FERMI POWER PLANT, UNIT 2 - TRIENNIAL FIRE PROTECTION INSPECTION REPORT 05000341/2023011

Dear Peter Dietrich:

On March 9, 2023, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at Fermi Power Plant, Unit 2 and discussed the results of this inspection with Eric Olson, Site Vice President and other members of your staff. The results of this inspection are documented in the enclosed report.

No NRC identified or self-revealing findings were identified during this inspection.

A licensee-identified violation which was determined to be of very low safety significance is documented in this report. We are treating this violation as a non-cited violation (NCV)

consistent with Section 2.3.2 of the Enforcement Policy.

SUMMARY

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting a triennial fire protection inspection at Fermi Power Plant, Unit 2, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. INSPECTION SCOPES

Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

REACTOR SAFETY

71111.21N.05 - Fire Protection Team Inspection (FPTI) Structures, Systems, and Components (SSCs) Credited for Fire Prevention, Detection, Suppression, or Post-Fire Safe Shutdown Review (IP Section 03.01)

The inspectors verified that components and/or systems will function as required to support the credited functions stated for each sample. Additional inspection considerations are located in the fire hazards analysis (FHA) or safe shutdown analysis (SSA).

Fire Protection Program Administrative Controls (IP Section 03.02) (1 Sample)

The inspectors verified that the selected control or process is implemented in accordance with the licensees current licensing basis. If applicable, ensure that the licensees Fire Protection Program (FPP) contains adequate procedures to implement the selected administrative control. Verify that the selected administrative control meets the requirements of all committed industry standards.

(1) Combustible Control Program

Fire Protection Program Changes/Modifications (IP Section 03.03) (3 Samples)

The inspectors verified the following:

a.

Changes to the approved FPP do not constitute an adverse effect on the ability to safely shutdown.

b.

The adequacy of the design modification, if applicable.

c.

Assumptions and performance capability stated in the SSA have not been degraded through changes or modifications.

d.

The FPP documents, such as the Updated Final Safety Analysis Report, fire protection report, FHA, and SSA were updated consistent with the FPP or design change.

e.

Post-fire SSD operating procedures, such as abnormal operating procedures, affected by the modification were updated.

(2) EVAL-DE0035-01-Evaluation of Fermi 2 Wet Pipe Sprinkler Systems for Compliance with the Requirements of NFPA-13, Standard for the Installation of Sprinkler Systems.
(3) Update of FPEE-02-0020 for spatial and failure history of Steam Tunnel Seals.

INSPECTION RESULTS

Licensee-Identified Non-Cited Violation 71111.21N.05 This violation of very low safety significance was identified by the licensee and has been entered into the licensee corrective action program and is being treated as a non-cited violation, consistent with Section 2.3.2 of the Enforcement Policy.

Violation: Technical Specification Section 5.4.1.a states, in part, that written procedures shall be established, implemented, and maintained covering the applicable procedures recommended in Regulatory Guide 1.33, Revision 2, Appendix A, February 1978. NRC Regulatory Guide 1.33, Appendix A, Section 8 addresses Procedures for Control of Measuring and Test Equipment and for Surveillance Test, Procedures, and Calibration. Section 8 b 1 (h), further specifies, in part, that fire protection system functional test inspection procedures should be written for each surveillance, test, inspection, or calibration listed in the technical specifications. TRSM Section 3.12.2.13 Surveillance Requirements, requires the licensee to verify that each fire pump develops a minimum calculated value of 2500 gpm at 150 psig at the discharge flange of the pump adjusted for the minimum allowed lake level of 562-0.

Procedure 28.504.03 is the sites implementing procedure for the TRSM 3.12.2.13 required test. Contrary to the above, between 2001 and 2021, the licensee failed to maintain a written procedure associated with Fire Suppression Water System Simulated Automatic Actuation test. Specifically, the site failed to maintain procedure 28.504.03, Fire Suppression Water System Simulated Automatic Actuation test, because licensee failed to incorporate appropriate acceptance criteria for fire pump discharge flow and pressure. 28.504.03 contained the acceptance criteria for the fire pumps to be 1825 gpm at 149 psig resulted in insufficient automatic flow and pressure to be delivered to the Emergency Diesel Generating (EDG) Fuel Oil Storage Tank (FOST) rooms for fire suppression.
