filed their First Set of Quality Assurance Interrogatories and Request to -

filed objections to certain of those discovery requests and provided to Intervenors' counsel a partial response to Intervenors' discovery requests. Applicant's first partial response was revised and resubmitted on August 1, 1985. Applicant filed a second partial response on August 5, 1985, a third on August 10, 1985, and a fourth on August 13, 1985. j This submission, whi.ch is Applicant's fifth partial response to Intervenors' discovery requests, provides responses to Specific l l

Interrogatories 19, 20, 21, 22, 23, 52. 53, 54,.55, 61 and 62 and  ;

For each such person no longer employed in Quality Assurance / Quality Control, indicate the reason for termination. For each such person involuntarily terminated, describe in detail the circumstances of termination.

Applicant has objected to the scope of Specific Interrogatory 19 and this response is limited accordingly. A list of Commonwealth Edison Quality Assurance personnel employed at Braidwood, (Attachment 1.)

is aware that there may be a possible problem.

When a person leaves the employ of Comstock, an exit meeting is held between the QC Manager, or his designee, and the employee. At this time, i

company policies concerning such matters as accrued sick, personal or {

vacation time, and insurance are explained. Employees are given a form at this exit meeting on which they are asked to sign voluntarily a statement to the effect that they are unaware of any reportable )

i conditions and/or defects that have not been properly reported to l

Comstock's management. If the employee does not wish to sign the l statement, the employee is asked to indicate to management in writing all i

items of concern so that corrective action may be taken to correct the problem or concern. See page 00001027. Any such items are brought to the attention of Commonwealth Edison Company. Since the Commonwealth  !

4 Where the cable' terminator installs a piece of sealtite, whether or not it constitutes a complete run or is used to complete a previously installed run, he must initiate a Conduit Installation Report per procedure 4.3.1 (Conduit Installation) Paragraph 3.19 and submit to his foreman who must inspect the conduit per Procedure 4.3.1, Paragraph 3.20. This applies to both safety and non safety related conduits.

Examples of these situations would be a sealtite run from a junction box to a valve in which the sealtite has been assigned a unique conduit number, or a piece of seal, tite used to connect the end of a 3 rigid conduit with a cable pan, instrument or panel, etc. which had 1 j not been attached upon original installation of conduit.

Procedure requirements as started in paragraph 3.1.8 are that proper bolts are to be installed and that bolted connections are tight and that any applicable bolt'tarques have been checked in accordance to section 3.2. Section 3.2 gives you cap screw size and the specific ft. lbs. these cap screws are to be torqued to. This inspection is not being performed by configuratiqn inspectors.

The configuration inspectors I have talked to and my lead and supervisor told me to check N/A because the conduit inspector will check for bolt

torque. Regardless of who else checks torque, procedure 4.8.12 Rev. .  ;

states that the configuration inspector shall check torque. The '

l configuration inspec'.or is instructed to do so by his procedure and thus should perform this inspection. It war suggested to me that the config. i inspection could verify torque by researching for a Form 60A (conduit ,

chccklist). This is not a valid argument since none of the configuration '

inspectors are doing this and have not been instructed to do so.

RESOLUTION This is presently being evaluated by Engineering.

Procedure 4.1.2, Rev. B, Paragraph 1.1 states in part that the Project Manager "is responsible for all construction activities." This means that the evaluation of inspection results are being performed by a group related to production.

SUPPLEMENTAL, RESPONSE Yes. NRC Inspection Report No. 54-456/83-09 and 50-457/83-09 (Audit conducted in 6/83, 8/83, and 10/83) identified the following deficiency with L.K Comstock Internal Audit. -

i L.K. Comstock Company /L.K. Comstock Engineering Company auditing activities neither conformed with"the comprehensive annual schedule of planned and periodic audits established as required by Quality Assurance Program Manual Section 4.14.1, nor did they verify

. , compliance with all aspects of the Quality. Assurance._ Program. _ . . -

Applicant acknowledged that Comstock did not fully implement their 1983 audit schedule. Comstock's inability to fully implement the audit schedule was due to the illness and subsequent resignation of the Comstock Quality Assurance Engineer, C. Stiles, responsible for these audit activities. Mr. Stiles' illness occurred in May 1983, prior to the NRC inspection in June 1983. Mr. Stiles resigned from Comstock in July l 1983 and his position was filled by Mr. Seltman in September 1983.

Although Comstock audits were not conducted as scheduled, adequate Commonwealth Edison Quality Assurance audits were performed to cover all applicable Comstock activities. During the inspection visit, the NRC 0099H/ August 13, 1985

acknowledged that Commonwealth Edison Quality Assurance provided adequate quality audit coverage and that Site Quality Assurance was previously i

aware of the Comstock failure to conform to its annual audit schedule.

This review disclosed that there was sufficient objective evidence recorded to support the conclusions stated in the audit reports.

Additionally, Procedures (4.14.3 and 4.14.1) have been revised to more clearly define the Comstock on site auditing activities and qualification processes for site auditors.

Comstock site Quality Assurance submitted an audit plan for 1984 which indicates that the Quality Assurance Program and their active safety related activities will be adequately covered. All applicable site procedures were audited by the week of' January 9, 1984 Procedures l

4.14.3 and 4.14.1 were revised to reflect on site auditing activities and _ ,_

qualification processes. Interim approval was granted on 2/29/84 and l

11/02/83 respectively.

NRC Inspection Report 50-456/84-44 and 50-457/84-40 (conducted 12/84 and 1/85) revealed LKC Internal Audits were comprehensive audits. This is illustrated through the following statement in the subject NRC audit.

The element of the applicant's audit program which included audit j reports, schedules and follow-up to audit findings, were reviewed l and determined to be comprehensive. These audits are identifying I not only lack of implementation, but also programmatic problems.
