licensee.
A follow-on inspection to those documented on IE Reports 50-250/80-18, 50-251/80-18, 50-250/80-27 and 50-251/80-26 was performed to verify licensee compliance with the bulletin requirements and their commitments.
The inspection of the licensee's corrective action for infraction 50-281/
80-18-01 is noted on paragraph 3.
As part of that inspection, engineering evaluation of the following notices of discrepancies (NOD) were reviewed 80-600,80-184, 80-219,80-467, 80-493,80-494, 80-804,80-753, 80-090,80-071, 80-069,80-785, 80-488,80-491, 80-481 and 80-802.
NODs80-600, 80-090,80-481 and 80-802 indicated that IEB 79-02 inspection documentation did not provide baseplate thickness but this condition was not considered to be technically significant.
Discussions with the A/E revealed that the condition was not considered significant because the plate thicknesses had been recorded on the IEB 79-14 records.
The licensee committed to clarify similar engineering evaluations by indicating that the missing information was obtained by IEB 79-14 walkdowns.
This clarification would be contained in a letter from the A/E to the licensee.
The Unit 4 pipe supports shown on the following drawings were inspected:

Pending completion of repairs and re-analysis, this Bulletin shall remain open.
Seismic Analysis for As-Built Safety-Related Piping ( IE Bulletin 79-14)
On December 21, 1979, the licensee submitted its response to IEB 79-14.
A follow-on inspection to those documented on IE Report Nos.
50-250/80-18, 50-251/80-18, 50-250/80-27 and 50-251/80-26 was conducted to verify compli-ance with Bulletin requirements and licensee commitments.
The pipe supports detailed on the following sketches were inspected:
5177-102-SK-P-902 5177-102-SK-P-887 5177-102-SK-P-903 5610-C-675 Discussions with the licensee revealed that the IEB 79-14 final analysis will be performed by Teledyne Engineering Services.
The system walkdowns and functional analysis were performed by Bechtel Power Corporation.
IEB 79-14 system packages were assembled by Bechtel and turned over to Teledyne by the licensee.
The licensee is coordinating the interface activities between Bechtel and Teledyne.
The licensee indicated that all IEB 79-14 walkdowns and repairs are complete for Unit 4.
Teledyne will be performing the final seismic analysis and hanger analysis for Unit 4.
Any further repairs resulting from the final analysis will be identified by Teledyne and the licensee committed to provide this information to the Region by telephone.
Unit 3 systems and shared systems are to be analyzed and hanger design loads are to be obtained by Teledyne.
Hanger analysis will subsequently be performed by Teledyne.
Any additional inspections will be determined by Teledyne.
The licensee will perform the subsequent inspections and Bechtel will disposition any nonconformances.
The licensee estimated the completion of all Unit 4 IEB 79-14 work during the current outage.
The licensee's May 5, 1979, response to IEB 79-04, "Incorrect Weights for Swing Check Valves Manufactured by Velan Engineering Corporation",
was reviewed and discussed with the licensee.
The inspector had no further questions at the site regarding IEB 79-04.
Inspections for correct valve weights used in the piping stress analysis will be included in any IEB 79-14 inspection of the analysis.
IE Bulletin 79-04 shall be closed.
The licensee's September 18, 1981, response to NRR Generic Letter 81-14,
"Seismic gualification of Auxiliary Feedwater System",
was discussed with the licensee to clarify portions of the response.
With regard to paragraph

IV(8)A. of the response, the licensee stated that although the turbine building was not rigorously analyzed, their engineering judgement was that the building was capable of withstanding an SSE.
In further clarification of the response, the licensee stated that their walkdowns were performed on a sampling basis and that the samples chosen were the items considered to be the worst cases.
The licensee stated that discrepancies noted were corrected and that the auxiliary feedwater was now considered to be adequate for an SSE.
Finally the licensee also stated that an overhaul of the entire auxiliary feedwater system was scheduled for subsequent outages of Units
and 4.
This information will be forwarded to NRR.
7.
Deep Draft Pump Deficiencies ( IEB 79-15)
The licensee's September 18, 1979, and September 11, 1979, responses to IEB 79-15 were reviewed and discussed with the licensee.
The inspector had no further questions on this item.
On Nay 4, 1981, IE Headquarters provided an evaluation of licensee responses to the Bulletin.
The letter had no further questions or comments for Turkey Point Units 3 and 4.
