  • Attachment ST-HL-AE-4299 Page 1 of 2 South Texas Project Unit 1 Docket No. STN 50-498 Special Report Regarding the Seismic Monitorinci Syst;em Being Inopfrable for Greater than 30 Days DESCRTETIj2N OF EVE 11T Technical Specification 3.3.3.3 requires that with one or more of the Seismic Monitoring System instruments inoperable for more than _

30 days, a special report be written within the next 10 days outlining the cause of the malfunction and the plans for restoring the instrument (s) to operable status. This report fulfills that requirement.

On November 28, 1992, at 0628 hours0.00727 days <br />0.174 hours <br />0.00104 weeks <br />2.38954e-4 months <br /> while Unit 1 was in a refueling outage, surveillance testing was started on the Seismic Monitoring System. The Seismic Monitoring System was declared inoperable and placed in the out of service log. The surveillance test requires several days to perform.

During the performance of this surveillance test the following hardware problems were identified:

  • While performing the calibration of the triaxial accelerometer, the backup batteries were discharged. The batteries were recharged and replaced as necessary.
  • The Digital Cassette Accelerograph (DCA) analog trigger card was found to be out of tolerance and would not adjust. The trigger card was replaced.
  • A problem with the seismic trigger and the master relay was encountered which was determined to be a faulty thumbwheel and toggle switch board. These were subsequently replaced.

. A broken wire was found on the regulator board of the master DCA, resulting in a failure of the cassette tape deck. The wire was repaired and the trigger card was found to be within tolerance limits.

  • The playback response test was not working due to the failure of two master logic cards were replaced.
  • A cassette deck was replaced on the DCA master due to intermittent operation.

.CAUSE OF MMZUECTION The Seismic Monitoring System was inoperabic for greater than 30 days as a result of failure to properly manage Technical Specification required equipment priorities. An evaluation is being performed to determine the cause for the failure to properly manage the initial problem and why there were numerous equipment problems. ,
