This Supplemental Licensee Event Report entitled "Failure to Plug Steam Generator Tubes due to Missed Eddy Current Indications" is being submitted pursuant to the requirements of the Code of Federal Regulations

        • 1OCFR50.73 (a)(2)(i)(B)****.

The supplement is being submitted to discuss the cause and safety significance of the event. When the original LER was submitted, Public Service Electric and Gas (PSE&G) anticipated that a supplement would be issued by March 29, 1996. However, during the first quarter of 1996, due to the condition of the steam generators, all Unit 1 work was suspended while PSE&G determined the proper strategy.

Focus was shifted to Unit 2 which became the lead outage unit (i.e. first to return to service).

On May 16, 1996, PSE&G announced its intent to replace the Salem Unit No. 1 steam generators prior to the restart of the unit. The replacement was essentially complete by mid-1997.

By letter (LR-N96219) dated August 12, 1996 PSE&G notified the NRC that steam generator tube analysis, and testing on the old steam generators was being suspended.

In light of the commitment to replace the generators, further financial expenditures and additional man-rem burden on site personnel resulting from these activities was no longer warranted.

Because of the change in focus to Unit 2 as the lead outage unit, and the enormous work load associated with the expeditious replacement of the steam generators on Unit 9801130415 971230
