This report addresses Salem Unit 1 and Salem Unit_2-fire barrier penetration seal impairments which have not been restbred to functional status within seven (7) days.
These penetrations have been impaired in support of-planned work.
This report has been-prepared in accordance with.the reporting requirements of Technical Spe~ification Action Statement 3.7.ll:a pursuant to Technical Specification 6.9.2.

.Technibal Specification 3.7.11; Fire Barriers Impaired For Greater Than Seven (7) Days in Support of Planned Work Event Dates:

CONDITIONS PRIOR TO OCCURRENCE:

~~SCRIPTION OF OCCURRENCE:
Per Technical Specification Action Statement 3~7.11.a (for both salem Unit 1 and Salem Unit 2), a Special Report is re~uired when fiie ba~rier penetration seals are dmpaired for greater than seven (7) days.
This report satisfies t~is requirement.
The subject fire parrier impairments were initiated in support of design changes and other Salem Unit 2 outage activities.. Normally, the planning of penetration impairments includes their repair within seven (7) days.
However, to fully support the work identified in this report, the penetrations could not be repaired within seven (7) days.
On January 4, 1992, two (2) fire barrier penetrations were impaired in support of power cabl~ routing ~equired for Service Water System modifications.
On January 6, 1992, one (1) penetration was impaired in support of Service Water System modifications.
On January 25, 1992, one (1) penetration was impaired to support the Radiation Monitoring System upgrade project (DCP 2SC-2229).
These penetrations are located as follows:

(cont 'd) above fire areas due to other fire protection concerns.
Therefore, the requirements of Technical Specification Action Statement 3.7.11.a, were met.
Salem Unit 2 Technical Specification 3.7.11 states:
"All fire penetrations (including cable penetration barriers, fire doors and fire dampets), in fire zorie boundaries, protec~ting safety related areas shall. be OPERABLE."
Salem Unit 2 Action Statement 3.7.11.a states:
"With one (1) or ~ore of t~e abov~ required ~iie barrier
'penetrations inoperable, within one (1) hour either establish a continuous fire watch on at least one (1) side of the affected penetration, or verity the OPERABILITY of.fire detectors on at least one (1) side of the inoperable fire barrier and establish
.an hourly fire watch patrol.
Restore the inoperable fire barrier penetration(s). to OPERABLE status within 7 days or, in iieu rif any other report requiied by Specification 6.9.1, prepare and submit a Special Report to the Commission pursuant to Specification 6.9.2 within the next (30) days outlining the action taken, the cause of the inoperable penetration and plans and schedule for restor~ng the fire barrier penetration{s) to OPERABLE status."
The Sa*lem Unit 1 Technical Specifications are identical to the above Unit 2 Technical Specifications; however,. the term "functional" is used instead of the term "OPERABLE".
APPARENT CAUSE OF OCCURRENCE:
The cause *of the fire barrier.penetration impairments is to support outage related activities.
ANALYSIS OF OC~Q~~E~~~:
The *functional integrity of fire barrier penetrations ensu~es that fires will be confined or adequately retarded from spreading to adjacent portions of the facility.
This design feature minimizes the possibility of a single fire involving seve.ral areas of the facility.
The fire barrier penetration se~ls are a passive element
- in the facility fire protection program and are subject to periodic
,inspections.
This report satisfies reporting requirements of Technical Specification Action Statement 3~7.11.a,.pursuant to Technical* Specification 6.9.2 sine~ the time between discovery and' eventual repair of the fire barrier impairments is greater than seven (7) days.
Appropriate remedial actions were already in place in accordance with the requirements of Technical Specification Action Statemeni 3.7.11.a to establish an hourly roving fire watch patrol for the impaired fire barriers once the impairments were identified.*
The stibject fire areas contain det~ction devices in addition to the roving fire watch patrol.
Therefore, a fire in the areas should be detected before it could involve an adjacent are~. This occurrence therefore involved no undue risk to the health rir safety of the public.

-: CORRECTIVE ACTION:.
The-repair of the planned impai~ed penetrations was n6t accomplished

The Unit 1 penetrat'ions have been resealed as of January 12, 1992 at 2300 hours0.0266 days <br />0.639 hours <br />0.0038 weeks <br />8.7515e-4 months <br />.
The Unit 2 penetrations will be sealed upon completion -0f associated work.
The hourly fire watch, as addressed in the D~scription of Occurrence section, will continue until all fire protection concerns asso~iated with these areas are resolved.
