On April 23,  1976 we submitted Reportable Occurrence 76-12, 30-day report, Containment      Personnel Airlock Handwheel Shaft Seal Excessive Leak-age During Testing.        In that report the statement was made that action to prevent a recurrence      of that event was being investigated.
This is to inform you that the corrective action planned is to place keepers on the packing gland nuts to prevent them from rotating with the shaft, precluding their loosening and permitting leakage through the packing.
These keepers will be installed at a future plant outage when the door assemblies can be dismantled for this maintenance.
In addition, procedure changes have been submitted to require as initial conditions that a mechanic verify proper seating of the packing gland nut prior to performing the applicable tests.
Two additional copies of this letter are enclosed to conform to the original submittal of Reportable Occurrence 76-12, 30-day report.
