| | The purpose of this letter is to provide the FirstEnergy Nuclear Operating Company (FENOC)visual examination results for the DBNPS Reactor Vessel Closure Head (RVCH). During the DBNPS fourteenth refueling outage, completed on April 27, 2006, FENOC performed a bare metal visual inspection of 100 percent of the reactor vessel head penetrations, including 360 degrees around each of the vessel head penetration nozzles. Visual inspection of greater than 95 percent of the RVCH surface was also performed to identify any degradation. |
| | In addition, FENOC performed a visual inspection to identify potential boric acid leaks from the pressure retaining components above the RVCH. These inspections were conducted using VT-2 qualified personnel and procedures. |
| | As identified in a letter from FENOC (Serial Number 1-1400, dated February 3, 2005), inspection conducted during a mid-cycle outage in January 2005 identified a small build-up of material and stains on or near some of the RPV upper head nozzles.
