It is postulated that the valve was lifted duetto improper initial setting of the test pressure and failed to reset due 4
to buildup of sodium pentaborate on the valve disc. A pro-cedure change has been issued cautioning the operator about care fu setting the pressure in future tests.

.inoperability of the Station swing Diesel due to a blown fuse ACRS in the Control Circuitry. The failure occurred during the period between June 29, the last test of the engine, and July 1, when the condition was discovered. The fuse was found to have blown due to arcing at a defective indicator light socket.

A modification which would provide an alarm on a Control Circuit point failure is being considered.
