The circumstances related to the apparent violations are described in a letter providing the results of NRC Investigation Report 3-2013-018. This letter is available in the NRC Agencywide Documents Access and Management System (ADAMS), accessible from the NRC Web site at http://www.nrc.gov/reading-rm/adams.html at Accession Number ML14085A472; however, the enclosure to the letter is not publicly available.
