ATTACHMENT-BUNPLANNEDGASEOUSEFFLUENTRELEASEofNOVEMBER4,19921)Descriptionandanalysisofevent2)HourlyaveragesitemeteorologicalforNovember4,199237 tLJkIg~4I~I'I' AttachmentB1)DescriptionandanalysisofeventOnNovember4,1992thenitrogensystemwasisolatedatthebulkgashousetorepairasafetyreliefvalvethatwasliftingprematurely.Duringthetimethatnitrogenwasisolated,thenitrogensystempressureinthedownstreampipingdecreased.SometimelatertheSteamjetAirEjector(SJAE)RadMonitoralarmedandControlRoompersonnelalertedChemistrypersonnel.ChemistrybegansamplingtheSteamGeneratorsforradioactivityandobtainednegativeresults.AgrabsampleontheSJAEexhaustgasesindicatedradioactivitywaspresent.TheSJAEexhaustwasconfirmedtobealignedtothePlantVentExhaust,it'snormalexhaustpathprecedingthisevent.ThePlantVentPathwayisaTechnicalSpecificationContinuousReleaseMonitoredPathway.AninvestigationbyChemistryandOperationspersonnelindicatedthatGasDecayTank(GDT)1Bwasbackpressuring(withflow)thenitrogensystemthroughthenormallyclosedvalveV-6596.ThisvalveisthenitrogensupplytoGDT1B.Thenitrogensystemto,the"GDT'swasisolatedbyclosingrootnitrogensupplyvalveV-6788andaPlantWorkOrder(PWOI92047980)wasissuedtorepairV-6596.TheChemistryRadioassayoftheAirEjectorExhaustindicatedanactivitylevelof1.1E-07Curiespersecondwhichis0.00003percentoftheSiteLimitof0.35CuriespersecondaspertheODCM.Nonoticeableincrease,oralarmcondition,wasdetectedbythePlantVentMonitorduetothelowconcentrationsinthePlantVentduringthisevent.NoTechnicalSpecificationLimitswereexceeded.PlantManagementdirectedarootcauseanalysisbeperformedwithcorrectiveactiontopreventarecurrence.ThefindingandrecommendationwastoinstalladoublecheckvalveonthenitrogensupplylinetotheReactorAuxiliaryBuilding(RAB)I1,aspertheoriginaldesignoftheUnitI2RABnitrogensupply,topreventbackflowfromthe.potentiallyradioactiveRAB01sidetotheSecondaryPlantsideofthenitrogensystem.UnitI1wasnotdesignedwithadoublecheckvalvetotheRABside.ThisdesignchangewassubmittedasperthePlantChangeModification(PCM)processandiscurrentlybeingreviewedbyEngineering.ThefinaldesignchangePCMwillbesubmittedtotheFRGforapproval.Inaddition,plantmanagementhasemphasizedtheneedforprecautionstobetakenpriortoanyplannedmaintenanceontheNitrogenSystemuntilthePCMiscompleted.38
