-SCENARIO REVIEW for-SOUTH TEXAS EXERCISE, JULY 13, 1988 Major Deficiencies None Minor Deficiencies

1. The plant parameters data provided do not appear complete enough to allow operators to' follow the' event sequence and demonstrate more than.the'most general understanding of'the accident.

No status of operating equipment-is provided (pumps.run'ning, etc.)

No list of key annunciators and alarms received or cleared is provided.

g' Between 10:35 and 11:15 no new data is provided.

After 11:15 no data is provided.

2. The Narrative Summary (p. 5.1.1) states that a " double ended shear of the pump suction line of RCS Loop 'C' occurs". . The Radiological Narrative Summary (p. 5.9.1) describes the failure as a " guillotine break". This discrepancy could lead to confusion on planning corrective actions.
3. Radiation Monitors /Onsite Survey (5.9.2) ' Airborne monitor readings with an
  • indicate " readings are as indicated on instrument".: Several columns have blank spaces with no explanation of what this means.
4. Radiation Monitors /0nsite Survey (Section 5.9.2, Room 326) - There are two purge line columns on this data sheet. The
  • indicates an 'as read' condition, however, one column has dose rates up to 100 R/hr, and the other is blank. This data sheet should be clarified.
5. Radiction Monitors /0nsite Survey (5.9.2) - No units are provided on the following'inplant data sheets:
