DPR-75 DOCKET NO. 50-311 UNIT NO. 2 SPECIAL REPORT 92-9 This Special Report addresses the circumstances surrounding the inoperability of the 2R46C Radiation Monitoring System (RMS) channel for greater than seven (7) days. This report is submitted in accordance with the requirements of Technical Specification 3.3.3.1 Table 3.3-6 Action 26 pursuant to Technical Specification 6.9.2. It is required within fourteen {14) days from when the RMS channel failed.

Radiation Monitoring Channel 2R46C inoperable for greater than seven days.

Event Date(s): 12/15/92 Report Date:. 12/21/92 This.report was initiated by Incident Report No.92-830.

The 2R46C main steamline Radiation Monitoring System (RMS) channel has been inoperable for greater than seven days. This Special Report is required *in accordance with Technical Specification 3.3.3.1.b

  • Table 3. 3-6 Action 26 which states: * *

"With the number of OPERABLE channels less than required by the Minimum Channels OPERABLE requirements, initiate the preplanned alternate method of monitoring the appropriate parameter(s),

1. either restore the inoperable Channel(s) to OPERABLE status within 7 days of the event, or *
2. prepare a_nd submit. a Special 'Report to the Commission pursuant to Specification 6.9.2 within 14 days following the event outlining the action taken, the cause of the inoperability and the plans and schedule for restoring the system to OPERABLE status.

CONDITIONS PRIOR TO OCCURRENCE:

DESCRIPTION OF OCCURRENCE:

On December 8, 1992 at 1033 hours0.012 days <br />0.287 hours <br />0.00171 weeks <br />3.930565e-4 months <br />, the 2R46A RMS channel functional surveillance was initiated. Technical Specification 3.3.3~1.b Table 3.3-6 Action 26 was entered at this time. As part of the functional surveillance, the Chilled water system supply is isolated. Chilled Water is used to protect the RMS channel detectors from excessive

. main steam heat. Per design, the main steam supply valves to the four (4) 2R46 RMS channel detectors (A,B, c and D)* close upon loss of Chilled Water System supply.

Upon completion of the 2R46A RMS channel functional surveillance, the

Chilled Water supply could not be restored. The control and indication panel circuit fuse, for the Chilled water 2R46 supply valve, had blown. Investigation revealed that the 2R46C RMS channel solenoid valve coil had shorted. The coil was isolated and the fuse replaced. The other 2R46 RMS channe1s*were then *restored to.service

  • leaving the 2R46C RMS channel inoperable. Also, upon removal of the solepoid valve, the piston rod link was found broken where.- it attaches to the piston assembly. This break would prevent the valve from opening thereby preventing main steam sample flow to the 2R46C.

detector. It cannot be determined when the break occurred.

The solenoid valve is an Atkomatic Solenoid Valve Model #41840-361, Size 1.

APPARENT CAUSE OF OCCURRENCE:

The cause of the 2R46C RMS channel failure is equipment failure.

Investigation, to determine the specific causal factors for the failed solenoid valve operator coil and the break located where the piston rod link attaches to the piston assembly, is continuing.

The 2R46C RMS, channel has been inoperable past seven (7) days due to the unavailability of replacement parts. Upo'n receipt* of a new solenoid valve, the 2R46C RMS channel will be returned to service.

ANALYSIS OF OCCURRENCE:

The 2R46 A-E RMS channels are used to monitor main steam line radioactivity *. This is done to identify significant-primary to secondary leakage. They provide alarm indication. They do_not provide any interlocking functions. The 2R19 steam Generator Blowdown RMS channels and the 2R15 RMS air ejector channel corroborate the 2R46 channels' .indications. Therefore, with the availability of the corroborating channels, this event did not affect the health or safety of the public.

CORRECTIVE ACTION:

A new solenoid valve operator has been ordered. Upon receipt it will

,be installed* and.the 2R46C channel returned to service.

Investigation to determine the specific c"ausal factors for the failed solenoid valve operator coil and the break located where the piston rod link attaches to the piston rod is continuing. Upon c_ompletion, appropriate corrective action .will be taken.

System Engineering is reviewing what appropriate corrective action will be implemented to assist.identification of solenoid valve failure .in the future.
