Grand Gulf Nuclear Station Report Pursuant to 1 0CFR71.95(b) to report a failure to comply with conditions of Certificate of Compliance during a shipment

In accordance with 1 0CFR71.95(b), Entergy Operations Inc. (Entergy) herby submits this report regarding a failure to comply with the Certificate of Compliance (CoC) for liner Serial Number (SIN) TNRM75GG13.

In the second half of 2018 Entergy performed a spent fuel pool clean-up project, which included six shipments of control blades (CRBs) offsite for disposal. Subsequently, due to a number scheme error in the special nuclear material tracking system, it was identified that Entergy shipped six CRBs that were not scheduled to be shipped. This numbering scheme error resulted in the failure to comply with the CoC for liner S/N TNRM75GG13 on October 9, 2018.

This error was discovered on October 17, 2022.

Entergy entered the error into its corrective action program to evaluate and establish actions to prevent recurrence. Corrective actions included the performance of an inventory of the special nuclear materials, correction of the special nuclear material log, and the performance a dose profile for the six CRBs that were incorrectly shipped. These actions determined CRB 1903 has a 1.06E+03 Curies of Co-60 equivalence with an allowable shielding material weight of 102kg.

This exceeded the cask limit of 10 Curies/kg Co-60 (1.06E+03 Curies/1.02E+02kg = 10.3 Curies/kg). Thus, resulting in the TM-RAM cask CoC being exceeded. This did not result in a failure to properly survey the cask, placard the cask, or exceeding dose limits to the public.

The control rod blades were shipped to Waste Control and Storage Services in Andrews, Texas for disposal.
