Enclosed is one copy of the Final After Action Report Improvement Plan (AAR/IP) for the August 21, 2012, Radiological Emergency Preparedness (REP) Partial Participation Plume Exposure Pathway Exercise and the September 6, 2012, Medical Services (MS-i) Drill for the Fermi 2 Nuclear Power Plant. Under separate cover, four copies of this report are being sent to the State of Michigan for distribution to the Counties of Monroe and Wayne along with an additional copy for the state.

The State of Michigan, Monroe and Wayne Counties, and the utility owner/operator, Detroit Edison Company, participated in this exercise. The AAR/IP was prepared by the U.S. Department of Homeland Security/Federal Emergency Management Agency (DHS/FEMA) Region V, Radiological Emergency Preparedness Program.

There were no Deficiencies identified for the State of Michigan or the Counties of Monroe and Wayne during this exercise.

There were no Areas Requiring Correction Action (ARCAs) identified for during this exercise for the State of Michigan or Wayne County.

There was one ARCA identified during this exercise for Monroe County.

The ARCA for Monroe County was identified under Criterion 5.a.1 - Activities associated with primary alerting and notification of the public are completed in a timely manner following the initial decision by authorized offsite emergency officials to notify the public of an emergency situation, whereby the initial instructional message to the public did not include as a minimum the elements required by current REP guidance. This ARCA remains open.

There were no new Plan Issues identified during this exercise for the State of Michigan or the Counties of Monroe and Wayne.

There was one ARCA from a previous exercise for Wayne County which was resolved.

NRC Headquarters Document Control Desk Page 2 The ARCA' for Wayne County was identified under Criterion 3.b. 1 - Potassium Iodide (KI) and appropriate instructions are available should a decision to recommend use of KI be made.

Appropriate record keeping of the administration of KI for emergency workers and institutionalized individuals (not the general public) is maintained, whereby the Health Operations Officer instructed TACP personnel to "run the course" when taking KI; i.e., to take all of the 14 tablets (one per 24-hour period) contained in the KI bottle issued to them once they start taking KI. This ARCA was successfully re-demonstrated.

With the publication of the REP Program Manual in October 2011 and subsequent update in April 2012, this ARCA would now be identified under Criterion 3.a. 1 - The Offsite Response Organizations (OROs) issue appropriate dosimetry, KI, and procedures, and manage radiological exposure to emergency workers in accordance with the plans/procedures. Emergency workers periodically and at the end of each mission read their dosimeters and record the readings on the appropriate exposure record or chart. OROs maintain appropriate record-keeping of the administration of KI to emergency workers.

There was one Plan Issue each from a previous exercise for the State of Michigan and the Counties of Monroe and Wayne which were resolved.

The Plan Issues from a previous exercise which were resolved by the State of Michigan and the Counties of Monroe and Wayne were identified under Criterion 5.a. 1 - Activities associated with primary alerting and notification of the public are completed in a timely manner, whereby the process between the State and Monroe and Wayne Counties appears to be cumbersome, and as a result, the time of the Protective Action Order at 1036 hours0.012 days <br />0.288 hours <br />0.00171 weeks <br />3.94198e-4 months <br />, through the completion of the EAS broadcast at 1102.hours, was 26 minutes. The various coordination steps between the State, two Counties, the EAS development, the delivery to the radio station and siren activation was not completed in a timely manner.

A detailed discussion of these issues can be found in Part IV of the Final AAR/IP Report.

Based on the results of the August 21, 2012, exercise and the September 6, 2012, drill, the offsite radiological emergency response plans and preparedness for the State of Michigan and affected local jurisdictions, site-specific to the Fermi 2 Nuclear Power Plant, can be implemented and are adequate to provide reasonable assurance that appropriate measures can be taken offsite to protect the health and safety of the public in the event of a radiological emergency at the site.

Therefore, the Title 44 CFR, Part 350, approval of the offsite radiological emergency response plans and preparedness for the State of Michigan site-specific to the Fermi 2 Nuclear Power Plant, granted on March 9, 1987, remains in effect.

SUMMARY

On August 21, 2012, a Radiological Emergency Preparedness (REP) Partial Participation Plume Exposure Pathway Exercise and on September 6, 2012, a Medical Services (MS-1) Drill were conducted in the 10-mile Emergency Planning Zone (EPZ) around the Fermi 2 Nuclear Power Plant (FNPP) by the U.S. Department of Homeland Security (DHS)/Federal Emergency Management Agency (FEMA) Region V. The purpose of this exercise was to assess the level of State and local preparedness in responding to a radiological emergency. This exercise was held in accordance with DHS/FEMA's policies and guidance concerning the exercise of State and local Radiological Emergency Response Plans (RERPs) and procedures.

The most recent exercise at this site was conducted on June 8, 2010. The qualifying emergency preparedness exercise was conducted on February 2, 1982.

The DHS/FEMA wishes to acknowledge the efforts of the many individuals who participated in this exercise. In the State of Michigan, the risk counties of Monroe and Wayne participated along with the State government.

Protecting the public health and safety is the full-time job of some of the exercise participants and an additional assigned responsibility for others. Still others have willingly sought this responsibility by volunteering to provide vital emergency services to their communities.

Cooperation and teamwork on the part of all the participants was evident during this exercise.

This Final After Action Report/Improvement Plan (AAR/IP) contains the evaluation of the biennial exercise and the evaluation of the following out-of-sequence interviews and activities:

There was one ARCA from a previous exercise for:Wayne County, 23-10-3b I-A-05, which was resolved.

CQNDITION: The Health Operations Officer instructed TACP personnel to "run the course" when taking KI; i.e., to take all of the 14.tablets (one perz24-hour period) contained in the, KI bottle issued to them once they start taking KI. However, the PATIENT PACKAGE INSERT for THRO-BLOCK warns that taking more KI "will not help you because the thyroid can "hold" only limited amounts of iodine. .Larger doses will increase the risk of side-effects. You will probably be told not to take the drug for more than 10 days." These instructions suggest .thit taking KI for a longer period than necessary may increase the risk of side effects. The KI RECORD CARD contains spaces for recording only 10 doses of KI, supporting the written instructions, and does not provide sufficient blocks for recording the taking of all 14 tablets issued.

CORRECTIVE ACTION DEMONSTRATED: The dosimetry briefing now states that the emergency worker should ingest only one tablet when ordered to do so by the Incident 7

After Action Report/Improvement Plan Enrico Fermi 2 Nuclear Power Plant Commander. Additionally, the emergency worker's KI Record Card now reflects that only 10 doses (vice 14) are issued. Instructions are for emergency workers to ingest one tablet daily until ordered to cease daily ingestion by the Incident Commander. The total dose was not to exceed 10 tablets.

INTRODUCTION - EXERCISE BASIS On December 7, 1979, the President directed FEMA to assume'the lead -responsibility,for all offsite nuclear planning and response. DHS/FEMA's activities are conducted pursuant to 44, Code of Federal Regulations (CFR) Parts 350 "Review and Approval of State and Local Radiological Emergency Plans and Preparedness", 351 "Radiological Emergency' Planning and Preparedness" and 352 "Commercial Nuclear Power Plants: Emergency Preparedness Planning."

These regulations are a key element in the Radiological Emergency Preparedness (REP) Program that was established following the Three Mile Island Nuclear Station accident in March 1979.

The Federal Code, 44 CFR 350 establishes the policies and procedures for DHS/FEMA's initial and continued approval of State and local governments radiological'emergency planning and preparedness for cormmercial nuclear power plants. This approval is contingent, in part, on State and local governments' participation in joint exercises with licensees.

  • Taking the lead in offsite emergency, planning and in the review and evaluation of RERPs and procedures developed by State and local governments;

- Determining whether such plans and procedures can be implemented on the basis of observation and evaluation of exercises of the plans and procedures conducted by State and local governments;

  • Responding to requests by the U.S. Nuclear Regulatory Commission (NRC) pursuant to the Memorandum of Understanding between the NRC and FEMA dated June 17, 1993 (Federal Register, Vol. 58, No. 176, September 14, 1993); and
  • Coordinating the activities of Federal agencies with responsibilities in the radiological emergency planning process:

- U.S. Department ofAgriculture;.

- U.S. Department of Commerce;

- U.S. Department of Energy;

- U.S. Department of Health and Human Services; . .

- U.S. Department of the Interior;

- U.S. Department of Transportation;

- U.S. Environmental Protection Agency;

- U.S. Food and Drug Administration; and.

- U.S. Nuclear, Regulatory Commission. -

Representatives of these agencies serve on the DHS/FEMA Region V Regional Assistance Committee (RAC), which is chaired by DHS/FEMA.,

Formal submission of the RERPs for the- Fermi 2 Nuclear Power Plant to FEMA Region V by the State of Michigan and involved local jurisdictions occurred on November 19, 1981. Formal approval of these RERPs was. granted by FEMA on March 9, 1987, under 44, CFR 350.

A REP Partial Participation Plume Exposure Pathway Exercise was conducted on August 21, 2012, and MS-1 Drill was conducted on September 6, 2012, and evaluated by DHS/FEMA to 9

After Action Report/Improvement Plan Enrico Fermi 2 Nuclear Power Plant assess the capabilities of State and local offsite emergency preparedness organizations in implementing their RERPs and procedures to protect the public health and safety during a radiological emergency involving the Fermi 2 Nuclear Power Plant.

The purpose of this exercise report is to present the exercise results and findings on the performance of the Offsite Response Organizations (ORO) during a simulated radiological 3 emergency.

The findings presented in this report are based on the evaluations of the Federal evaluation team, i with final determinations made by the DHS/FEMA Region V RAC Chairperson, and approved by the DHS/FEMA Headquarters.

The criteria utilized in the FEMA evaluation process are contained in: 3

  • NUREG-0654/FEMA-REP- 1, Rev. 1, "Criteria for Preparation and Evaluation of Radiological Emergency Response Plans and Preparedness in Support of Nuclear Power Plants," November l 1980;
  • FEMA*"Radiological Emergency Preparedness: Exercise Evaluation Methodology" as published in the FEMA REP Program Manual, dated April 2012.

Section 1 of this report, entitled "Exercise Overview", presents information pertaining to the team that planned and coordinated the exercise. This section also provides a listing of all participating jurisdictions and functional entities that were evaluated.

Section 2 of this report, entitled "Exercise Design Summary", contains.the purpose and design of the exercise, a description of the plume :pathway EPZ and presents basic information and data relevant to the exercise scenario.

Section 3 of this report, entitled "Analysis of Capabilities," presents detailed information on the demonstration of applicable exercise criteria at each jurisdiction or functional entity evaluated in a jurisdiction-based,.issues-only format. Section 4 of this report, entitled "Conclusion" presents the DHS/FEMA summary of overall exercise conduct and results, as evaluated against the requirements of 44 CFR 350.

EMERGENCY PLANNING ZONE (10-Mile EPZ) DESCRIPTION The FNPP is comprised of approximately 1,120 acres of land owned by :DTE Energy. The plant operator is the Detroit Edison Company. The site is bounded on the north by Swan Creek, on the east by Lake Erie, on the south by Pointe Aux Peaux Road, and on the west by Toll Road.

Entrance to the site is from Enrico Fermi Drive (a private road owned by Detroit Edison) to the west and from Pointe Aux Peaux Road to another private road owned by Detroit Edison to the.

south.

The plant is approximately six. miles northeast of Monroe, Michigan; 30 miles southwest of downtown Detroit, Michigan; and 25 miles northeast of downtown Toledo, Ohio. The latitude of the site is 410 57' 48" north and the longitude is 830 15' 3 1" west. Site elevations range from the level of Lake. Erie, on the eastern edge of the site, to -approximately 25 feet. abovethe lake level on the western edge of the site..... . * .

The 10-mile EPZ includes parts of Monroe and Wayne Counties in Michigan; parts of Lake Erie; and the southern tip of Essex County, (Bay Point) Canada. Most of the 10-mile EPZ lies within Monroe County. Coordinated efforts occur between the two, counties, which are divided into five Protective Action Areas. The following jurisdictions are located within the 10-mile EPZ: Ash Township., Carleton Village, Berlin Township-, Estral Beach Village, Exeter Township, South Rockwood Village, Frenchtown Township, Monroe City, Monroe :Township, Raisinville, Township, Brownstown Township, Gibraltar City, Flat Rock City, and Rockwood City.. The population in the 10-mile EPZ, based on the 2000 census is 88,897.

Land use within the EPZ is diverse. About 55 percent of the land- is farmland. The majority of agricultural land is located in Monroe County. The portion of Wayne County nearest to Detroit is a growing residential suburb. Twelve state parks and recreational areas and several light industries are located within the 10-mile EPZ.

After Action Report/Improvement Plan Enrico Fermi 2 Nuclear Power Plant There are three major roads within 10 miles of the plant, Interstates 75 and 275 and U.S. Route

24. Their closest approach to the plant is 4.1 miles, 4.2 miles and 5.8 miles, respectively. Five railroad lines operate within 10 miles of the plant. These include the Canadian National Railroad, the Norfolk Southern Railroad, the Chesapeake and Ohio Railroad, the Detroit Toledo and I Ironton Railroad and the Detroit and Toledo Shore Line Railroad. SUMMARY

I 2.1 Exercise Purpose and Design The DHS/FEMA Region V Office evaluated the Fermi 2 Nuclear Power Plant Radiological Emergency Preparedness (REP) Partial Participation Plume Exposure Pathway Exercise 3 conducted on August 21, 2012, and Medical Services (MS-i) Drill conducted on September 6, 2012, to assess the capabilities of local emergency preparedness organizations in implementing their Radiological Emergency Response Plans (RERP) and procedures to protect the public health and safety during a radiological emergency involving the Fermi 2 Nuclear Power Plant.

The purpose of this report is to present the results and findings on the performance of the Offsite Response Organizations (OROs) during a simulated radiological emergency.

2.2 Exercise Objectives, Capabilities and Activities 1 Exercise objectives and identified Capabilities/REP Criteria selected to be demonstrated are discussed in Appendix E "Exercise Plan."

This exercise scenario was submitted by the State of Michigan and DTE Energy and accepted by DHS/FEMA Region V on August 16, 2012.

During the exercise, in addition to information and data provided through the Fermi 2 Nuclear Power Plant onsite scenario, controllers from the State of Michigan provided "inject messages" containing scenario events and/or releivant data to those persons or locations who would normally receive notification of such events. These inject messages were the method used for invoking additional specific r'esponse actions by OROs.

After Action Report/Improvement Plan Enrico Fermi 2 Nuclear Power Plant SECTION 3: ANALYSIS OF CAPABILITIES I 3.1 Exercise Evaluation and Results Contained in this section are the results and findings of the evaluation of all jurisdictions and functional entities that participated in the August 21, 2012, REP Partial Participation Plume 3 Exposure Pathway Exercise and September 6, 2012, Medical Services (MS-1) Drill to test the offsite emergency response capabilities of State and local governments in the 10-mile EPZ surrounding the Fermi 2 Nuclear Power Plant.

Each jurisdiction and functional entity. was evaluated based on its demonstration of exercise 3 criteria delineated in the FEMA REP Program Manual, dated April 2012. Detailed information on the exercise criteria and the extent-of-play agreement used in this exercise are found in i Appendix E of this report.

Presented below are definitions of the terms used in this report relative to the criteria demonstration status:

- M - Met: The status-of a REP exercise Evaluation Area Criterion indicating that the participating ORO demonstrated all demonstration criteria for the Evaluation Area Criterion to i the level required in the extent-of-play agreement with no Deficiencies, Areas Requiring Corrective Action (ARCA) or Plan Issues assessed in the current exercise and no unresolved prior ARCAs.

PRIOR ISSUES - UNRESOLVED: None 3.3.1.2 State of Michigan - State Emergency Operations Center The State of Michigan Emergency Operations Center (SEOC) demonstrated the Target Capability of Emergency Operations Center Management including the use of effective procedures to alert, notify and mobilize emergency personnel and activate facilities in a timely manner. The State of Michigan provided sufficient multi-agency coordination for an incident at the Fermi 2 Nuclear Power Plant by promptly activating and operating for the duration of the incident. The SEOC was managed efficiently and effectively. The SEOC completed activation, notification and completed staffing to an operational level. The Governor's Authorized Representative provided solid management, direction and control using briefings. This allowed the staff of the SEOC to maintain a common operating picture and overall response to the event.

The SEOC manager coordinated with other jurisdictions to make decisions and ensure that appropriate actions were taken to protect the health and safety of the public.

CONDITION: The facsimile sent to Radio Station WJR did not include text of an EAS message or any instructions to broadcast an EAS message. Monroe County did not instruictWJR to simulate broadcast of the EAS message during. a telephone call to verify that the station received the EAS information.

The content of the initial EAS instructions to the public did not contain all required elements from the Radiological Emergency Preparedness (REP) Program Manual, dated April 2012 and contained inaccurate instructions. The initial EAS did not identify the off-site response organization (ORO) responsible or the official with l authority for providing the EAS. It did not reference public information materials for the general public or instruct the public to stay tuned for additional information. It did not clearly state that an emergency existed at Fermi 2 Nuclear Power Plant (FNPP), but did include FNPP on the PAA description with the evacuation box checked. The initial EAS message incorrectly indicated that the marine evacuation area was to two miles, instead of five miles as in the PAO, and incorrectly listed six Reception/Congregate Care Centers as being open.when there were only two open.

The second EAS messasge did not include the name of the official responsible for the EAS message nor did it include the organization he was.affiliated with. It also included information that the Dundee and IDA**Reception Centers should be evacuated instead of reiteraing that the residents should be evacuated from PAAs #1 and #3 as previously stated in this EAS message.

POSSIBLE CAUSE: The failure to simulate broadcast of the initial EAS message appears to be due to inadequate pre-exercise coordination with the radio station and the lack of instructions in the messages to the radio station during the exercise. Radio station personnel were not fully aware of the exercise or-what to do. with the information they received. The station personnel were under the impression that Monroe County would be activating the EAS and that the information Sent to the station was for information. The Monroe County EOC did not request EAS activation in the facsimile transmission to the station nor did they verbally request activation during a telephone call to verify that the station received the facsimile. The errors in the EAS content may be due to not properly checking the pre-scripted text in the 34 I I

broadcast an EAS message. Monroe County did not instruct WJR to simulate broadcast of the EAS message during a telephone call to verify that the station received the EAS information.

The content of the initial EAS instructions to the public did not contain all required elements from the Radiological Emergency Preparedness (REP) Program Manual, dated April 2012 and contained inaccurate instructions. The initial EAS did not identify the off-site response organization (ORO) responsible or the official with authority for providing the EAS. It did not reference public information materials for the general public or instruct the public to stay tuned for additional information. It did not clearly state that an emergency existed at Fermi 2 Nuclear Power Plant (FNPP), but did include FNPP on the PAA description with the evacuation box checked. The initial EAS message incorrectly indicated that the marine evacuation area was to two miles, instead of five miles as in the PAO, and incorrectly listed six Reception/Congregate Care Centers as being open when there were only two open.

The second EAS messasge did not include the name of the official responsible for the EAS message nor did it include the organization he was affiliated with. It also included information that the Dundee and IDA Reception Centers should be evacuated instead of reiteraing that the residents should be evacuated from PAAs #1 and #3 as previously stated in this EAS message.

RECOMMENDATION: Monroe County should review procedures for EAS message transmission to the radio station to insure that proper coordination is completed with the radio station. Pre-scripted EAS messages need to be reviewed prior to dissemination to insure all elements required are included.

CORRECTIVE ACTION DESCRIPTION: Clarify the responsibilities and procedures between Monroe County and the Radio Station WJR for EAS activation. Pre-scripted EAS messages should be re-evaluated to insure they contain the required elements as described in the REP Program Manual. In addition, review assigning someone to review the entire EAS message to insure that the information identified in the EAS message is correct at the time it is being given to the radio station for dissemination to the public.

After Action Report/Improvement Plan Enrico Fermi 2 Nuclear Power Plant APPENDIX E: EXERCISE PLAN This-appendix lists the exercise-criteria, Which Were scheduled for demonstration in the Fermi 2 Nuclear Power Plant Radiological Emergency Preparedness Partial Participation Plume Exposure Pathway Exercise on August 21, 2012,the Medical Services (MS-1) Drill on September 6, 2012, and the offsite extent-of-play agreement accepted by DHS/FEMA Region V on August 16, 2012.

The exercise criteria, contained in the FEMA REP Program Manual dated April 2012, represent a functional translation of the planning standards and evaluation criteria of NUREG-0654/FEMA-REP- 1, Rev 1, "Criteria for the Preparation and Evaluation of Radiological Emergency Response Plans and Preparedness in Support of Nuclear Power Plants," November 1980. Because the exercise criteria are intended for use at all nuclear power plant sites, and because of variations among offsite plans and procedures, an extent-of-play agreement is prepared by the State and accepted by DHS/FEMA to provide evaluators with guidance on expected actual demonstration of the criteria.

EVALUATION AREA 3 - PROTECTIVE ACTION IMPLEMENTATION Criterion 3.a.l: The Offsite Response Organizations (OROs) issue appropriate dosimetry and procedures, and manage radiological exposure to emergency workers inaccordance with the plan and procedures. Emergency workers periodically and at the end of each mission read their dosimeters and record the readings on the appropriate exposure record or chart.

The issuance of dosimetry and KI to Monroe Emergency Workers was successfully demonstrated on August 20th, 2012 and will not be evaluated. Similarly, the Issuance of dosimetry and KI to MDEQ staff was successfully demonstrated on August 20th, 2012 and will not be evaluated. .

For purposes of this exercise, if there is no medical need to bring equipment into and out of the treatment room, swabs will be taken at the injury locations and passed out of the room to demonstrate movement of equipment and, supplies into and out of the controlled area.

EVALUATION AREA 6.d - TRANSPORTATION AND TREATMENT OF CONTAMINATED INJURED INDIVIDUALS Criterion 6.d.1: The facility/ORO has theappropriate space, adequate resources, and trained personnel to provide transport, monitoring, decontamination, and medical services to contaminated injured individuals.

Monroe Community Ambulance will demonstrate the capability to transport a contaminated, injured individual to Mercy Memorial Hospital in Monroe. The ambulance crew will pick up a contaminated injured patient near the Fermi 2 Nuclear Plant (should inclement weather conditions exist, the Lead Controller may simulate the outdoor accident scene and begin the I

drill from the Monroe EOC Office Building. The ambulance crew will be met by Michigan Department of Environmental Quality (MDEQ) staff that will perform initial radiation i monitoring, and will provide information regarding contamination levels on the patient.

Monroe Community Ambulance will utilize universal precautions and good housekeeping practices to minimize the spread of contamination, and will focus on treating the patient's I medical condition.

After Action Report/Improvement Plan Enrico Fermi 2 Nuclear Power Plant Monroe Community Ambulance will call in the information regarding the patient to Mercy Memorial Hospital in Monroe so they can prepare f6r receipt of a contaminated patient.

MDEQ personnel will accompany the patient to the hospital along with the ambulance, bringing instrumentation to provide radiation readings and guidance to the hospital.

Mercy Memorial Hospital will implement their plan for receipt, isolation and treatment of an injured contaminated patient. Medical personnel will utilize universal precautions and good housekeeping practices to minimize the spread of contamination, and will focus on treating the patient's medical condition. Simple decontamination efforts will be demonstrated after the patient has been medically stabilized. MDEQ personnel will discuss the need to take additional samples for further radiological analysis. Hospital personnel will demonstrate their knowledge of who to call beyond MDEQ for assistance in Radiological Accidents, e.g.,

REAC/TS.

Another MDEQ staff member will be dispatched to .Mercy Memorial Hospital with radiation detection and measurement equipment in advance of the ambulance arriving.

The drill will conclude with the hospital representativ'e and MDEQ personnel supervising the removal of protective clothing and surveying of the emergency room and hospital personnel.

MDEQ will also advise on the proper procedure for release or disposal of contaminated material, Termination will b6 at the disciretion of the Lead Drill Controller based on successful demonstration of objectives.:.
