The enclosed Licensee Event Report (LER) 2011-001-00 is submitted in accordance with 10 CFR 50.73, paragraph (a)(2)(i)(B) as an event that resulted in a condition prohibited by plant Technical Specifications. This report describes an event in which a failed safety-related tornado damper caused the Containment Vacuum Relief system to be inoperable. In accordance with 10 CFR 50.73(a) requirements, this LER is submitted within 60 days following discovery of the event.

This document contains no Regulatory Commitments.

There are no previous similar events at Harris Nuclear Plant.

IMMEDIATE CORRECTIVE ACTION

Corrective maintenance was performed and post-maintenance testing on CB-Z2SN was completed at approximately 3:37 AM on April 13, 2011, and the "B" Train Containment Vacuum Relief system was declared operable. The other tornado dampers have been inspected and repaired as needed. Inspections and repairs were completed by April 19, 2011.

CORRECTIVE ACTIONS

The planned corrective action to prevent recurrence is to establish a new procedure just for tornado damper maintenance and testing. The new written procedure will include step-by-step instructions for re-validating counterweight set-up and performing functional testing per technical manual VM-NFZ, "Tornado Protection Dampers," and will clearly document "as found" and "as left" conditions, clearly document that no binding exists with independent verification, and include sign-offs at a minimum. Blade seal replacement will also be included in the new procedure.
