'I.

Introduction The Executive Director for Operations on January 21, 1987 directed the formation of the Comanche Peak Report Review Group (CPRRG). The CPPRG was directed to evaluate the safety significance of OIA findings in 0IA file #86-10 and recommend resolution of the items. Three Task Groups were formed to evaluate and report to the CPRRG their findings on specific issues raised by the OIA report 86-10. Task Group I was assigned the task to " identify and report whether the total agency review and inspection effort at Comanche Peak meets, or is equivalent to, the current IE construction inspection program." This task should significantly contribute to determining -if the current augnwnted review and inspection effort at Comanche Peak is sufficient to compensate for i

any identified weaknesses in Region IV's implementation of the prescribed inspection program.

This document is the report of Task Group I's evaluation. The report contains four Sections.

Section II presents the Summary of Results.

Section III describes the Evaluation of Inspection 1

Coverage of the Task Group's review. References to the documents that provide the basis for the information contained in this report are presented in Section IV.

When considering all inspection and review activities at Comanche Peak, i

few gaps were found regarding inspection coverage relative to the curren l

NRC construction inspection program as described in Inspection and Enfor l

ment Manual Chapter 2512 (IE MC 2512).

The most significant weaknesses 1

identified in Region IV inspection coverage were in the quality assuranci area. These weaknesses have been substantially compensated for directly by inspection and review activities accomplished by Construction. Apprais.

l Teem (CAT), Special Review Team (SRT), Technical Review Team (TRT) and other NRC headquarters led inspection and review activities.

It is the Task Group's view that these weaknesses have also been compensated for indirectly by the extensive hardwere re-inspections and documentation reviews done as part of the licensee's Comanche Peak Response Team (CPRT:

prog ram.

While it should be recognized that missed inspections of a i

programmatic nature cannot be fully compensated for by later hardware anc l

record review inspections, the Task Group considers that the collective inspection coverage of other augmented inspection and reviews, which were performed by all NRC sponsored activities along with the CPRT program, sufficiently addressed the quality assurance programmatic requirements

to compensate for weaknesses found in Region IV's quality assurance inspections.

In its review, the Task Group identified several specific gaps in inspection coverage (see Table 1) where additional attention may be necessary either in the form of additional inspections or reviews to ensure that adequate NRC coverage has been provided or will be provided l

for ongoing construction efforts.

Of the identified specific gaps, the Task Group considered the more significant to be:

1.

Electrical Components and Systems.

Review of installation contractor quality assurance progrum (IP-51051-02.01 and

'IP-51061-02.01).

2.

Instrument Components and Systems.

Review of installation contractor quality assurance program (IP-52051-02.01) and

[

inspection of receiving, storage, testing and calibration activities (IP-52053-02.02).

3.

Containment Structural Integrity Test.

Review of Test Records (IP-63050-023) 4.

Inservice Inspection (IP-73051).

5.

Fire Protection / Prevention (IP-42051C, 640518 and 64053B).

6.

Structurel Masonry Construction (IP-46061).

The Task Group's efforts were primarily focused on determining the extent of completion of the inspection program Et Comanche Peak Unit 1.

This was accomplished primarily by the review of documents.

In performing this review, the Task Group did not evaluate technical findings not.ed in the 1

inspection reports nor the quality of inspections in its determination of the adequacy of the construction inspection efforts performed.

The details of the Task Group's assignment and methodology are provided in Enclosure 1 to this report.

l III. Evaluation of Inspection Coverage l

Table 1 provides a listing of individual inspection procedure requirements where the Task Group could not find documentation to support a conclusions that the requirement was completed.

The analysis from which Table 1 was prepared is presented in Enclosure 2.

This enclosure consists of summary

sheets which includes those inspection procedure line items that were not completed - or partially completed - by Region IV. Also provided is infor-mation to show whether the uncompleted line item was completed by other, current augmented review and inspection efforts.

Inspection procedures are grouped into 17 inspection areas.

For each inspection procedure, the specific requirements (line items) are identified.

For each line item the Task Group, through review of documentation, determined the extent of inspection coverage of that item at Comanche Peak Unit 1 by:1/

(1) inspections conducted by Region IV (2) inspections or reviews conducted by SRT, CAT, or other headquarters-led efforts, (3) inspections or reviews conducted by TRT, and (4) inspections or reviews conducted by CPRT.

The inspection requirement line items listed in T6ble 1 as not being completed were determined after an analysis of the accumulative effort for all four of the above of inspections was made.

In general, the Task Group found that for Comanche Peak Unit 1 Region IV has essentially completed the total inspection effort required by the December 1986 version of IE MC 2512.

In the technical disciplines, only a few are6s were found where inspection requirements had not been covered.

For the most part, the Table 1 listings show the primary line items to be those of a programmatic nature where licensee /

contractor quality assurance and control programs and procedures may not have been inspected by Region IV in advance of the start of work.

A few of the line items on the list deal with record review and observation of work.

The most notable inspection deficiency is in the area of fire protection / prevention where it appears - at least by the records - that significant portions of the three inspections procedures were not implemented by Region IV. Another area where there is an apparent lack of inspection records or actual implementa-tion of special inspection requirements is in the followup to IE Bulletin 80-11, Masonry Wall Design.

This requirement is contained i t. IP-46061.

The Task Group could not establish that the Region had initiated timely followup on the Bulletin which would have been reflected to some extent in inspection efforts relevant to completion of IP-46061.

In the quality assurance area, there were some inspection procedures in which not all of the individual line items were completed.

Failure to complete the line items in these procedures does not necessarily mean that the inspection objective of the procedure was not met, as this determination is made from the collective inspection of many line items in various areas as stated in the insper'.n procedure.

However, for those inspection procedures where specific line items

. ere not inspected by regional inspection effort, they were inspected w

by inspections conducted by SRT and CAT. Although the Task Group determined that QA/QC inspections were not conducted at the programmatic frequencies specified, which represents lost inspection opportunities, the program was completed with the assistance of other inspections such as CAT and SRT. The inspection frequencies for the past few years have exceeded program requirements.

Since the evaluation of inspection coverage was made against the December 1986 version of IE MC 2512, some gaps in inspection coverage (particularly by the region) may be the result of changes in inspection requirements over time.

This factor was taken into consideration in concluding whether or not a gap in coverage represented a significant weakness.

Robert D. Martin's February 2,1987 memorandum to John Davis provides a detailed evaluation of Region IV's inspection activities at Comanche Peak, particularly at Unit 1 (ref. CPRRG 22).

The SRT, CAT, QA program review, Cable Tray and Supports inspection, and HVAC and Supports inspection appear to have been comprehensive team efforts that selectively focused on specific areas.

Similarly, the TRT effort as reported in SSER 7, 8, 9, 10, 11 and 13 covered a large number of specific issues in great detail.

In many cases, these inspection and reviews covered areas not literally included in the NRC inspection program.

However, the inspections were adequate in detail and scope to cover many 1

inspection program objectives.

specific in nature and we.re equivalent to inspection requirements which were also covered by Region IV inspections.

to inspection activities.

This program was uniquely designed to address concerns raised regarding the adequacy of the construction and design of Comanche Peak. Anulysis of efforts conducted by the CPRT to meet the objectives and requirements of the current IE MC 2512 inspection program indicates that their primary objectives were similar, in that both attempt I

to assess the quality of construction and adequacy of documentation of the plant.

While MC 2512 utilized an in-process review approach and CPRT utilized a post-construction hardware / documentation review, it is

the Task Group's judgment that the methodologies are sufficiently similar to yield comparable results.

The data sheets in Enclosure 2 and 3 identify where the licensee's CPRT inspections cover NRC inspection program requirements.

In many cases, the CPRT inspections were found to complement inspection requirements already covered by NRC inspection and review work.

However, there are cases where the CPRT inspection activitihs represent the sole basis for the Task Group's determination that NRC inspection requirements were covered. The Task Group concludes that this is reasonable based on the NRC's approval of the CPRT program and Region IV's monitoring of the re-inspection activities.

Region IV provided the Task Group with a status of the construction i

inspection program at Unit 2 (see References 5).

The status of the inspection program was considered by the Task Group to be consistent with the facility's construction completion status. There appeared to be no major weaknesses in inspection coverage according to this summary data.

A.

Task Group 1 was directed by the CPRRG to accomplish the following:

Selectively audit information provided by IE, Region IV, the Comanche Peak Project Office (CPP0) and others against the primary or sourci l

occument or other information to assess the accurucy and completeness

of the input provided.

2.

Compare the review and inspection efforts accomplished by (a) Region IV; (b) the Comanche Peak Project Office; and (c) any other elements cf NRC (such as inspections managed by IE Headquarters, NRR, or.othe'r Regions) against the current construction inspection program of IE.

This comparison shall be in terms of coverage of inspection objectives or requirements.

3.

Identify with respect to accomplishment of the inspection requirements or objectives of the current construction inspection program, as described in Inspection and Enforcement Manual Chapter 2512 (IE MC 2512):

(a) Whether or not inspections performed by Region IV inspectors covered the requirements of the current inspection program, and i

(b) Whether inspections or reviews performed by any other elements of NRC covered the requirements or objectives of the current inspection program.

The report should provide references to documents and other information sources that provided the basis for the identification and evaluation of inspection performance for items (a) and (b) above.

If, in the judgment of the Task Group, a requirement was only particily covered by inspections performed by Region IV, the report should, if available, state the basis for the determination,

Identify where inspections and reviews conducted as part of the Comanche Peak Response Team (CPRT) program demonstrate the accomplishment of the objectives or requirements of IE MC 2512.

Particular attention should be given to those inspection program requirements found not to be covered or only partially covered by Region IV or other NRC inspections or reviews.

The report should provide references to show the information that provided the basis for the identification 6nd evaluation of inspection performance.

For selected inspection attributes, conduct a detailed evaluation and comparison of the records addressing completion of MC 2512.

This evaluation and comparison shall be in terms of level of effort and timeliness of inspection or review.

B.

Methodology To accomplish the task described above a group consisting of five IE staff, one Senior Regional manager / staff, and a contractor was formed.

The Task Group was lead by Phillip f. McKee, Chief, ORPB, IE.

The members of the Task Group collectively had experience and knowledge in each of the construction inspection program functional areas, familiarity with the NRC construction inspection program, and familiarity with the inspection and review activities at Comanche Peak.

The Task Group review and analysis focused on completion of the NRC con-struction inspection program requirements at Comanche Peak Unit 1.

This was because the construction of Unit 1 is essentially complete and the construction inspection program as implemented by Region IV is also nearly complete. The Task Group has not conducted a comprehensive review of the 17

status of the inspection program at Unit 2.

The construction of Unit 2 is about 80% complete.

Commensurate with Unit 2's construction status, there remains a significant amount of NRC inspection yet to be accomplished, The CPRRG has asked Region IV to view and report on the status of the inspection program for Unit 2.

The tasks described above were performed primarily by review of the documented record.

Documents that are the basis for the findings in this report are identified in Section VI.

The Task Group's review methodology consisted of assigning individuals to-each of the major construction inspection program areas. The assigned individual was responsible for reviewing the documentation of inspection and reviewing activities for that ared and comparing the results of the review to the inspection requirements of the current (December 1986 version) construction inspection program.

The purpose of this review was to identify NRC inspection requirements that were not covered by any inspection and review activity.

For two areas, Quality Assurance and Welding /NDE, a more extensive review and comparison was made.

For these areas the comparison looked more closely at the level of effort and time-liness of inspection and review.

The specific results of the review for each inspection area were documented by the Task Group members.

The details of this documentation are presented in Enclosure 2 and 3 described in Section III of this report.

Based on the results of the review in each inspection area, the Task Group has identified several specific inspection items that were not covered, 10

or only partiallj covered.

The results of this review are presented in Section 11 and III.

SUMMARY

OF INSPECTION AND REVIEW ACTIVITIES

SUMMARY

OF INSPECTION AND REVIEW ACTIVITIES This section provides a brief summary of construction inspection and review activities at Comanche Peak.

It is being provided to help in understanding the results in this report. Only those inspection and review activities that have a bearing on the accomplishment of the current construction inspection program (IE MC 2515) are identified in this Section.

It should be recognized that there are a number of inspection and review activities that have occurred regarding Comanche Peak which are not addressed in this report.

For example,.

the numerous NRC inspection and review activities related to plant design are not ad Mssed.

The following is a listing and general description of five significant inspection and review activities conducted at Comanche Peak:

1.

Region IV Inspections Region IV has the responsibility to implement the NRC construction phase inspection program (IE MC 2512) at Comanche Peak.

These inspections are conducted by the onsite resident inspector (s) and Region IV based inspectors. The NRC construction phase inspection program begins following issuance of the LWA and/or the Construction Permit and continues until issuance of an Operating License.

The construction inspection program as specified in IE MC 2512 hos bee 1 20

a dynamic program.

IE NC 2512 was first issued in 1975 and has since been revised and updated on several occassions.

In addition to the above described program, a special onsite inspection effort was initiated at Comanche Peak by Region IV.

In June 1985, Region IV assigned several additional inspectors to the Comanche Peak site. These inspectors, who were supplemented by as many as 13 contractors, had the primary responsibility to monitor the CPRT program.

The special Region inspection program included witnessing of about 10% of the licensee re-inspections and 5% inde-pendent inspection of the areas subjected to licensee re-inspections.

The CPRT program is described in item 5 below.

2.

Construction Aporaisal Team (CAT) Inspection A CAT was conducted at Comanche Peak during January-February 1983.

The CAT is a multi-discipline team inspection which concentrates on hardware installation. The CAT also reviewed QC inspector effectiveness ano qualifications, overall QA program effectiveness,-

and design change control. The inspection team consisted of six qualified NRC inspectors from IE Headquarters and four consultants.

The inspection effort entailed about 1600 staff-hours.

Special Review Team (SRT) Inspection l

This inspection was conducted to evaluate the implementation of the licensee's management control of the construction, inspection and test programs and also to evaluate the extent of problems associated 21

with design documentation and construction deficiencies:. The SRT Wds Composed of nine qualified NRC inspectors from Region II which conducted the field inspection in April 1984.

The inspection effort entailed about 640 staff-hours.

Technical Review Team (TRT) Ir.spections and Reviews This effort, which consisted of a series of inspections and reviews, covered the technical disciplines of electrical and instrumentation

test programs, civil / structural, protective coatings, mechanical /

piping, and quality assurance. The TRT was composed of over 50 technical experts drawn from NRR and the Regions, national labora-tories, and consulting organizations.

The TRT conducted most of its onsite inspections during five, two-week sessions from July through l

October 1984.

It is estimated that the on-site inspection effort involved about 20,000 stoff hours.

5.

Comanche Peak Response Team (CPRT)

The CPRT represents a licensee corrective action plan (in response to concerns raised regarding the construction and construction process at Comanche Peak) covering both design and construction at both Unit 1 and Unit 2.

The plan was reviewed and approved by the CPPO. The plan includes licensee inspections to resolve specific concerns and a separate effort involving a comprehensive re-inspection of completed hardware and documentation review of those hardware attributes that j

1 are either inaccessible (like Reinforcing steel) or non-reCredtable (like a PT of a weld root pass).

This licensee re-inspection effort 22

Wds accomplished from August 1985 through December 1986, by, among others, about 150 ANSI N45.2.6 qualified third-party QC inspectors.

TUGC0 Quality Assurance Program Inspection A special inspection of the Texas Utilities Generating Company (TUGCO)

Quality Assurance Program was performed in late 1985 to detemine if the licensee's program meets NRC requirements. This effort consisted of an item-by-item comparison of TUGCO's QA Program with past and present NRC Standard Review Plan requirements.

It was performed by three qualified QA inspectors from IE expending about two and one-half staff months of effort.

Cable Tray and Hangers Inspection A special NRC inspection was performed to evaluate the conformance of Comanche Peak Unit 1 as-built cable trays and supports with drawings.

This inspection encompassed about 220 staff-hours in November 1985 and was conducted by one NRR engineer and three consultants who were experienced in structural engineering. The team inspected 32 cable tray supports.

3.

HVAC and Supports - Inspection 1

A specici NRC inspection was conducted to evaluate the conformance of Comanche Peak as-built HVAC support systems with installation drawings.

This inspection was conducted by four NRC consultants in February 1986.

It encompassed 32 HVAC supports and 250 staff-hours.

CPRT OA Program Review I

A deteiled review was made of the CPRT Program Plan quality assurance l

I program to assure that all elements of the CPRT-PP met applicable NRC requirements and licensee commitments. The CPRT-PP is an extensive corrective action program developed by the licensee with review and concurrence by NRC CPPO. A re. view and evaluation of this QA program is contained in Section 4.0 of SSER 13.

This review was conducted by 3 NRC QA inspectors and reviewers and took 2 staff months, mostly in December 1985.
