This report describes relevant conditions that required evaluation and repairs and replacements to be performed during the period April 2012 through April 2014.

CR-IP2-2014-01714- Some threads were found to be damaged on the threaded support rod. Engineering evaluated F-A / F1.1OC Class 1 Piping Supports - Thermal Movement/ SIH 206 the condition and the rod is structurally adequate to carry out its design function.
CR-IP2-2014-01171 - The examiner found that approximately one inch of weld was missing from the joint between F-A / F1.20A Class 2 Piping Supports - One Directional/ SIH 165 items 1 and 2 as identified on drawing SIH-165-04. Engineering evaluated the condition and the pipe support is functional and structurally adequate for the design loads.
F-A / Fl.20A Class 2 Piping Supports - One Directional/ SR 91 CR-IP2-2014-00871 - A loose pipe clamp bolt was found. Engineering evaluated the condition and the clamp bolt was tightened for future design configuration.
F-A / F1 .20C Class 2 Piping Supports - CR-IP2-2014-01944- The field configuration was different from the drawing. Engineering evaluated the condition and Thermal Movementl/ SIH 194thcodinwaaceedsi.

CR-IP2-2014-01124- Sliding clearance on support base plate exceeded tolerance identified on the design drawing F-A / F1.30B Class 3 Piping Supports - Multi-directional/ SR 788 (cut sheet). Maximum allowable dimension is per side is 1/8" (1/4" total). Engineering evaluated the condition and the support will perform its design function as installed for horizontal and vertical seismic restraints.
