LER 2023-001-00 Human Performance Error Resulted in Compensatory Measures Not Properly Employed In accordance with the requirements of Title 10 of the Code of Federal Regulations Part 73, Appendix G, Reportable Safeguards Events, paragraph (I) (10 CFR 73 Appendix G(I), Holtec Decommissioning International, LLC (HDI) on behalf of Holtec Palisades, LLC (Holtec Palisades) is submitting the enclosed License Event Report (LER), 2023-001-00 for Big Rock Point. The event is reportable under 10 CFR 73 Appendix G(I)(c), Any failure, degradation, or the discovered vulnerability in a safeguard system that could allow unauthorized or undetected access to a protected area, material access area, controlled access area, vital area, or transport for which compensatory measures have not been employed. The enclosed Licensee Event Report (LER) describes discovery of a condition where compensatory measures were not properly employed.

This letter contains no new and no revised regulatory commitments.

On February 16, 2023, a Big Rock Point Central Alarm Station (CAS) operator ZDVWDVNHGZLWKUHPRYLQJa Protected Area (PA) early warning system (EWS) zone alarm point IURPVHUYLFHdue to frequent alarms caused by environmental conditions.

Compensatory measures were established for a single PA alarm point out of service. Shortly after task completion, a security officer identified that all alarm points associated with the EWS zone were removed from service and that compensatory measures for an EWS zone UHPRYHGIURPVHUYLFHwere not established. Alarm capability was immediately returned to service.

As a result, a one-hour event report was submitted to the NRC, in accordance with 10 CFR 73, Appendix G, section I(c).

As a result, a one-hour event report was submitted to the NRC, in accordance with 10 CFR 73, Appendix G, section I(c).

CAUSE OF THE EVENT The direct cause of the event was a human performance error by a CAS operator and peer checker that resulted in bypassing an entire alarm zone instead of the single alarm point. Contributing to this human error was the failure to use human performance error prevention toolV. Corrective actions were implemented to ensure compliance with regulatory and procedural requirements.

CORRECTIVE ACTIONS TAKEN The individuals involved in the event were UHPRYHGIURPGXW\DQGprovided remedial training on bypassing PA EWS zone alarm points ZLWKUHLQIRUFHPHQW of the expectation to use human error prevention tools. ,QIRUPDWLRQVKDULQJRQWKH
