| Description | Technical Specification 3.4.16 required RCS leakage detection instrumentation (one containment sump A monitor and one containment radionuclide monitor) to be operable during Modes 1 through 4. Technical Specification 3.4.16, Condition D, required that Limiting Condition for Operation 3.0.3 be entered immediately when all required monitors were inoperable. As discussed in Section 4OA3.3 of this report and CAP 1300034, the containment radionuclide radiation monitor and both containment sump A monitors were inoperable on August 18 and 19, 2011. The failure to comply with TS 3.4.16 was determined to be a performance deficiency as defined in IMC 0612, Power Reactor Inspection Reports, Appendix B, Issue Screening. The inspectors determined this issue was more than minor because it was associated with the configuration control attribute of the Barrier Integrity Cornerstone and impacted the cornerstone objective of providing reasonable assurance that physical design barriers protect the public from radionuclide releases caused by accidents or events. The inspectors determined this finding was of very low safety significance because other instrumentation was available to provide indications of RCS leakage. Corrective actions included revising procedures to ensure that complex electrical equipment isolations were appropriately developed and approved. |
