Title | Failure to Implement Compensatory Measures for a Risk-Management Activity |
Description | The inspectors identified a finding of very low safety significance and anassociated NCV of 10 CFR 50.65(a)(4) for failure to assess and manage the riskassociated with maintenance activity affecting the low pressure core spray system.Specifically, the licensee removed floor plugs in the auxiliary building and failed toimplement risk control measures to assure operability of low pressure core spray. Aspart of their immediate corrective actions, the licensee personnel re-installed buildingfloor plugs and returned low pressure core spray to an operable status.The finding was considered more than minor because the licensee failed to prescribesignificant compensatory measures for external conditions; and if the practice were leftuncorrected, the issue would become a more significant safety concern. The findingwas of very low safety significance because the incremental core damage frequencyassociated with the activity was less than 1 X 10-6. This finding has a cross-cuttingaspect in the area of Human Performance as defined in IMC 0305 H.3(a), because theorganization failed to adequately plan work activities that are associated with risk.(Section 1R13.1 |
