The root cause, of this event was inadequate procedures.

Operations is not required to be notified when equipment related to the TS is removed from service.Instead, procedures require Operations to be notified when it becomes apparent the the LCO time limit will expire.The immediate corrective action was to declare the affected systems inoperable.

Additional'orrective actions will include training of the affected plant personnel and revision of plant.procedures.

On May 14, 1991 at 1438 hours0.0166 days <br />0.399 hours <br />0.00238 weeks <br />5.47159e-4 months <br />, a 24-hour LCO was entered when a reactor vessel water level (RVWL)transmitter

[JCI was removed from service for calibration.

On May 14, 1991 at 1828 hours0.0212 days <br />0.508 hours <br />0.00302 weeks <br />6.95554e-4 months <br />, Operations was notified that the RVWL transmitter could not be calibrated and was therefore inoperable..

At this time, a log entry was made in the Operations LCO tracking log to document the time of entry into the LCO.However, the.time documented in the LCO tracking log was the time the RVWL transmitter was determined to be inoperable (1828 hours0.0212 days <br />0.508 hours <br />0.00302 weeks <br />6.95554e-4 months <br />)not the time the transmitter was removed from service (1438 hours0.0166 days <br />0.399 hours <br />0.00238 weeks <br />5.47159e-4 months <br />).As a result of the incorrect log entry,, Operations personnel did not declare the systems affected by the RVWL transmitter inoperable within the allotted 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> as required by the TS.'At the time of this event, Unit 2 was in cold shutdown and Units 1 and 3 defueled.In addition, no fuel handling or other operations over spent fuel were in.progress.

This event resulted in a violation of the plant TS and is reportable in accordance with 10 CFR 50.73(a)(2)(i).

The RVWL transmitter provides input to.the reactor low water level instrument channel.This instrumentation is required for the operability of the following systems: Residual Heat Removal, Core Spray, Diesel Generators, Automatic Depressurization,.High Pressure Coolant Injection, and Reactor Core Isolation Cooling.The TS allows the RVWL transmitter to be out of service for 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> for calibration or repair.In the event that'he calibration or'epair of this transmitter cannot be completed within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> then the affected systems must be declared inoperable There were.no safety consequences or.implications resulting from this event.If Unit 2 had been operating at.power when this event occurred, the plant TS would have required the unit to be in hot standby within six hours and in cold shutdown within the following 30 hours3.472222e-4 days <br />0.00833 hours <br />4.960317e-5 weeks <br />1.1415e-5 months <br />.However, Unit 2 was already in cold shutdown during this event.Therefore, this event had no safety significance.

The procedure used to perform this calibration documented the time the RVWL transmitter was removed from service.However, it did not require Operations to be notified of the time the instrument was removed from service.Instead, the procedure only required Operations to be notified if it appeared the work would exceed the LCO time limit.In addition, a.contributing factor to this event was inadequate communication.

Plant personnel did not ensure the time entered in the Operations LCO tracking log was the actual time the RVWL transmitter was taken out-of-service.

The immediate corrective action was to declare the systems affected by the RVWL transmitter inoperable.

This action was completed on May 15, 1991 at 1550 hours0.0179 days <br />0.431 hours <br />0.00256 weeks <br />5.89775e-4 months <br />.Additional corrective actions will include the, following:

First, affected plant personnel will review the.event investigation.

Second, affected Maintenance personnel will be trained on the TS definition of"operable" and on how Operations interprets the terms"inoperable" and"out of service."
