Occurrence Date: 18 December 1978 Id:ntification of Occurrence:

Inoperability of Auxiliary Building ventilation fan AEF-14C contrary to Technical Specification 3.7.8.1.

Conditions Prior to Occurrence:

Mode 1 power operation (98*).

Description of Occurrence:

i At 0600, during routine operation of AHF-14A and AEF-14C, it was discovered that AHF-14C was overloaded and secured to attempt to balance fan air flow.

Redundant fans AHF-14B and AHF-14D are out of service for maintenance. AHF-14C tripped during the restart attempt. AHF-14C was restarted and returned to service at 0610.

Dasignation of Apparent Cause:

The cause of the event was attributed to not allowing sufficient time for the overload condition to reset before attempting to restart AHF-14C.

Analysis of Occurrence:

There was no safety hazard to the plant or general public. AHE-14A remained operational and AHF-14C was promptly returned to service at 0610.

Operations personnel were instructed in fan operating characteristics.

Failura Data:

Five previous reportable occurrences, but first occurrence of this type.
