DATE OF OCCURRENCE: November 9,1983 IDENTIFICATION OF OCCURRENCE:
' One ' safety-related hydraulle snubber, MSH-252, was inoperable on November 9, 1983, contrary to Technical Specification 3.7.9.1.
CONDITIONS PRIOR TO OCCURRENCE:
Mode 1,97% FP DESCRIPTION OF OCCURRENCE:
On November 9,1983, during routine visual inspection, the sightglass for snubber MSH-252 (Main Steam System) indicated no oil level. It was also observed that the snubber had rotated to another position. The snubber MSH-252, was declared inoperable at 1400 and maintenance was initiated.
ANALYSIS OF OCCURRENCE:
There was.no significant effect on public health and safety as a result of this
- i. . occurrence since the probability of seismic events in the peninsula'of Florida is
- . very low and considerable safety margin is maintained in piping stress analyses.
DESIGNATION OF APPARENT CAUSE:
This event was caused by a loose jam-nut on the stationary end of the snubber which allowed the snubber to rotate. As the snubber rotated, the fluid drained out.
. CORRECTIVE ACTION:
! MSH-232 was replaced with a spare snubber. Operability was restored on l November 10,1983 at 1800.
' Procedure MP-130, Pipe Snubber Maintenance, will be revised to ensure proper snubber orientation is maintained.
' FAILURE DATA:
This is the ninth occurrence reported under Technical Specification 3.7.9.1.
