I.-DCRIPTIONFTECHNICALPECIFICATINREUIREMENTGinnaStationTechnicalSpecification(TS)3.4.2.1requiresthatbothmotor-drivenAuxiliaryFeedwater(MDAFW)pumpsbeoperablewiththeRCStemperatureM350'F.ActionStatement3.4.2.1.astatesthatifoneMDAFWpumpisinoperable,thepumpmustberestoredtooperablestatuswithin7daysortheplantshallbeinatleastHotShutdownwithin6hoursandatanRCStemperature<350'Fwithinthefollowing6hours.2.DCRIPTINOFREOnAugust24,1994at0530hours,MDAFWPumpBwasremovedfromservicetoperformscheduledtestingandmaintenanceoftheassociatedservicewatercoolingsupplystrainerandtheAClubeoilpump(ProceduresEM-785andM-44.13,andWorkOrder&#xb9;19400838).ThemaintenanceactivitieswerecompletedonAugust25,1994andalimitedtestwassubsequentlyperformedonMDAFWPumpBbeginningat1430hourstoverifytheoperabilityoftheAClubeoilpumpbreaker.At1432hours,theoperatorsnoticedthatthenormallyclosedrecirculationlineair-operatedvalve(AOV4310)forMDAFWPumpBdidnotopenasrequired.ThepumpwasthensecuredandatroublecardwasinitiatedforAOV4310at1435hours.PreparationsweretheninitiatedtoperformtherepairofAOV4310.Thevalvewasrepairedandafullpumptest(ProcedurePT-16Q-B)wasthenperformedonAugust26,1994toverifypumpandvalveoperability.At2220hours,itwasdeterminedthattheMDAFWpumpdidnotmeetthe~prequirementsandatroublecardwasinitiatedforthepump.MajorrepairsonMDAFWPumpBweresubsequentlyinitiatedwhichincludedcompletedisassemblyofthepump.ItisnotedthatProcedurePT-16Q-BwassuccessfullyperformedonMDAFWPumpBonAugust18th.RGB'asconservativelyassumedthatMDAFWPumpBhasbeeninoperablesinceAugust24,1994at0530hourswheninitialworkactivitiesbeganandtheLCOwasentered.However,sincerepairactivitiesofthepumpwerenotinitiateduntillateonAugust26,1994,andduetotheleadtimeforreplacementpartsandtheneedtoperformpost-maintenancetesting,RG&Edoesnotanticipatethatthenecessaryrepairs'andtestingcanbeaccomplishedwithintheTSrequired7days.EnforcementDiscretionisthereforerequestedtograntRGB'elieffromcompliancewiththerequirementsofTS3.4.2.1.a.ThisEnforcementDiscretionisrequestedtobeginonAugust31,1994at0530hourswhenthecurrentLCOforTS3.4.2.l.awillexpire.TheEnforcementDiscretionisrequestedtoendonSeptember2at0800hourswhenallnecessaryrepairandtestingactivitieswillbecompleted.Thisisanextensionof50.5hours. 3..BASTFORRKTThereareatotaloffiveAFWpumpsinstalledatGinnaStation.ThepreferredAFWsystemhastwo100%capacityMDAFWpumpsandone200%capacityturbinedriven(TDAFW)pump.TheStandbyAuxiliaryFeedwater(SAFW)systemhastwo100%capacityMDAFWpumpswhicharemanuallystartedbytheoperatorsifnormalAFWislost(e.g.,highenergylinebreaksintheIntermediateBuilding).Thesafety-relatedfunctionoftheAFWsystematGinnaStationistomitigatethefollowingaccidentsorevents(UFSARSection10.5.2.1):a.b.C.d.e.f.g.h.Lossofmainfeedwater(MFW)withoffsitepoweravailableLossofMFWwithoutoffsitepoweravailableMFWlineruptureMainSteamlineruptureLossofallACpower(onsiteandoffsite)LOCASteamGeneratorTubeRupture(SGTR)CooldownItemscanddhavebeenevaluatedforGinnaStationassumingthatonlytheredundantSAFWsystemisavailable(UFSARSection15.2.7.3)whileitemeonlyrequirestheTDAFWpump(UFSARSection15.2.5).ItemsaandbareboundedbytheanalysisforthehighenergylinebreaksintheIntermediateBuilding(i.e.,itemscandd)duetotheavailablewaterinventoryintheintactSGs.TheSAFWsystemcanalsobeusedforcooldownevents(itemg)duetothetimewhichwouldbeavailableasaresultoflowdecayheatlevels.Consequently,onlyLOCAsandSGTRs(i.e.,itemsfandg)mustbeconsideredwithrespecttotheinoperableMDAFWpump.FollowingaLOCAorSGTR,thepreferredAFWsystemwillreceiveanautomaticstartsignaltoprovidecoolingwatertothesteamgenerators.However,forallLOCAs,boththeTDAFWpumpandMDAFWPumpAwillbeavailabletoprovidethenecessarycoolingwater.ThereisnosinglefailurewhichcanresultinthelossofbothofthesepumpsduringaLOCA.InthecaseofaSGTR,itcanbepostulatedthataruptureoccursinSGAwhichwouldresultintheisolationofautomaticAFWflowfromMDAFWPumpAandtheTDAFWpump(toS/GAonly).OnlyautomaticflowfromtheTDAFWpumpto'GBwouldbeavailableinthisinstance.OperatoractionisalreadyrequiredforaSGTReventtodepressurizetheRCS.Sincethistenminutedelayisalreadyassumedintheaccidentanalysis,itcanbeassumedthattimeisalsoavailabletousetheSAFWsystemorcross-tieMDAFWAtoSGBiftheTDAFWpumpweretofail.Inaddition,itisnotedthattheincreasedSGlevelintherupturedSGprovidesadditionaltimebeforeAFWwouldberequired. AreviewoftheGinnaStationPRAindicatesthatatuberuptureinSGAwithasubsequentfailureoftheTDAFWpump,thusrequiringtheneedforSAFW,isofverylowprobability(-1.4E-06fortheadditional2dayMDAFWPumpoutage).Inaddition,theAFWsystemisinstandbyduringnormalpoweroperations.ItisnotconsideredprudenttorequireaplantshutdownthroughmodesinwhichthepreferredAFWsystemisusedtoprovidenormalSG'coolingwaterneedswithoneMDAFWpumpinoperable.ThepotentialforplanttransientsandthesubsequentneedforAFWduringthesemodetransitionsisalsoincreased.MPENSATRYACTINRG&EwillnotperformanytestingormaintenancerelatedtotheremainingAFWandSAFWpumpsandtheirnecessarysupportsystemsduringtherequestedtwodayextension.Inaddition,operatorawarenessofRCSleakagehasbeenincreasedsinceleakagecanbeusedasanindicationofapotentialLOCAorSGTR.TheOperationsPlanoftheDay(POD)hasalsobeenrevisedtoenhanceoperatorawarenessoftheuseoftheMDAFWpumpcross-tielinesifrequired.TIFICATINFRDURATINOFREUESTTherequestedtwodayLCOextensionisnecessarytoensurecompletionrepairsonMDAFWPumpBandtoperformtherequiredtesting.Thetwodayextensionisbasedontheneedtocompletethealignmentofthepumpincludingthespeedincreaser.Basedonpreviousexperience,theseactivitiesmayrequiremultipleiterations.Duringthisrequestedextension,three100%capacityAFWpumpsandone200%capacityTDAFWpumpareavailabletoprovidethenecessarycoolingwatertotheSGsfordecayheatremoval.AsdiscussedinSection3,theprobabilityofanaccidentwhichrequirestheAFWpumpsduringthetwodayextensionisverysmall.MDAFWPumpAwassuccessfullytestedonAugust16,1994(ProcedurePT-16Q-A)~TestingoftheTDAFWpumpwasalsosuccessfullyperformedonAugust22,1994(ProcedurePT-16Q-T)providingadditionalassurancethecontinuedoperabilityofthesepumps.AttachmentAprovidesasummaryofthemostrecentAFWandSAFWpumptests.IGNIFICANTHAZARDSEVALUATIONRG&EhasevaluatedtheEnforcementDiscretionrequestandconcludedthatthereisnopotentialdetrimenttothepublichealthandsafetyandthatasignificantsafetyhazardisnotinvolved.Thisbasisforthisconclusionissummarizedbelow:

AreviewoftheGinnaStationPRAindicates thatatuberuptureinSGAwithasubsequent failureoftheTDAFWpump,thusrequiring theneedforSAFW,isofverylowprobability

<350'Fwithinthefollowing 6hours.2.DCRIPTINOFREOnAugust24,1994at0530hours,MDAFWPumpBwasremovedfromservicetoperformscheduled testingandmaintenance oftheassociated servicewatercoolingsupplystrainerandtheAClubeoilpump(Procedures EM-785andM-44.13,andWorkOrder¹19400838).

Themaintenance activities werecompleted onAugust25,1994andalimitedtestwassubsequently performed onMDAFWPumpBbeginning at1430hourstoverifytheoperability oftheAClubeoilpumpbreaker.At1432hours,theoperators noticedthatthenormallyclosedrecirculation lineair-operated valve(AOV4310)forMDAFWPumpBdidnotopenasrequired.

Thepumpwasthensecuredandatroublecardwasinitiated forAOV4310at1435hours.Preparations weretheninitiated toperformtherepairofAOV4310.Thevalvewasrepairedandafullpumptest(Procedure PT-16Q-B) wasthenperformed onAugust26,1994toverifypumpandvalveoperability.

At2220hours,itwasdetermined thattheMDAFWpumpdidnotmeetthe~prequirements andatroublecardwasinitiated forthepump.MajorrepairsonMDAFWPumpBweresubsequently initiated whichincludedcompletedisassembly ofthepump.ItisnotedthatProcedure PT-16Q-Bwassuccessfully performed onMDAFWPumpBonAugust18th.RGB'asconservatively assumedthatMDAFWPumpBhasbeeninoperable sinceAugust24,1994at0530hourswheninitialworkactivities beganandtheLCOwasentered.However,sincerepairactivities ofthepumpwerenotinitiated untillateonAugust26,1994,andduetotheleadtimeforreplacement partsandtheneedtoperformpost-maintenance testing,RG&Edoesnotanticipate thatthenecessary repairs'andtestingcanbeaccomplished withintheTSrequired7days.Enforcement Discretion istherefore requested tograntRGB'elief fromcompliance withtherequirements ofTS3.4.2.1.a.

ThisEnforcement Discretion isrequested tobeginonAugust31,1994at0530hourswhenthecurrentLCOforTS3.4.2.l.a willexpire.TheEnforcement Discretion isrequested toendonSeptember 2at0800hourswhenallnecessary repairandtestingactivities willbecompleted.

Thisisanextension of50.5hours. 3..BASTFORRKTThereareatotaloffiveAFWpumpsinstalled atGinnaStation.Thepreferred AFWsystemhastwo100%capacityMDAFWpumpsandone200%capacityturbinedriven(TDAFW)pump.TheStandbyAuxiliary Feedwater (SAFW)systemhastwo100%capacityMDAFWpumpswhicharemanuallystartedbytheoperators ifnormalAFWislost(e.g.,highenergylinebreaksintheIntermediate Building).

Thesafety-related functionoftheAFWsystematGinnaStationistomitigatethefollowing accidents orevents(UFSARSection10.5.2.1):

a.b.C.d.e.f.g.h.Lossofmainfeedwater (MFW)withoffsitepoweravailable LossofMFWwithoutoffsitepoweravailable MFWlineruptureMainSteamlineruptureLossofallACpower(onsiteandoffsite)LOCASteamGenerator TubeRupture(SGTR)CooldownItemscanddhavebeenevaluated forGinnaStationassumingthatonlytheredundant SAFWsystemisavailable (UFSARSection15.2.7.3) whileitemeonlyrequirestheTDAFWpump(UFSARSection15.2.5).ItemsaandbareboundedbytheanalysisforthehighenergylinebreaksintheIntermediate Building(i.e.,itemscandd)duetotheavailable waterinventory intheintactSGs.TheSAFWsystemcanalsobeusedforcooldownevents(itemg)duetothetimewhichwouldbeavailable asaresultoflowdecayheatlevels.Consequently, onlyLOCAsandSGTRs(i.e.,itemsfandg)mustbeconsidered withrespecttotheinoperable MDAFWpump.Following aLOCAorSGTR,thepreferred AFWsystemwillreceiveanautomatic startsignaltoprovidecoolingwatertothesteamgenerators.

However,forallLOCAs,boththeTDAFWpumpandMDAFWPumpAwillbeavailable toprovidethenecessary coolingwater.ThereisnosinglefailurewhichcanresultinthelossofbothofthesepumpsduringaLOCA.InthecaseofaSGTR,itcanbepostulated thataruptureoccursinSGAwhichwouldresultintheisolation ofautomatic AFWflowfromMDAFWPumpAandtheTDAFWpump(toS/GAonly).Onlyautomatic flowfromtheTDAFWpumpto'GBwouldbeavailable inthisinstance.

OperatoractionisalreadyrequiredforaSGTReventtodepressurize theRCS.Sincethistenminutedelayisalreadyassumedintheaccidentanalysis, itcanbeassumedthattimeisalsoavailable tousetheSAFWsystemorcross-tie MDAFWAtoSGBiftheTDAFWpumpweretofail.Inaddition, itisnotedthattheincreased SGlevelintherupturedSGprovidesadditional timebeforeAFWwouldberequired. AreviewoftheGinnaStationPRAindicates thatatuberuptureinSGAwithasubsequent failureoftheTDAFWpump,thusrequiring theneedforSAFW,isofverylowprobability

(-1.4E-06fortheadditional 2dayMDAFWPumpoutage).Inaddition, theAFWsystemisinstandbyduringnormalpoweroperations.

Itisnotconsidered prudenttorequireaplantshutdownthroughmodesinwhichthepreferred AFWsystemisusedtoprovidenormalSG'cooling waterneedswithoneMDAFWpumpinoperable.

Thepotential forplanttransients andthesubsequent needforAFWduringthesemodetransitions isalsoincreased.

MPENSATRYACTINRG&Ewillnotperformanytestingormaintenance relatedtotheremaining AFWandSAFWpumpsandtheirnecessary supportsystemsduringtherequested twodayextension.

Inaddition, operatorawareness ofRCSleakagehasbeenincreased sinceleakagecanbeusedasanindication ofapotential LOCAorSGTR.TheOperations PlanoftheDay(POD)hasalsobeenrevisedtoenhanceoperatorawareness oftheuseoftheMDAFWpumpcross-tie linesifrequired.

TIFICATINFRDURATINOFREUESTTherequested twodayLCOextension isnecessary toensurecompletion repairsonMDAFWPumpBandtoperformtherequiredtesting.Thetwodayextension isbasedontheneedtocompletethealignment ofthepumpincluding thespeedincreaser.

Basedonpreviousexperience, theseactivities mayrequiremultipleiterations.

Duringthisrequested extension, three100%capacityAFWpumpsandone200%capacityTDAFWpumpareavailable toprovidethenecessary coolingwatertotheSGsfordecayheatremoval.Asdiscussed inSection3,theprobability ofanaccidentwhichrequirestheAFWpumpsduringthetwodayextension isverysmall.MDAFWPumpAwassuccessfully testedonAugust16,1994(Procedure PT-16Q-A)~TestingoftheTDAFWpumpwasalsosuccessfully performed onAugust22,1994(Procedure PT-16Q-T) providing additional assurance thecontinued operability ofthesepumps.Attachment AprovidesasummaryofthemostrecentAFWandSAFWpumptests.IGNIFICANT HAZARDSEVALUATION RG&Ehasevaluated theEnforcement Discretion requestandconcluded thatthereisnopotential detriment tothepublichealthandsafetyandthatasignificant safetyhazardisnotinvolved.

Thisbasisforthisconclusion issummarized below:

-4,a"Extending thecurrentoutageofMDAFWPumpBforapproximately twodaysdoesnotresultinasignificant increaseintheprobability orconsequences ofanaccidentpreviously evaluated.

Theremaining AFWandSAFWpumpsprovidesufficient redundancy tomitigateallpotential accidents.

b.Extending thecurrentoutageofMDAFWPump8forapproximately twodaysdoesnotcreatethepossibility ofanewordifferent kindofaccidentfromanypreviously evaluated.

Theonlyaccidentwhichcanbeinitiated byAFWisanovercooling eventcausedbyspuriousactuation.

SinceMDAFWPumpBisnotoperable, thisaccidentisnotcredibleduringtherequested extension.

Noothernewordifferent kindsofaccidentarecreatedbytheplantcondition duringthetwodayextension.

C.Extending thecurrentoutageofMDAFWPump3forapproximately twodaysdoesnotinvolveasignificant reduction inamarginofsafety.Theremaining AFWandSAFWpumpsprovidesufficient redundancy tomitigateallpotential accidents.

Inaddition, operators aresufficiently trainedandprocedures addresstheresponsetoalossofMFW.7.NVIRNMKNTALIMPACTTherearenoenvironmental releasesassociated withthecurrentplantcondition.

Theprobability ofrequiring useoftheinoperable MDAFWpumpisverysmall.Inaddition, sufficient guidanceiscurrently available totheoperators torespondtoany,postulated scenariowhichwouldrequireAFWortheSAFWsystem.Therefore, therearenoenvironmental consequences associated withthisrequest.8.REVIEWBYRCTheGinnaStationPORChasreviewedandapprovedthisrequestforEnforcement Discretion.
