APPLICANT'S FOURTH PARTIAL' RESPONSE TO ROREM'S SECOND SET OF QUALITY ASSURANCE INTERROGATORIES AND REQUESTS TO PRODUCE On October 21, 1985, Intervenors Rorem, et. al.
("Intervenors") filed their Second Set of Quality Assurance Interrogatories and Requests to Produce.
Applicant filed a first partial response on November 15, 1985 and responded to the remainder of the specific interrogatories in Intervenors' Second Set on November 22, 1985.      On December 6, 1985, Applicant filed a third partial response which included indices to DCAP documents. This submission which is Applicant's Fourth              i Partial Response supplements the indices in the Third Partial Response and provides indices to BCAP documents maintained by Sargent & Lundy.

Most of the BCAP Task Force and BCAP-QA documents are stored on the Braidwood site. Sargent & Lundy documents are maintained at either the Braidwood site or at Sargent &
Lundy's downtown offices located at 55 East Monroe Street, Chicago, Illinois 60603. Applicant needs at least one week's notice to arrange inspection of documents at the Braidwood site or at Sargent & Lundv's downtown offices.
Documents for which Applicant is asserting privi-lege have not been provided and are not available for inspection or copying. A privilege index will be provided indicating whether attorney client ("AC") or attorney work product ("WP") privilege is being asserted.
The first two attachments supplement the indices for the BCAP Task Force and BCAP QA provided in the Appli-cant's Third Partial Response. Attachment 1 supplements the Index Listing of BCAP Informal Files (Attachment 4 of Applicant's Third Partial Response). Attachment 2 is a listing of personnel associated with BCAP over the life of the program and their positions. Further investigation is proceeding to identify any other documents that concern BCAP. Supplements to indices will be provided as this process is completed.

the Structural and Electrical disciplines are located at Sargent & Lundy's downtown offices; files for the Mechanical discipline are located at the Braidwood site.
Sargent & Lunay_ files for the Mechanical discipline cover all piping populations (M-001 through M-015) and some HVAC/ Mechanical (H 002, H-005, H-006, and H-008). They consist of:
: 1)    4 Binders indexed by population number which typically contain for each population
              - NCR signoff
              - Copy of the inspection counting form
              - Copy of the discrepancy counting form
              - S&L write-up for that population
              - Supplemental materials
: 2)    1 Calculation book organized by population number which contain copies of relevant calculations from Project Management Division (PMD)
: 3)    2 Boxes organized by population number which contain copies of observation records used for S&L evaluations related to that population.
The Structural Department calculations which relate to BCAP include calculations on the determination of the more highly stressed elements, inspection point counts and design significance of observations.      Attach-ments 3 and 4 are indices for these calculations. Attach-ment 5 is an index of Structural Department's calculations for BCAP Trending.
Attachment 6 is a file index for the Electrical Department. The total volume of these files is approximately one file drawer and three binders. Calculations prepared by Structural Department to resolve discrepancies found by i
Electrical-Department have been integrated.with other calculations maintained by Structural Department.
Investigation continues for Sargent & Lundy documents which are not maintained in formal official filing systems.

APPLICANT'S FOURTH PARTIAL' RESPONSE TO ROREM'S SECOND SET OF QUALITY ASSURANCE INTERROGATORIES AND REQUESTS TO PRODUCE On October 21, 1985, Intervenors Rorem, et. al.

("Intervenors") filed their Second Set of Quality Assurance Interrogatories and Requests to Produce.

Applicant filed a first partial response on November 15, 1985 and responded to the remainder of the specific interrogatories in Intervenors' Second Set on November 22, 1985. On December 6, 1985, Applicant filed a third partial response which included indices to DCAP documents. This submission which is Applicant's Fourth i Partial Response supplements the indices in the Third Partial Response and provides indices to BCAP documents maintained by Sargent & Lundy.

Most of the BCAP Task Force and BCAP-QA documents are stored on the Braidwood site. Sargent & Lundy documents are maintained at either the Braidwood site or at Sargent &

Lundy's downtown offices located at 55 East Monroe Street, Chicago, Illinois 60603. Applicant needs at least one week's notice to arrange inspection of documents at the Braidwood site or at Sargent & Lundv's downtown offices.

Documents for which Applicant is asserting privi-lege have not been provided and are not available for inspection or copying. A privilege index will be provided indicating whether attorney client ("AC") or attorney work product ("WP") privilege is being asserted.

The first two attachments supplement the indices for the BCAP Task Force and BCAP QA provided in the Appli-cant's Third Partial Response. Attachment 1 supplements the Index Listing of BCAP Informal Files (Attachment 4 of Applicant's Third Partial Response). Attachment 2 is a listing of personnel associated with BCAP over the life of the program and their positions. Further investigation is proceeding to identify any other documents that concern BCAP. Supplements to indices will be provided as this process is completed.

Sargent & Lundy files which relate to BCAP are maintained by each of three departments within Sargent &

Lundy: Structural, Electrical and Mechanical. Files for J

the Structural and Electrical disciplines are located at Sargent & Lundy's downtown offices; files for the Mechanical discipline are located at the Braidwood site.

Sargent & Lunay_ files for the Mechanical discipline cover all piping populations (M-001 through M-015) and some HVAC/ Mechanical (H 002, H-005, H-006, and H-008). They consist of:

1) 4 Binders indexed by population number which typically contain for each population

- NCR signoff

- Copy of the inspection counting form

- Copy of the discrepancy counting form

- S&L write-up for that population

- Supplemental materials

2) 1 Calculation book organized by population number which contain copies of relevant calculations from Project Management Division (PMD)
3) 2 Boxes organized by population number which contain copies of observation records used for S&L evaluations related to that population.

The Structural Department calculations which relate to BCAP include calculations on the determination of the more highly stressed elements, inspection point counts and design significance of observations. Attach-ments 3 and 4 are indices for these calculations. Attach-ment 5 is an index of Structural Department's calculations for BCAP Trending.

Attachment 6 is a file index for the Electrical Department. The total volume of these files is approximately one file drawer and three binders. Calculations prepared by Structural Department to resolve discrepancies found by i

Electrical-Department have been integrated.with other calculations maintained by Structural Department.

Investigation continues for Sargent & Lundy documents which are not maintained in formal official filing systems.
