Dear Sir:

This letter submits the Indian Point Nuclear Generating Unit No. 2 (IP2) Owners Activity Report, IP2-RPT-05-00021, Rev. 0 (Enclosure 1) which contains ISI results and repair/replacement activities performed during the 3rd Period of the Third 10-Year ISI Interval. All work performed met the requirements of 1989 Edition, No Addenda, of the ASME Section XI Code.

Entergy will use NRC-approved ASME Section XI Code Case N-532-1 for submitting inspection results completed for 1P2. This Code Case was approved for use through Regulatory Guide 1.147, Revision 13. Code Case N-532-1 provides alternatives for the documentation requirements for repair and replacement activities, and allows the use of Form NIS-2A in lieu of Form NIS-2. Enclosure 1 was prepared in accordance with the requirements of Code Case N-532-1 as approved.

F-A SR 14A Rigid Strut Support CR-IP2-2004-3871 - The thread engagement Is about 1/2-. On Yes the 3/4- diameter rod, two thirds of full thread engagement Is present (10 threads per Inch). This strut is designed for 3000#.

The seismic design load for this support Is 1062#. With the partial thread engagement, the support capability would be about 2000#.

The required design load is below the 2000# capability therefore the restraint will perform its design function In the as found condition. Therefore, the pipe support is operable.

F-A SR 44A Rigid Strut Support CR-IP2-2004-3872 - The thread engagement Is about 1/2'. On Yes the 1 1/4- diameter rod, there are 4 threads engaged (8threads per Inch). This strut Is designed for 10,000#. The seismic design load for this support is 1221 #. Each thread provides 1000# of design load capability. With the partial thread engagement, the support capability would be about 4000#. The required design load Is below the 4000# capability therefore the restraint will perform its design function in the as found condition.Therefore the restraint Is operable.

F-A 27 H SA Valve Support CR-IP2-2004-5295 - Follow-up Inspection of valve support 27-H- Yes 5A Indicates that the 4 lock washers for Item #5 of drawing #

1991 M02113 were not installed. Also the bolts and nuts are very tgh and no sign of loosing. This condition Indicates that the structural angle (Item # 1). bolts and nuts (Item # 5)are in full contact with each other.

One of the nuts lack full thread engagement by 1 thread. This condition is acceptable based on remaining thread engagement requirements.Based on engineering Judgment, there Is no operably concern at this time.

F-A SIH 58 Rigid Strut Support CR-IP2-2004-5291 - Operability has been documented. Nut was Yes tightened.

F-A CH 130 Hanger Rod Support There are two nuts Installed at the top structural attachment with Yes the steel. The nut Is reported loose and hanger rod Is still taking the load of the piping system. There Is no operability concern and pipie support Is performing its design function of carrying the load of the piping system. Work order no. IP2-04-30207 has been initiated to tighten the nuts at the top structural attachment.

F-A CH 140 Hanger Rod Support CR-1P2-2004-5293 - The original concern of the loose rod/nut on Yes support CH-140 was addressed by tightening them under WO IP2 04-30368. In addition, calculation IP-CALC-04-30368 was Issued that verified system operability without pipe support CH-140.

F-A CH 126 Hanger Rod Support CR-IP2-2004-5374 -Hanger support has a slightly bent rod. Yes Engineering reviewed and found that hanger rod Is acceptable as is. No further action equired.

E-D VCM-01 & 02 Moisture Barrier CR-IP2-2004-5852 The VC moisture barrier (Chaulking) was Yes (IWE) Columns 1to 7 found degraded during 2R16 Inspections. Engineering has reviewed the finding and recomemded repairs.
