This report is due within 90 days after the end of the Unit 1 May 2011 refueling outage, which is September 8, 2011. This report documents examinations performed during the third period of the fourth inspection interval.

Outage, Period and Interval Start and End Dates This Inservice Inspection Summary Report encompasses examinations and repairs performed from May 25, 2009 through June 10, 2011 during the fourth ten-year interval third inspection period. This most recent refueling outage was the 2 1 st refueling outage for Quad Cities Nuclear Power Station Unit 1 and is the first outage for this unit during the third inspection period. The outage started May 9, 2011 and was completed June 10, 2011 when the unit returned to service.

Tests, examinations, and repair/replacement activities associated with American Society of Mechanical Engineers (ASME) Code,Section XI were witnessed or otherwise verified by the Authorized Nuclear Inservice Inspectors. Non-ASME Section XI or augmented examinations were not witnessed or otherwise verified by the Authorized Nuclear Inservice Inspectors and are marked "Not Required" or "N/A" on the applicable examination reports.

The Authorized Nuclear Inservice Inspectors are inspectors of the Hartford Steam Boiler Inspection & Insurance Company of Connecticut (HSB-CT). The HSB-CT Regional Office is located at 2443 Warrenville Road Suite 500, Lisle, Illinois, 60532-1098.

1.2. Examination Program Requirements The examination requirements of ASME Boiler and Pressure Vessel Code,Section XI, Division 1, "Rules for Inservice Inspection of Nuclear Power Plant Components", 1995 Edition through 1996 Addenda were implemented for Class 1, 2 and 3 components except where written relief has been granted by the NRC. Written relief, in the form of Relief Request 14R-02 approving Risk Informed Inservice Inspection (RI-ISI) as an inspection program alternative for Code Categories B-F, B-J, C-F-1 and C-F-2, was utilized during Q1R21. The ultrasonic examination volumes for RI-ISI components were increased from the Section XI requirements to meet the RI-ISI Program.

The ultrasonic examination requirements of ASME Boiler and Pressure Vessel Code,Section XI, Appendix VIII, 1995 Edition with Addenda through 1996, as required by 10 CFR 50.55a, "Codes and Standards", and amended by the Federal Register Notice 64 FR 51370 dated September 22, 1999 is executed through the Performance Demonstration Initiative (PDI) Program according to the schedule defined in 10 CFR 50.55a(g)(6)(ii)(C),

for wrought austenitic piping welds, ferritic piping welds, bolts/studs, reactor vessel welds, nozzle to vessel welds, nozzle inside radius sections and full structural overlaid wrought austenitic piping welds.

Section XI Examinations Summary A combination of ultrasonic (UT), liquid penetrant (PT), magnetic particle (MT) and visual (VT) testing were performed prior to and during the 2 1st refueling outage on ISI Program components. Examinations were performed by General Electric (GE) and Exelon Nuclear personnel. Volumetric and surface examinations were compared with the recorded results of previous examinations for applicable Class 1, 2 and RI-ISI components. ISI inspections comprised of 44 volumetric (21 were RI-ISI, Category R-A welds), 10 surface examinations and 44 visual examinations were performed on Class 1, 2 and RI-ISI components. One of the visual exams was a subsequent exam due to IWF-2420(b) for corrective action from the Second Period. There were 4 volumetric examinations that did not meet coverage requirements of ASME Section XI Code (i.e. greater than 90 percent of the specified examination volume) during Q1R21 (Table 3.1-1B). Code cases used for ISI or Repair/Replacement activities are listed in the ISI Program Plan.

Pressure testing VT-2 examinations not related to repair/replacement activities are documented in Table 3.1-2, "Quad Cities Unit 1 Pressure Testing Summary". The Class 1 and 2 System Leakage Tests were performed in accordance with the 1995 Edition 1996 Addenda of the ASME Code and applicable Relief Requests as listed in Table 3.1-2 for the third period of the fourth Interval.

Exelon personnel performed functional tests on a total of twenty-seven (27) snubbers for operability per the Service Life Monitoring/Failure Trending Sample and ASME OM Code Subsection ISTD. Thirteen (13) snubbers were selected for the 10% sample group testing with the remaining fourteen (14) being tested for Service Life Monitoring. Nineteen (19) of the twenty-seven (27) snubbers were ASME Section XI Code designated. In addition, twenty-seven (27) replacement snubbers were tested and installed in accordance with a planned snubber replacement plan which is part of the Service Life Monitoring strategy. Of the fifty-four (54) snubbers tested, one (1) Mechanical snubber and one (1) hydraulic snubber that were removed from the plant for Service Life Monitoring testing were found to be unacceptable for continued operation. The failures were reviewed and no sample expansions were required. The failed hydraulic snubber was replaced with a like for like unit and the failed mechanical was replaced with a hydraulic snubber that is more robust to the conditions experienced at the installation location. Snubber testing activities are summarized in Table 3.1-3, "Quad Cities Unit 1 Snubber Functional Tests.

Table 3.1-4 "Quad Cities Unit 1 Conditions Noted on ASME Code Components and Corrective Actions" summarizes the conditions noted during the examinations and corrective measures taken prior to returning the unit to service.

Table 3.3 identifies number and percentage of examinations completed in the fourth interval.

BWRVIP-75-A/Generic Letter 88-01 Augmented examinations required by Generic Letter 88-01, "NRC Position on Intergranular Stress Corrosion Cracking (IGSCC) in BWR Austentic Stainless Steel Piping" and BWRVIP-75-A, "BWR Vessel and Internals Project Technical Bases for Revisions to Generic Letter 88-01 Inspection Schedules", were implemented in this outage (Table 3.2-1).

Thirteen Category C Welds (susceptible to IGSCC but mitigated by stress improvement) were examined. Three Category E Welds (cracked weldments that have been mitigated by a weld overlay) were examined. Two of these welds (02AD-F12 and 02BS-F4) were overlaid in Q1R18 and received their subsequent examination in accordance with B WRVIP-75-A.

All welds had no relevant indications. This is consistent with previous examination results.

3.3. In-Vessel Inspections (IVI)

Relief Request 14R-15 was approved as an alternative to Category B-N-1 and B-N-2 requirements. Attachment Weld Exams performed under BWRVIP-48-A during Q1R21 included:

" Visual inspections of the Steam Dryer wall support lugs in accordance with BWRVIP A (EVT-1, VT-3)

" Visual inspections of Jet Pump (JP) 4 RB-la Primary Riser Brace Leaf to Vessel Pad (EVT- 1)

• Moisture Separator Guide Rod Support Bracket Attachment Upper Weld at 200 Degrees (EVT-1, VT-1, VT-3)

Augmented examinations were performed to implement additional BWRVIP requirements in accordance with the guidelines listed:

• Visual inspections of Jet Pump Assemblies in accordance with BWRVIP-41 Revision 2

• Visual inspections of Jet Pump riser welds in response to Laguna Verde operating experience supplemental guidance per BWRVIP Letter 2009-202

• Visual inspections of Core Spray piping welds and spargers in accordance with BWRVIP-18 Revision 1

• Visual and UT inspections of Core Spray piping welds in accordance with BWRVIP-18 Revision 1.

• Visual inspections of Core Shroud Vertical welds in accordance with BWRV1P-76-A, EVT-1

• Visual inspections of Core Shroud Horizontal welds H8 and H9 in accordance with BWRVIP-76-A, EVT- 1

• Visual inspections of the Steam Dryer in accordance with GE recommendations and guidance provided in BWRVIP-139

• Visual inspections of the Access Hole Covers in accordance with BWRVIP-180, VT-i.

• Visual inspections of the Jet Pump Riser RS-2 welds on all 10 risers in accordance with BWRVIP-41 Revision 2.

• UT inspections of eight Jet Pump Hold down Beams in accordance with BWRVIP-41 Revision 2

• Visual inspections of Core Shroud support structure welds in accordance with BWRVIP-38 7 of 19

Quad Cities Nuclear Power Station 300 Exelon Way Unit 1 Kennett Square, PA 19348 22710 206h Avenue North, Commercial Service Date February 18, 1973 Cordova, IL 61242 All eight Feedwater Sparger End Bracket pins were inspected during QIR21 as a follow-up to minor bracket wear from the pins identified during previous outages. The pinheads are wearing into the bracket hardware due to flow-induced vibration. This is not a concern for the immediate future since the brackets still have greater than 90% of original thickness remaining. No new or additional wear was noted in Q1R21. However, an indication was identified in the 206 degree Feedwater Sparger End Bracket Pin nut tack weld. These indications are documented in IR 1218182 and EC 384463.

Core Spray piping welds were examined (34 visual exams and 39 UT exams). In this population were 8 welds with previously identified indications. Each of these indications was examined in Q1R21 with no significant change in size. No additional indications were identified in the rest of the population. These indications are documented in IR 1218748 and evaluated under EC 384463.

Re-examination of indication of wear due to minor movement of the Core Spray Sparger Brackets was performed. The Core Spray Sparger Bracket at 190 degrees showed no change from the previous inspection in Q1R19. This indication is documented in IR 1218692 and evaluated under EC 384463.

Indications were observed in HAZ of Jet Pump 13/14 Riser pipe RS-2 weld. The scope of the RS-2 weld inspections was expanded to the remaining 5 RS-2 welds. No additional indications were observed in the other 9 RS-2 welds. These indications are documented in IR 1217155 and evaluated for continued service under EC 384624.

Multiple indications (gouges and rub marks) were identified on all four of the Steam Dryer wall support lugs. These indications were previously observed in Q1R19.

A comparison of the inspection results from Q1R21 to Q1R19 found no change to the previous indications and no new indications were identified. These indications are documented in IR 1218242 and evaluated for continued service under EC 384463.

Quad Cities will continue to monitor these conditions in accordance with the Reactor Internals Program.
