DESCRIPTION OF OCCURRENCE:
At 0730 hours0.00845 days <br />0.203 hours <br />0.00121 weeks <br />2.77765e-4 months <br />, during routine rounds, it was observed that water was in the gauge glass of No. 11 Safety Injection Pump lube oil cooler.
Action Statement 3.4.2.a was entered which required if one ECCS subsystem is inoperable, it must be restored to operable status within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> or be in hot shutdown within the.next 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.
DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:
Cooler Tube Failure ANALYSIS OF OCCURRENCE:
The Safety Injection Pump was removed from service and a pressure test performed to determine the cause of the water in-leakage.
It was determined a tube had failed.
CORRECTIVE ACTION
The failed tube was plugged and pressure tested satisfactorily.
The Safety Injection Pump was proved operable and the Action Statement terminated at 2125 hours0.0246 days <br />0.59 hours <br />0.00351 weeks <br />8.085625e-4 months <br />.
Because of the two recent failures of lube oil to service water coolers, a pressure test will be conducted on the other Safety Injection Pump and both Centrifugal Charging Pumps, which have similar type coolers, in the current outage to insure their integrity.
