Technicial Specification 3.7.11;. Fire Barrier Impaired For Greater Than Seven (7) Days Event Date(s): bl/12/93 Report Date: 02/04/93 This report was initiated by Incident Report No.93-025. Per Technical Specification Action Statement 3. 7 .11._a,. a Special Report is required when fire barrier penetrations are impaired for greater than seven (7) days.* This report satisfies that requirement.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 Reactor Power 100% - Load 1140 MWe DESCRIPTION OF OCCURRENCE:

On January 4, 1993, during normal power operation, a fire barrier penetration seal (the equipment hatch associated with F-25305-007) was impaired by maintenance personnel in support of planned work (i.e., welding of Residual Heat Removal System drain piping).

Welding cables had been run through an access door in the hatch.

Work was completed on January 15, 1993 at which time the cables were removed. During the period of penetration impairment, an hourly fire watch patrol was in place, per Technical Specification 3~7.lf Action "a" .

. The hatch is a steel plate which is*an acceptable fire barrier due to the limited fire loading of .the respective fire areas. However, by running cable through the hatch, a transfer medium was provided for fire propagation between the 55 1 Elev~tion and 64' El~vation fire areas, thereby impairing the fire barrier.

Technical Specification 3.7.11 states:

"All* fire barrier penetrations (including cable penetration barriers, fire doors and fire dampers), in fire zone bounda~ies, P.rotecting safety related areas shall be OPERABLE."

Action Statement 3.7.11.a states:

"With one or more of the above required fire barrier penetrations inoperable, wi~hin one hour either establish a continuous fire watch on at least one side of the affected penetration, or verify the OPERABILITY of fire detectors on at

_least one side of _the inoperable fire barrier and establish an hourly fire watch patrol. Restore the inoperable fire barrier penetration(s) to OPERABLE status within 7 days or, in lieu of

taken, the cause qf the inoperable penetration and plans_ and schedule for restoring the fire barrier penetration(s) to OPERABLE status."

APPARENT CAUSE OF OCCURRENCE:

The fire barrier penetration impairment exceeded seven (7) days as part of a planned effort.

ANALYSIS OF OCCURRENCE:

The functional integrity of fire barrier penetrations ensures that fires will be prevented from spreading to adjacent portions of the facility. This design feature minimizes the possibility of a single fire involving several areas of the facility. The fire barrier penetration seals are a passive element in the facility fire protection .program and are subject to periodic inspections. This report satisfies reporting requirements of Technical Specification Action Statement 3.7.11.a, pursuant to Technical Specification 6.9.2 since the time between impairment and eventual repair of the fire barrier penetration was greater than seven (7) days.

  • Appropriate remedial actions were already in place, in accordance with the requirements of Technical Specification Action Statement 3.7.11.a, to establjsh an hourly roving fire watch patrol for impaired fire barriers.

The subject fire areas contain detection devices in addition to the roving fire watch patrol. Therefore, a fire in the areas should be detected before it could involve an adjacent area. This occurrence therefore invcilyed no undue risk to the health or safety o{ the public.

CORRECTIVE ACTION:

The repair of the planned impaired penetration was not accomplished within seven (7) days due to the time required to complete work. -The cables have since been removed and the access hatch closed restoring the penetration to an operable status; 7£~
