Per Technical Specttication ~ction Statement 3.7.11.a, a Special Report is required when fire barrier penetrations are impaired for greater than seven (7) days.      This report satjsfies this requirewent.
On February 16, 1992, during normal power operation, the fire door between the 100' Elevation Mechanical Penetration Area and the Auxiliary Building was found impaired by a roving fire watch during patrol.      The door's hinge screws were broken causing misalignment of the door.      The door could not adequately close and latch. Therefore, it was declared inoperable and Techni~al Specification 3.7.11 Action "a" was applied.
Detail~d    inspection revealed that the internal hinge plate welds were broken and the internal frame was    loose. It was determined to replace the door. *A new door was    ordered; however, it was not received within the seven (7) day    Technical Specification Action Statement time constraint.
An hourly rovi-ng fire watch had previously been established for the fire door (see Apparent Cause of Occurrence section); therefore, the requir~ments of Technical Specification A~tion Statement 3.7.11.a, were met.
Technical Specification 3.7.11 states:
                  "All fire penetrations (including cable penetration barriers, fire doors and fire dampers), in fire zone boundaries, protecting safety related areas shall be functional."
Action Statement 3.7.11.a states:
                  "With one (1) or more of the above required fire barrier penetrations non-functional, within one (1) hour either establish a continuous fire watch on at least one (1) side* of

(cont'd) the affected penetration, or verify the OPERABILITY of fire detectors on at least one (1) side of the non-functional fire barrier and establish an hourly fi~e watch patrol.                            R~store the non-functional
          ~ire barrier penet~ation(s) to iunctional status within 7 days or, in lieu of any other report reqGired by Specification 6.9.1-, pr~pare and submit a Sp-ecial Report to the Commission pursuant to Specification 6.9.2 within the next _(30) days outlining the action taken, the cause of the non-functional penetration and plans and schedule for restoring the fire barrier penetration(s) to functional status."

-*----------          CAUSE OF OCCURRENCE:
The cause of the door not-being returned to a functional condition is equipment failure.                          The damage was such that door replacement was necessary.                A replacement door was ordered since these doors are not a stock it~m.                    It was not received withj_n seven (7) day~ to satisfy the Technical Specification Action Statement.
Inv~stigation identified that this door is found non-functional at least once each year.                            The door is ~art of a high traffic pattern.
Additionally, the Auxiliary Building is maintained at negative pressure.                Doors *such as this, which access the Auxiliary Building, tend to-close "hard" promoting a potentially higher failure rate.
The concern for contihued functional reliability of these doors has been previously identified.                            Until the ventilation concerns aie resolved, PSE&G has committed to maintaining an hourly fire watch rove for all subject fire do9rs (i.e., reference response to NRC Inspection Report 272/90-01)

The functional integrity of fire barrier penetrations ensures that fires will be confined or adequately retarded from spreading to
_adjacent portions of the facility.                              This design feature minimizes the possibility of a single fire involving several areas of the facility.                Fire doors are a passive element in the facility fire protection program and are subject to periodic inspections.                            This report s~tisfies reporting requirements of Technical Specification Action Statement 3.7.11.a, pursuant to Technical Specification 6.9.2 since the time between discovery and eventual repair of the fire door im~airment is greater than seven (7) days.                            Appropriate remedial actions were in place in accordance with the requirements of Technical Specification Action Statement 3.7.11.a to establish an hourly roving fire watch patrol for the impaired fire barriers once the impairments were identified.
The subject fire areas contain detection devices in addition to the roving fire watch patrol.                            A fire in the area~ would be detected before it could involve an adjacent aiea.                            Thi~ occurrence, therefore, involved no undue risk to the health or safety of the public.

The repair of the impaired fire door was not accomplished within seven (7) days as discussed previously.                            The fire door will be replaced upon receipt of the new door.

The hourly fire watch will continue until the ventilation concerns, associated with the Auxiliary Building, are resolved.
