Dear Sir or Madam:

By way of Reference 2, Entergy replied to the notice of violation described in Reference 1.Reference 2 contained one new commitment with a due date of September 28, 2015, as follows: "Develop case study on the lessons learned for not identifying the feedwater regulating valve seat leakage as an operator workaround.

Case study will be presented to the Condition Review Group (CRG) members, all Operations instructors, and at supervisor training." The case study has been developed, but presentation to the target audience has not been completed.

By way of this letter, Entergy is supplementing the initial response to the Notice of Violation in order to establish a new commitment due date of October 23, 2015. To account for this new date and the partially completed state of the originally proposed commitment, the revision is delineated in the Attachment to this letter.If you have any questions on this matter, please contact me at 225-381-4177.
