SALEM GENERATING STATION LICENSE NO. DPR-70 DOCKET NO. 50-272 UNIT NO. 1 SPECIAL REPORT 92-6 August 21, 1992 This report addresses the inoperability of the R45 Radiation Monitoring System channels for greater than seven (7) days.

This report has been prepared in accordance with the reporting requirements of Technical Specification 3.3.3.1.b Table 3.3-6 Action 23 pursuant to Technical Specification 6.9.2.

Radiation Monitoring Channels 1R45B and 1R45C inoperable for greater than seven days due to equipment failure Event Date(s):

8/21/92 This report was initiated by Incident Report No.92-512.

CONDITIONS PRIOR TO OCCURRENCE:

8/7/1992:

Mode 3 (Hot Standby) 8/14/1992: Mode 2 (Startup)

DESCRIPTION OF OCCURRENCE:

On August 7, 1992 at 0545 hours0.00631 days <br />0.151 hours <br />9.011243e-4 weeks <br />2.073725e-4 months <br />, Radiation Monitoring System (RMS) channels 1R45B and 1R45C were declared inoperable and were removed from service.

The sample pump was running but there was no indication of air flow.

one sample pump is required for system operability.

The backup sample pump was also inoperable.

since the R45 RMS channels have not been returned to service within seven days, this Special Report is required in accordance with Technical Specification 3.3.3.1.b Table 3.3-6 Action 23 which states:

"With the number of OPERABLE channels less than required by the Minimum Channels OPERABLE requirements, initiate the preplanned alternate method of monitoring the appropriate parameter(s)~

within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />, and:

1.

either restore the inoperable Channel(s) to OPERABLE status within 7 days of the event, or

2.

prepare and submit a Special Report to the commission pursuant to Specification 6.9.2 within 14 days following the event outlining the action taken, the cause of the inoperability and the plans and schedule for restoring the system to OPERABLE status."

APPARENT CAUSE OF OCCURRENCE:

The root cause of this event is attributed to equipment failure.

The impellers of the two (2) sample pumps were found degraded.

The cause of the impeller failure is under investigation.

Rebuild kits have been ordered and upon receipt of new parts, the sample pumps will be reassembled, tested for required flow and the system returned to service.

ANALYSIS OF OCCURRENCE:

The 1R45B and 1R45C channels monitor the Plant Vent radioactive noble gas releases via representative sampling.

The 1R45B monitor is a medium range monitor and the 1R45C channel is a high range monitor.

They are both required in operating modes 1 -

4 (power operation to Hot Shutdown).

Their purpose is to monitor significant releases of radioactive materials in compliance with the Updated Final Safety Analysis Report (UFSAR) and federal regulation.

The sample pumps are manufactured by Gast Manufacturing Company and purchased through Eberline Instrument Co.

CORRECTIVE ACTION:

Upon completion of the repairs to the sample pumps, they will be tested.

A preventive maintenance (PM) applicability review of the sample pumps will be performed to determine if there are PM activities that can be applied.

The activites, as applicable, will be incorporated into the PM program.

The timeliness of the actions to restore the inoperable sample pump(s) will be reviewed.
