On 8/10/2011, Entergy Palisades reported to HydroAire the failure of a pump in their service water system (see attached email dated 8/10/11.) The current failure occurred in a threaded line shaft coupling in Palisades' 7C service water pump. The pump coupling was purchased under Entergy Palisades P.O. 10253715 Rev. 03, HydroAire job NQ-5912. (Entergy Cat I.D. 0032069405 1).
This notification is to conform to the requirements of 10 CFR Part 21 and HydroAire PCP 041 Rev. 6.
At this point, the failure cause is not known, however, Palisades and their outside consultants are currently performing the root cause analysis (RCA). HydroAire has been requested by Entergy Palisades to not participate in the RCA currently underway.
This same pump (7C) was documented in HydroAire's December 16, 2009 Part 21 notification to the NRC (attached). While the current line shaft coupling failure appears similar to the 2009 line shaft coupling failure, any correlation between the two can be determined once the current RCA is complete.

Palisades Failure - Update Dibu Update from Palisades as of 7:45 am on 8/10/11:
They do not have a cause of failure as of yet Location at plant - P7C Pump that is in that position - HydroAire job NC-5912 Palisades PO 10253715 (This was the same pump that failed where we supplied new parts back in October 2009)
Palisades is in the process of removing the motor and head this morning.
Upon instructions from Palisades (John Kasishke), the new service water pump (NQ-5940) shipped out late last night from Chicago and arrived at the station at 4:00 am. Palisades QA and engineering are in the process of receiving the new service water pump and documentation.

Part 21 Notification

Dear Sir or Madam:

On 8/10/2011, Entergy Palisades reported to HydroAire the failure of a pump in their service water system (see attached email dated 8/10/11.)
