filed objections to certain of those discovery requests and provided to Intervenors' counsel a partial response to Intervenors' discovery requests. Applicant's first partial response was revised and resubmitted on August 1, 1985. Applicant filed a second partial response on August 5, 1985 and a third on August 10, 1985. . o Applicant's fourth partial response to Intervenors' discovery requests, provides responses to Specific Interrogatories 28, 30, 31, 37, 38, 42, 44, 45, and 49. Investigation continues for responses to all of the requests to which Applicant has not objected.

Each document identified in Applicant's responses or incorporated by reference into Applicant's responses has been identified with a number. In addition, Applicant has provided indices for these documents organized by interrogatory number, which identify for each numbered document the date, author, recipient, and type of document.

Revised and updated indices are also provided for Specific Interrogatories which Applicant answered in prior partial responses. The indices will be updated further with each partial response submitted by Applicant.

Documents which are incorporated by reference to answer interrogatories are being provided to the parties indicated on the service list. The indices include a notation as to which documents have been so provided with either this partial response or prior partial responses. Documents which are simply identified in answers to interrogatories or on the indices, but not incorporated by reference, are available for inspection and copying. Documents for which Applicant is asserting privilege have not been provided, and an indication can be found on the indices as to whether the attorney client ("AC") or attorney work product ("WP") privilege is being asserted.

INTERROCATORY 28 Subsequent to these NRC findings, did Phillips-Cetschow management suspend the engineering department support selection program for process and instrument piping and instrument tubing? If so, please describe in detail the circumstances and explain why this problem occurred and corrective action taken. Please identify any documents which reflect these answers.

RESPONSE

The PCCo Site Manager suspended the Phillips, Getschow engineering department support selection program for process piping, instrument piping and tubing in category I buildings on October 10, 1983, pending evaluation of the program's compliance with Sargent & Lundy (S&L) specification requirements and Phillips, Getschow procedure PGCP-22.

Commonwealth Edison Company concurred with the suspension of the subject program and directed that the suspension be maintained until their written concurrence for resumption of the program was obtained. Written concurrence for program resumption was issued by Commonwealth Edison in December 1983.

The following summary details the chronological events and actions which took place prior to the suspension of work and through its subsequent resumption.

In 1981, S&L developed a guideline for use by Phillips, Getschow to properly support small bore piping in category I buildings. The guideline provided maximum support intervals and basic rules for certain special situations, such as valves encountered in the piping configuration. The guideline was simplified to the extent that only arithmetic calculations, based on piping dead-weight load, were required to be performed. Based on this simplified load and span criteria, i

Phillips, Getschow was to select a pre-engineered, pre-qualified standard support from a series of S&L approved standard supports, meeting the requirements of the load. Upon approval of Phillips, Getschow Company Procedure PGCP-22, Rev. 1, which incotTorated the simplified guidelines, safety related instrumentation line support selection started in July, 1981. After enough work had been completed so that a representative sample could be taken, this area was checked by Commonwealth Edison company Quality Assurance in Audit QA-20-81-30 of October, 1981. This audit identified deficiencies that are summarized as follows:

1. Incorrect calculations.
2. Improper calculation reviews.
3. Three dimensional restraints not used per ECM 2194.
4. Adjacent line weights not used in line weight calculations.
5. Revision levels of applicable isometric drawings not listed on calculation sheets.
6. System component weights obtained from an uncontrolled source

'(uncalibrated scale).

7. Quality Control not monitoring instrument group activities.
8. Documanted training not given to Procedures.
9. Interface document not clearly written.
10. Organization chart in error.

This audit resulted in a stop work action being placed on Phillips-Cetschow Company until February,1982. The details of these deficiencies, including Quality Assurance follow-up review, corrective actions, and item close-outs, are described in the Ceco QA audit close-out surveillance reports. All items were resolved and closed by March, 1982.

As additional follow-up to the October, 1981 audit, a Commonwealth Edison Quality Assurance Audit QA-20-82-15 was performed in July, 1982.

The concerns of the October, 1981 audit were re-checked as evidenced by approved checklist questions 1,2,3,4,5,10,11,12,14,15,16, and 17. All question areas were found acceptably implemented. At that time, based on the audit history, Commonwealth Edison Company Quality Assurance believed that instrument support selection was being performed in an acceptable manner, and that Phillips-Getschow Company was adequately implementing their procedures in this area.

Small bore process pipe support selection started in January, 1983 upon approval of Revision 2 of PGCP-22 which added that scope of work.

Again, after work had been completed so that'a representative sample could be taken, small bore pipe support selection was checked and instrumentation line support selection was re-checked in July, 1983 during Ceco audit QA-20-83-33. It was identified that Phillips-Getschow Company Procedure PGCP-22 did not give a step-by-step method foe performing support selection calculations: that the procedure did not reference the current applicable ECM (4566); and that information on the calculation sheets was unclear or incomplete. Also, audit QA-20-83-33 identified that small bore pipe and instrumentation line support l

selection training flies were incomplete. Corrective actions were  !

1 defined and implemented to correct these items. Ceco Site Quality )

I Assurance' continued to audit this area. Site Quality Assurance audit QA-20-83-49 specifically audited support selection activities and design document control related to support selection work. Three (3) findings were identified which are summarized as follows:

1. Arithmetic errors were found in support selection calculations.

2. Drawings used to locate component supports were not verified l "as constructed" drawings.
3. Drawings used as input for calculations were not the most current revision.

These findings and similar concerns identified by the NRC in Inspection Report 83-09 were the basis for the new Phillips, Catschow management team to suspend the support selection work activity pending further evaluation.

In November, 1983 and after the work suspension had been imposed, Sargent & Lundy conducted a review of all safety-related Phillips, Getschow Company support selections on process piping. Their review of ,

172 supports indicated that 3 supports required installation tolerance I

adjustment, and 14 supports required re-calculation.

Sargent & Lundy also revised the piping support guidelines to better clarify their requirements such that support selectors would not need to l

interpret the guidelines. Procedure PGCP-22 Revision 8, was issued to '

I enhance and clarify the procedure itself, as well as its implementation and training aspects. The procedural revision provided for better calculation consistency and reduction of the chance for error. Extensive re-training has been given to support selection personnel. Revision 8 of

.this procedure was given interim approval by Commonwealth Edison Site i

Quality Assurance on November 10, 1983, and was accepted by Sargent &

Lundy on December 2, 1983.

In December, 1983, as a prelude to the possible resumption of work, Commonwealth Edison Company Quality Assurance conducted an audit (QA-20-83-62) that checked the implementation of Phillips-Getschow Procedure PGCP-22, Rev. 8. Fifty-five supports were reviewed in detail and no errors were identified. This indicated that the methodology used at that time was adequate to perform the support selection activity in an acceptable manner. Based on that review,. Commonwealth Edison written concurrence allowed the pipe support selection program to resume on December 21, 1983.

Site Quality Organizations, both CECO and PGCo, have continued to monitor support selection activities by audit and surveillances, and Sargent & Lundy has continued to perform periodic technical audits.

Based on these acceptable reviews and the implementation of the revised procedures, the corrective sctions discussed above have been effective in upgrading the program to its currently acceptable lavel.

SPECIFIC INTERROCATORY 30 Did Phillips, Getschow ever fail to implement corrective action to verify past installations of large bore piping components? If so, please describe in detail the circumstances and explain why this problem occurred and further corrective action taken. Please identify any documents which reflect these answers.

Phillips-Getschow has implemented effectively its corrective actions to verify past installations of large bore piping components. This judgment is based on my knowledge of the corrective actions and my participation in their management and oversight. A review of the activities under Phillips-Getschow NCR #989 serves as an example.

This nonconformance, which was issued after the matter had been flagged as a continuing problem by QC Monitor Surveillance Reports, documented that Phillips-Getschow Procedure PGCP-5 was not implemented in its entirety. This procedure applied to the control and warehousing of unused portions of revised fabricated spool and the control of their use in permanent systems. Contrary to PGCP-5, Field Change Orders did not reference the use of PGCP-5, fabrication documentation was not issued with directions to warehouse the unused portion of a revised spool, and files were not maintained for record purposes.

The corrective action was to implement immediately PGCP-5 and all users of PGCP-5 were to receive intensive training on its use. This was dono. The Phillips-Getschow Engineering Department was assigned the responsibility to conduct a random sample of pipe spools installed during the time the PGCP-5 was not being followed. These inspections were not being pursued promptly, and for this reason, I assumed responsibility for completing the correcti.e action. These inspections were included as a 0094H/ August 10, 1985

part of the MTV random sample program, and eventually the large bore pipe involved was included in the 100% reinspection effort under the MTV Program.

4 The Board of Boiler and Pressure Vessel Inspectors conducted an audit at Braidwood Station. The results of the audit are set forth in Interim Report No. 1, dated April 1, 1985. It was reported that dispositions under two NCR's were incorrect. Copies of the audit report and the responses prepared by Phillips-Getschow and Commonwealth Edison are enclosed. Since this matter involves deficiencies in the nature of the. corrective action, I do not-consider it to be an example of a failure by Phillips-Getschow "to implement ~a corrective action" as those words are used in the interrogatory.

inspections, were problems identified requiring items of large and small bore pipe to be removed? Problem identified with inaccessable pipe?

Were problems identified with lack of documentation identifying components installed? If so, please describe in detail the circumstances and explain why this problem occurred and all corrective action taken.

RESPONSE

Under the MTV program, PGCo Q.C. conducted a 100 percent field reinspection.of accessible ASME Section III small bore piping installed prior to September 6, 1983 and accessible ASMR Section III large piping installed prior to January 1, 1983. The reinspection effort has been

  • completed. 738 non-conforming items have been identified, and NCRs have been issued to track and disposition the small and large bore pipe whose traceability was in question. The NCR's referred to in this answer are available for inspection and reproduction upon request.

The following circumstances result in generation of an NCR for piping items due to traceability questions:

(1) No identification on either the piping item or the traceability l

document, the Stores Request.

(2) Only a portion of the full identification number is physically marked on the piping item.

(3) An unknown number is physically marked on the piping item.

(4) Identification number on the piping item is not the same identification number docunented on the Stores Request.

A further objective of the MTV program, for those items for which traceability had been established, was to assure that the installed hardware complied'with the design requirements, i.e., the specifications of the applicable Sargent & Lundy Piping Design Table. Any item that did not fully comply with each of the. design specifications was documented as nonconforming and an NCR was issued. This process resulted in 96 non-conforming items. The following circumstances gave rise to these nonconformances.

1) Piping item installed is of lesser wall thickness than specified.
2) Piping item installed is of greater wall thickness than specified.
3) Piping item installed is of lesser ASME Code Class than specified.
4) Piping item installed is of dif ferent material type / grade than specified.

Each of the documented nonconforming items was or is being evaluated for ASME Code compliance. As a result of this evaluation, each nonconforming item was (or will be) either accepted "as is" or rejected, 1

cut out, and replaced. All traceability problems were (are being) '

evaluated by reviewing documentation other than the Stores Requests which might establish the item's traceability. Any item that was/is not traceable by review of other documentation was/will be cut out and replaced. The nonconforming items which did not compl* with the design requirements were/are being evaluated by Sargent & Lundy. Those nonconforming items found to have design significance were/will be cut out and replaced.

SPECIFIC INTERROGATORY 37 Please describe in detall'any programs at Braidwood and Commonwealth Edison's LaSalle nuclear plant for the re-route of small bore piping, including particularly any material differences between the programs now or in the past. Has the NRC ever identified and deficiencies in such a program at Braidwood? If so, please describe in detail such identification and any response and corrective action. Please identify any documents reflecting such programs, deficiencies, responses, and-corrective actions.

RESPONSE

This answer is limited to the small bore process piping re-route program at Braidwood since an objection has been made with respect to any such program at LaSalle.

Prior to July, 1983, small bore (i.e., two inch and smaller nominal process pipe size) pipe re-routing was controlled by Phillips-Getschow Procedure QCP-B21 (Revision 4). This procedure reflected the Specification requirements and procedural criteria established to control small bore pipe re-routing. These criteria included drawing changes marked up by production personnel (for ASME Section III Class 2 and 3 only), review of weld and band additions or deletions by Quality Control and the ANI, and red line mark-up of the drawing by production to reflect actual field routed conditions.

The conceptual pipe routing drawing, initially designed by Sargent &

Lundy, was to be used as a basis for field installation. Additionally, the S&L Specification allowed Phillips-Getschow Company to re-route small bore piping when required to clear conflicts or interferences with certain pre-approved guidelines. This program required the submittal of "as constructed" piping drawings to S&L for review prior to system acceptance.

In Inspection Report 83-09, NRC Region III identified deficiencies in the previously described Braidwood small bore piping re-route program. The specific deficiencies are detailed in 83-09; however, they can be summarized generally as a lack of sufficient control of the pipe re-routing process.

The corrective action taken to respond to 83-09 was to revise the procedure QCP-B21 via supplement revision 2. This revision required Phillips, Getschow Field Engineer to review the proposed piping re-routes, via the marked up piping drawing, prior to final installation activities. Revision 6 of QCP-B21 added an engineering department office review and approval of the piping reroute prior to final installation completion. The Phillips-Getschow piping as-construct efforts which existed at that time included a Quality control dimensional verification of the piping configuration, addressing past piping reroutes.

In April 1983, 2 inch and under process piping was required to be installed in accordance with Phillips, Getschow Procedure QCP-B21, Rev.

4. The Phillips, Getschow Site Q.A. Group conducted an audit (Audit No.

83BR3) of this activity on April 12 thru 20, 1983, and identified deficiencies in the implementation of the following areas of the procedure:

FIELD-REROUTED PIPE:

1. The procedure allowed the Superintendent to make changes to the approved conceptual routing for ASME Class 2 & 3 small bore piping, provided he note the changes on the drawing in red ink and initialed and dated the entries. The audit identified inconsistencies in the marked up drawings, (i.e., changes were not noted, not initialed, not indicated in red ink).

SMALL BORE HEAT NUMBER TRACEABILITY:

2. In instances where two different heat numbers of material were used which were of the same size and type of material, the Superintendent was required to indicate the location of those heat numbers on the " plan view" of the drawing where they were used. The Q.C. inspector was required to verify these entries.

The finding identified instances where the Superintendent failed to indicate the location of these heat numbers on the " plan view".

LACK OF QC/ANI REVIEW:

3. If during installation, a routing change involved the addition or deletion of a weld or bend, the Superintendent was required to contact the Q.C. inspector and the ANI (ASME only) for assignment of Inspection Hold Points. During the audit, instances were noted where welds or bends had been added or deleted without Q.C. or ANI involvement.

In view of these audit findings, PGCo. Q.A. recommended that work be stopped on small bore pipe field routing. However, ,

based on the initiation of the following corrective and preventive actions, a stop work order was not issued: -

1. The status of all previous work was to be identified within approximately 90 days. This consisted of re-verificction of the as-installed dimensions and material identifications for small bore piping, per the requirement of Procedure QCP-B21.
2. Special training, concernin6 field routing identification requirements, and the need to involve the Q.C. inspector and the ANI, was to be given to all foreman and general foremen working in the area.

The special training noted above was completed on May 28, 1983.

During the re-verification effort, Procedure QCP-B21 was revised to include the requirements for Quality Control inspector verification of all small bore material. This procedural change was implemented to ensure small bore piping heat traceability of all future work.

In a follow-up audit 83-BR3 that was conducted'in July, 1983, it was determined that despite the training referred to above, production personnel were still not properly recording pipe re-routes. As a result of these problems, PGCo Q.A. issued a stop work order on July 22, 1983.

During this time period, Commonwealth Edison and the NRC had also identified similar problems.

The corrective action taken included a revision to Procedure QCP-B21, that required the production foremen to present all proposed routing changes to the PGCo Field Engineer for review and approval. The field Engineer was required to review the proposed routing change, to determine if they were within the acceptable design criteria, and properly incorporated onto the drawing. He was required to indicate his approval by initialing and dating each change, and to obtain Q.C. and ANI review, where required, prior to allowing production foremen to continue.

Training on these revisions was completed July 28, 1983 and the stop work order was lift by PG Q.A. on July 28, 1983. PG Q.A. performed four (4) follow-ups of these actions, from 7/26/83 to 9/26/83, and no problems were identified.

To verify the acceptability of previous installations, drawings identified in the initial re-verification commitment were placed under the scope of PGCP-40. This procedure consisted of a program for complete PGCo Q.C. dimensional verification. A subsequent PGCo Q.A. follow-up audit verified the effectiveness of these actions. This concludes the discussion regarding small bore re-routes.

PGCo Internal Audit 83-BR3 remained open to track the progress of efforts being made to establish material traceability of these previous installations. The subject audit was closed in April, 1984, after a formal mechanism had been established to ensure the re-verification of all existing installations for proper material traceability. Procedure QCP-B31, " Material Traceability Verification Program", was developed, and implemented to resolve these material traceability issues.

Subsequent audits by PGCO Q.A. of the installation and/or field routing of 2" and under Process Piping; Audit Reports 84-BR12 and 85-BR15 verified effective implementation of the corrective actions taken.

SPECIFIC INTERROCATORY 42 Has Commonwealth Edison ever identified incorrect or indeterminate piping items installed at Braidwood? If so, was such identification through a sample inspection? Is it true that the Braidwood program did not require independent verification of all piping components installed?

Was there ever a potential for inadequate quality of installed piping which could result in failures during plant operations? If so as to any of these questions, please describe in detail the circumstances and explain why these problems occurred and what corrective action has been taken. Set forth any facts which demonstrate that such corrective action has been effective. Please identify any documents which reflect these answers.

RESPONSE

Based on a review of the records, incorrect or indeterminate piping was identified as installed on three~ occasions.

1. In July, 1983, Ceco Quality Assurance performed scheduled Audit QA-20-83-83. This Audit reported Finding #1, that one material heat of ASME Section III Class 2 pipe was found installed.as a part of an ASME Section III Class 1 pipe system. Under the requirements of ASME Section III, for material to be designated as Class 1 it must have either a non-destructive examination (NDE) performed or a lower stress allowable used in piping analysis. For Class 2 material, such an examination is not normally required. It was subsequently identified that the heat number of the pipe material in question was traceable to both Class 1 and Class 2 certifications. However, unique identification to distinguish Class 1 material had not been established or maintained and as a result the Class 1 material could not be distinquished from the Class 2 material after installation. As such, potentially, some of the material had the required NDE and some did not. This condition is being l

addressed via PGCo. NCR #1159 and is also being evaluated per the MTV program for material correctness.

2. The answer to Interrogatory 31 identifies and discusses the pipe cut outs and piping items under NCR disposition identified during the course of the Material Traceability Verification Program.

3. On June 21, 1984, Commonwealth Edison Company notified the NRC 1

of a potential deficiency reportable pursuant to 10CFR 50.55(e) regarding wall thickness inadequaties for certain small bore piping procured as ASKE Class II. Edison reported that initial inspections had determined that, for piping heat #KD6751, approximately 25% of-the 2-inch schedule 80 carbon steel pipe in storage had wall thickness less than that required by the I

material specification. Subsequently, on July 20, 1984, Edison '

provided on interim report, assigning number 84-10 to this 50.55(e), and stated that the reduced wall thickness, primarily caused by pits and grooves, apparently was the result cf corrosion. Additionally, quantitles of this pipe had been installed in various safety-related systems. The 50.55(e) report also stated that "the reduced wall thickness could result in certain piping systems not meeting the code stress allowables. If left undetected, this condition could potentially result in pipe failure and negatively impact on its associated system safety functions."

Subsequent interim reports by Edison indicated that the subject pipe had been subjected to extend periods of outdoor storage, and also revised the scope of the problem to include additional heat numbers of small bore pipe which had been subject to similar conditions. It was also reported that subsequently, a chemical cleaning process had been purchased as a non-safety.

related standard commercial service to clean portions of this piping.

Edison initiated NCR#633 to track the problem, and subsequently revised it to include the additional corroded piping. NRC reported the corroded pipe problem in inspection Report 84-17, subsequent to CECO's report 55(e) No. C4-10.

An engineering evaluation of the installed corroded piping is in progress and nearing completion. A summsey of the approach taken and results obtained thus far was presented to the NRC on August 8, 1985. The results of the engineering evaluation, although not yet final, suggest that installed piping identified under NCR#633 Rev. I will be acceptable from an engineering viewpoint. The final results and engineering report will be made available when issued. Subsequently, the installed piping will be dispositioned by CgCo.

The interrogatory questions whether it is "true that the Braidwood program did not require independent verification of all piping components installed?" This question is not understandable because the " program" 0097H/ August 10, 1985

referred to is not identifiable in the context of interrogatory 42.

Moreover, there is uncertainty as to what the phase " independent verification" means. It is not clear what type of independent verifier the author of interrogatory 42 had in mind.

RESPONSE

In their Inspection Report No. 83-09, the NRC found that random visual checks of interface dimensions of piping components was not being done. Additionally, the NRC found that no documented record or log specifying that a calibrated instrument was used to measure numerous pipe bends for ovality. The NRC stated in their report that since measurements of wall thickness had not been made upon receipt of pipe to assure quality, the design basis for not taking thickness measurements after bending requires further review. This was identified as unresolved item 83-09-03. The NRC did not, to my knowledge, state that there existed a poteatial for minimum wall violations which could result in piping failures during plant operation.

The corrective action taken with respect to receipt inspections of large and small bore pipe involved a revision (Rev. 1, dated March 14, 1983) to a Phillips, Getschow document. QAP-33 advanced revision supplement, Rev.0, dated June 24, 1983, was issued to provide Q.C.

inspection for pipe wall thickness verification.

1 A program was initiated to determine whether problems existed with respect to the small and large bore received on site during the time period when receipt inspections did not include wall thickness measurements. Inspections were performed of large and small bore pipe in storage. A sample inspection of each pipe heat was performed and it was

, determined that the pipe was acceptable.

l Wall thicknesses inspections were performed on small bore pipe heat numbers, installed in safety related systems, that were no longer 1

available in storage. No wall thickness less than manufacturer's specifications were identified.

I More recently, it has been determined that with respect to two heats of small bore pipe, one piece of each heat was identified as not meeting l

the manufacturer's specification for wall thickness. Also, the origin of the material in two 20-foot lengths of large bore pipe is unknown. These most recent findings are presently being addressed, evaluated, and they

. are being tracked under the NCR system.

l Regarding the lack of documentation verifying that a calibrated instrument was being used to measure pipe bend for ovality, Phillips, Getschow's initial corrective action involved the issuance of instructions to all Q.C. Inspectors in July, 1983 requiring that a calibrated instrument number be documented on the appropriate bend traveler. In September, 1983, PGCP-11 was revised to require Q.C. to list the calibrated instrument number for each bend.

Corrective action was also taken to verify the acceptability of previous work. PGCo reviewed-its calibration records for dial calipers and they determined that none of the calipers previously used had ever gone out of calibration or required repair or adjustment. In addition, sample inspections were conducted with respect to small bore piping to provide assurance that pipe bends met ovality requirements. The sample of 64 pipe bends was inspected and 7 items failed to meet the manufacturer's specifications for wall thickness and/or ovality requirements. These sample results are currently under evaluation to determine what further action may be necessary. This matter is being tracked under Phillips, Getschow NCR #3962.

This answer addresses audits conducted by the Phillips, Getschow Company and Pullman Sheet Metal. In Inspection Report 83-09, the NRC found that Phillips, Getschow Company had not established and executed a plan for auditing the implementing procedures of the Quality Assurance Program on a periodic basis to determine the effectiveness of the program in accordance with the PG Quality Assurance Manual, Section 16. Although implementing procedures were the subject of audit during the time period of interest, NRC's finding was based on the fact that no formal written Phillips, Getschow document existing at the time described any plan or schedule for the conduct of these audits. The Phillips, Getschow Company Quality Assurance Program only. required that all Quality Assurance Manual sections to be audited during the course of a 12 month period. This requirement met and fulfilled ASME Code requirements.

The Phillips, Getschow Company Q.A. Manual was revised on August 16, 1983 to include auditing requirements for implementing procedures. The audit schedule was also revised to include such audits. The effectiveness of the revised audit plan was verified by CECO. Quality Assurance Audit 20-83-110.

' described, in 1983, another auditor was employed at Pullman in order to maintain the audit schedule as required by 10 CFR 50, Appendix B, Criteria XVIII, audits. Further improvements to the Pullman Audit Program are described in the July 25, 1984 letter from Michael Jarigese to Mr. Shamblin (pages M0001031-M0001032). Pullman Sheet Metal Works, Inc. (Braidwood Site) wrote an Audit Procedure (B18.1.F) and a Procedure, Qualification / Certification of Audit Personnel (B2.4.F) in early 1985.

(pages M0001033-M0001071). On January 2, 1985, Pullman Construction Industries, Inc. (Corporate) transferred'a full time auditor to perform scheduled and unscheduled audits at the Braidwood Site.
