| Title | Adequacy of Airlock Ball Valve Maintenance (4OA2.3) |
| Description | Green. A self-revealed finding of very low safety significance and an associated NCV of10 CFR Part 50 Appendix B, Criterion 5 , Procedures, was identified on June 1, 2008,when a containment airlock door seal failed during routine operations. OnMarch 26, 2008, the licensee failed to implement airlock maintenance proceduresappropriate to the circumstances and this led to a failure of the containment upperairlock outer door seal. As part of their corrective actions, the licensee (1) conductedworker training; (2) planned to revise the airlock maintenance procedures to includeadditional guidance; (3) planned to increase maintenance frequency for the airlocks; and(4) planned to reintroduce a requirement to grease the door mechanisms.The finding was determined to be more than minor because it was associated with theProcedure Quality attribute of the Barrier Integrity Cornerstone and affected thecornerstone objective of providing reasonable assurance that physical design barriersprotect the public from radionuclide releases caused by accidents or events. Theinspectors determined that the finding was of very low safety significance because theupper airlock inner door remained closed and the finding did not represent an actualopen pathway in the physical integrity of reactor containment. This finding has a crosscuttingaspect in the area of Human Performance as defined in IMC 0305, H.2(c),Resources, because the licensee did not ensure that procedures were complete andwere adequate to assure nuclear safety.
