GrandGulf NuclearStation(GGNS) discoveredthatoneofthestepsrequiredforrestorationoftheOffgasPost Treatment Radiation MonitorwasnotcompletedinMarchof2015.Grabsamplingoftheprocessstreamwas terminatedwhentheLimitingConditionfor Operation(LCO)wasimproperly exited.Consequently,thesitefailedtomeetthesampling requirementsofOffsiteDose CalculationManual(ODCM)6.3.10, "Radioactive GaseousEffluentMonitoring Instrumentation,"foraperiodof4days,9hours.Duringthisperiod,the monitorprovidedcontinuousand accuratemeasurementoftheprocessstreamandthecontrolroomalarmwasfunctional,withnoalarmsreceived.Whentheerrorwasidentified,theLCOActionwasenteredandgrabsamplingoftheprocessstreamwasinitiateduntilthe monitorwasreturnedtooperablestatus.Therewereno unmonitored gaseouseffluentreleasestothe environmentduringthisperiod.Theincidentis documentedintheGGNS CorrectiveActionProgramandistobeincludedintheAnnual Radioactive EffluentReleaseReport(ARERR).ThisomissioniscorrectedinTable6A-Supplemental Information,includedinitsentiretyas AttachmentIItotheamended2015ARERR (Attachment1tothisletter).Also,inFebruaryof2017,GGNS identified anerrorthatoccurredduringasoftwareupgradetotheradwasteshippingdatabase.Some documentation wasinadvertentlyomitted.ThisiscorrectedbyTable3-Supplemental Information,alsoincludedinitsentiretyas Attachment IItotherevised2015ARERR (Attachment1tothisletter).
