| Title | Failure to Periodically Test Reactor Protection system Key Lock Bypass Switches |
| Description | A finding of very low safety significance and associated NCV of 10 CFR Part 50, Appendix B, Criterion XI, Test Control, was identified by the inspectors for the failure to test reactor protection system key locked bypass switches. The licensee entered this issue into its corrective action program and initiated procedural changes to require periodic testing of the RPS bypass switches. The finding was determined to be more than minor because it affected the cornerstone objective of ensuring the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences. Specifically, the bypass circuitry needs to be tested periodically to verify that it remains functional for use when required. The finding screened as Green because it was a design deficiency that did not result in actual loss of safety function. This finding does not have a cross-cutting aspect because it is not indicative of current performance. (Section 1R21.5.b(1) |
