Entergy    concurswith  the  violation  ascitedinthe      Notice              (NOV) ofViolation      regarding Technical Specification  5.4.1.a  forfailure  to maintain procedures required        byRegulatory  Guide  1.33, Revision  2,Appendix    A.Specifically, the NRCissued            aviolation forfailuretomaintain adequate    procedures  for feedwater    system operation        onApril 29,2020, andthe  licensee  failed torestore                within compliance a reasonable              amount    of time.

M During    anNRCinspection      conducted    from  January    25through February 12,2021,      a violationof NRCrequirements      wasidentified.      Inaccordance      with  the NRCEnforcement Policy,    theviolation islisted  below:
Technical Specification    5.4.1.a,  requires,  inpart,  thatprocedures shall beestablished, implemented,  andmaintained      covering    theapplicable    procedures recommended      in Appendix A ofRegulatory      Guide    1.33,  Revision    2.Section 4.oofAppendix A to Regulatory Guide    1.33,  Revision  2,requires    instructions  fortheoperation of the feedwater system.
Contrarytotheabove,        from June1,2019,        toFebruary    10,2021,thelicensee  failed to establish,implement,    and  maintain    applicable    procedures  recommended    inAppendix A ofRegulatory  Guide Revision 1 .33,            2,  Section    4.o fortheoperation  ofthe feedwater system. Specifically,  thelicensee    established    Procedures  SOP-0009,    "Reactor Feedwater  System    (SYS  #107),"  Revision    80 82,andOSP-0053, "Emergency    and Transient Response      Support  Procedure,"    Revision    27and28,tomeet    theRegulatory Guide 1.33 requirement,      andthe    licensee  failed    tomaintainthese procedures  for feedwater operation    toinclude    necessary    precautions    toprevent operation  ofthesystem inanimproper    lineup.
This  violation isassociated    with  a Green    SDPfinding.
Reason.for..the  Violatio.n OnApril    29,2020  theNRCissued        "River  BendStation      -
IntegratedInspection Report 05000458/2020001."      Theinspection      report  documented      a non-citedviolation (NCV  2020001-
: 03) for  failure to maintain  adequate    procedures      forf  eedwater  system operation  because    ofthe inspectors  review ofa reactor    scram  that  occurred  onMay31,2019.      Additional details  were
 
RBG-48084 Enclosure Page  2of2 provided  intheinspectionreport        thattheNRCconcerns      werespecific  toSOP-0009,    "Reactor Feedwater  System,"    andOSP-0053,      "Emergency  andTransient    Response    Support  Procedure."
Although  the violation    was  documented    initially inthe Corrective  Action  Program  (CR-RBS-2020-00965)  the  condition    report wasclosed  tothetwoactions    taken  under  CR-RBS-2019-03891    that addressed      the  causal  evaluation forthereactor    scram. All  corrective actions were closed      to NRC issuing inspection priorthe                                report05000458/2020001.
Uponreceipt  oftheinspection report, RegulatoryAssurance            personnel  didnotrecognize    that a newcondition    report  needed to begenerated toensure        theadditional  details associated  with  the NCVwereadequately          addressed  asrequired byEntergy      procedure EN-Ll-123,  "NRCInspection Support."  Asa result      correctiveactions werenot  developed    inaccordance    with EN-LI-102, "Corrective Action    Program,"    toadequately address correcting    theSOP-0009    andOSP-0053 procedures.
Theidentified  condition    wasentered    into Entergy'scorrective  action program  (CR-RBS-2021-00644).
Thefollowing  corrective    actions  were taken:
Action 1:        Revised    OSP-0053,    Emergency  andTransient Response Support          Procedure, hard  card  toensure  Operators  understand    therequirements    that  atleast  one LowPressure      Heater Stringisinservice    whenrestarting feedwater pumps.
Action 2:        Revised    SOP-0009,  Reactor Feedwater    System, toadd a precaution and limitation  statement  tonot attempt tostart  a Reactor  FeedPump unless at least  oneLowPressure      Heaterstring  isvalved into  service on the condensate      side.
Asa result  ofthe    corrective  actions taken,  Entergycompleted    the required  changes  tothe affected  procedures, addressing therefore,              the issueof  concern  and  resulted  in restoringfull compliance.

Technical Specification 5.4.1.a, requires, inpart, thatprocedures shall beestablished, implemented, andmaintained covering theapplicable procedures recommended in Appendix A ofRegulatory Guide 1.33, Revision 2.Section 4.oofAppendix A to Regulatory Guide 1.33, Revision 2,requires instructions fortheoperation of the feedwater system.

Contrarytotheabove, from June1,2019, toFebruary 10,2021,thelicensee failed to establish,implement, and maintain applicable procedures recommended inAppendix A ofRegulatory Guide Revision 1 .33, 2, Section 4.o fortheoperation ofthe feedwater system. Specifically, thelicensee established Procedures SOP-0009, "Reactor Feedwater System (SYS #107)," Revision 80 82,andOSP-0053, "Emergency and Transient Response Support Procedure," Revision 27and28,tomeet theRegulatory Guide 1.33 requirement, andthe licensee failed tomaintainthese procedures for feedwater operation toinclude necessary precautions toprevent operation ofthesystem inanimproper lineup.

This violation isassociated with a Green SDPfinding.

Reason.for..the Violatio.n OnApril 29,2020 theNRCissued "River BendStation -

IntegratedInspection Report 05000458/2020001." Theinspection report documented a non-citedviolation (NCV 2020001-

03) for failure to maintain adequate procedures forf eedwater system operation because ofthe inspectors review ofa reactor scram that occurred onMay31,2019. Additional details were

Although the violation was documented initially inthe Corrective Action Program (CR-RBS-2020-00965) the condition report wasclosed tothetwoactions taken under CR-RBS-2019-03891 that addressed the causal evaluation forthereactor scram.
